6 Disability Bedfordshire Acquired Injury Service Bedfordshire Acquired Injury Service The Acquired Injury (ABI) Service has been developed to enable access to specialist rehabilitation for adults with complex needs following an acquired brain injury, whilst ensuring other health, psychological and social care needs are addressed. The service aims to provide rehabilitation to enable individuals to achieve their optimum recovery and potential following a severe acquired brain injury and associated physical disability where present. Episodes of rehabilitation can be extended across the recovery pathway as individual needs change. The Acquired Injury Service is a county wide service offering timely, person centred rehabilitation from the onset of injury onwards through the recovery pathway. The service is offered to individuals whose needs are complex and cannot be met by local services. This is achieved through assessment and identification of clinical need. Service is delivered at two levels: - 1) Case management, linking in with commissioning functions. Placement with a wide range of providers is facilitated and managed by the team on behalf of NHS Bedfordshire. 2) Community rehabilitation, where possible episodes of rehabilitation closer to home are provided including multi-disciplinary interventions and co-ordination with other support services Exclusions from the service include people who are acute medically unstable, have a currently unmanaged psychiatric disorder or who are engaging in the misuse of drugs or alcohol. Exclusion also includes congenital disorders, progressive conditions and learning disabilities (as defined by IQ or onset of ABI during childhood). Headway Bedford provides continued rehabilitation/enablement for clients living at home, where they may have received a period of rehabilitation from visiting health professionals, and are now ready to engage with activities outside home. The model delivers a transition service between home and the rest of the community during which there is increased emphasis on personal choice; encouraging confidence and developing independence. The active presence of the many different types of people at the centre - staff, volunteers, other clients enables practise of skills learned in a safe environment, guided and supplemented by professional therapists and key workers. While attending Headway, clients will be encouraged, supported and enabled to engage in activities which help them keep focussed on increasing their ability to function purposefully in the rest of the community. The service is a blend of services delivered by health professionals (OT, speech therapy, physiotherapy) and other staff and volunteers with a variety of backgrounds and experience. This provides clients with access to a range of diverse services under one roof to enable their recovery. It includes an educational focus which allows clients to learn new skills, including introduction to leisure opportunities such as digital photography, art, pottery and cookery. Must be capable of making progress and not have behaviour which is a danger to themselves or others. Bedfordshire 5

9 Disability Park House - Bedford (Christchurch Group) Park House Park House delivers assessment and rehabilitation for adults with a spectrum of neurological conditions, in particular acquired brain injury. Provides longer term support for individuals with physical disability as a result of complex neurological conditions who require on-going rehabilitation to maintain their level of functioning. The full multidisciplinary team works collaboratively to regulate and reduce episodes of challenging behaviours; and supports residents to develop emotional and social information processing strategies to enhance techniques for effective behaviour change. Complex physical conditions associated with injury are also carefully monitored and treated effectively. Park House offers a structured care pathway to enable us to support individuals throughout each stage of their rehabilitation. There is a range of accommodation to support each resident s level of physical ability and independence, including individual en-suite bedrooms, transitional bedsits and an independent living flat. Park House provides a wide range of rehabilitation programmes, including: cognitive rehabilitation, physical rehabilitation, individualised neuro-behavioural programmes, daily living skills, vocational rehabilitation, support in expanding educational opportunities, improving and enhancing social skills, positive risk management, improving communication skills, enhancing psychological well-being including emotional adjustment, financial management and budgeting. We provide a range of therapies including Physio, OT, SLT, Hydro, Psychology, and Medical alongside our daily activities, nursing, and care services. Askham Place is a home for young people with Long term neurological conditions causing physical disability, who seek to maintain as much function as possible as their condition evolves. Askham Court and Askham Place are equipped, staffed, skilled and trained to work with patients whose primary needs are for highly complex physical care, including tracheostomy/ventilation, very complex spasticity, prolonged disorders of consciousness, and who require specialist medical and complex therapy interventions. Also patients whose primary needs are for moderately severe cognitive and behavioural management and rehabilitation. These include moderately severe challenging behaviours that present moderate risk, those requiring close monitoring and part time 1:1 supervision, who present with neuropsychiatric issues and require the design and delivery of structured rehabilitation or management programmes. Askham Court and Askham Place are not secure units so if severe challenging behaviour is deemed to be a risk to other residents we would not admit. 13 Paediatric Cambridge Centre for Paediatric Neuropsychological Rehabilitation The Cambridge Centre for Paediatric Neuropsychological Rehabilitation Specialist assessment and neuropsychological rehabilitation for children with an acquired, non-progressive brain injury as a result of an accident, injury, illness or other condition. They may struggle with memory, concentration, speech, emotion, behaviour, learning, friendships etc. The out-patient, community-based specialist team is trained to support children and families, including adjustment to the consequences of brain injury. The service: adapts to meet every child's specific needs to help them achieve the goals that are most important to them provides a specialist assessment of learning, communication, social, practical and thinking skills helps everyone to understand the often confusing changes that can occur after brain injury is available to school staff and others, to support children and young people in managing specific challenges provides the vital link between hospitals or specialist in-patient rehabilitation centres, and community and school support services recommends how to practise specific thinking skills and strategies for managing tasks or activities provides training packages aims to help everyone to make the most of their own skills and resources. Cambridgeshire Community Services NHS Trust City Care Centre, Thorpe Road, Peterborough PE3 6DB Contact Telephone Number ough/injury/tabid/1484/language/en-us/default.aspx Dr Lizzie Doherr, Chartered Clinical Neuropsychologist ccscommunications@ccs.nhs.uk NHS High Level Service Description A small interdisciplinary team working with people who have suffered a traumatic brain injury or other non-progressive neurological conditions such as meningitis, encephalitis etc. The team looks at the impact of the condition on people's everyday life in terms of physical disability, cognitive and communication problems, assessing their emotional adjustment and vocational support if they are employed. The team works closely with the local GP's and has links with local hospitals. Discharge occurs following negotiation with the client once their goals have been achieved. Anyone under the age of 16, individuals with significant alcohol and/or drug use issues, individuals unable to actively engage in the rehabilitation process. Greater Peterborough Appropriate referrals accepted from any source, including self-referral to the team. 12

Also for patients being discharged from hospital where they need urgent follow up to continue specialised rehabilitation. P2 (High Priority)- to be seen within 3-12 working days due to a sudden change or reduction in their functional ability and they may improve quickly with specialised therapy intervention. P3 (Routine) - non-urgent, usually seen within 4 18 weeks: Clients who are coping but are likely to benefit from specialised therapeutic intervention to improve independence and maintain their quality of life. 18 Disability Evelyn Community Head Injury Service Evelyn Community Head Injury Service Community Neurorehabilitation for adults after traumatic brain injury Cambridge Community Services The Old Chapel and Davison House, Brookfields Hospital, 351 Mill Road, Cambridge, CB1 3DF Contact Telephone Number Part of CCS on: Dr Judith Allanson, Evelyn Consultant in Neuro-Rehabilitation NHS The service aims to help people make as good a recovery as possible after a head Injury, by offering them information, advice, support, specialised assessment and rehabilitation interventions, using the following: a. individual holistic assessments to define client centred, goal directed, bespoke rehabilitation plans b. one-to-one therapy sessions in NHS settings or at home or work, as appropriate c. client groups, run weekly for 5-10 weeks each ( e.g. on brain injury information, fatigue management, cognitive strategies) d. co-ordination of multi-agency case conferences; e. working with families and carers ( including carer training and family workshops) f. close working with employers to support return to work g. providing a forum for multi-agency discussion of people presenting with complex situations after brain injury h. annual reviews. The clinical team includes a Consultant in Neurorehabilitation, a Clinical Psychologist, an Assistant Psychologist, a Speech & Language Therapist, and an Occupational Therapist. The team work very closely with Headway Cambridgeshire, the Oliver Zangwill Centre, Ely, the locality community rehabilitation teams throughout Cambridgeshire, and The Physical & Sensory Disability Team at the County Council. This is achieved through intensive residential assessment and treatment, specialist on-going rehabilitation, outreach and day programmes. The programmes are delivered by a full clinical team and a team of rehabilitation support workers. The clinical team disciplines are neuro-physiotherapy, occupational therapy, speech and language therapy, and clinical psychology. The rehabilitation programmes include direct therapy from the clinical team as needed. The clinical team have also developed group treatment programmes which service users attend if related to their goals, for example, understanding brain injury, memory group, and exercise group. Community access is a central focus of both programmes, where service users are supported to implement their treatment in community-based settings. Leisure-based groups also feature in the programme, for example, art, baking, computer skills, and service users also have goals to achieve within these groups. In addition to the structured support, Headway also provides social inclusion opportunities such as gardening, bowling clubs, pubs, youth groups and access to our tutor led sessions, for instance the gym and art. Clients have access to counselling and to Headway s own Art Therapist and also Occupational Therapist, supporting all clients towards more independence and self-management, wherever it is appropriate. Alongside the structured and less structured activities, Headway s staff are able to provide information, advice, emotional support and mentoring for clients as well as supporting their families and carers. An important element of Headway s remit is to provide Understanding Injury workshops and other training opportunities for all groups including clients, families, schools, probation and police services, raising awareness about brain injury. The desired outcomes for all clients are personal progression, reablement, social inclusion and also friendship. For people post stroke there is also a family support service with psychology in-put. The team also has close working links with the Evelyn Community Head Injury Service and Oliver Zangwill Centre. The team work on an out-patient and domiciliary basis depending on the goal of the client. This may include working in one-to-one sessions or in groups. We also link with other agencies to facilitate activities in the community, were possible, and develop social networks. We provide fast, professional and independent assessments for the rehabilitation needs of injured adults of working age. Our interdisciplinary team of professionals work to support the client to aid recovery, increase independence and to consider vocational options. The service that is suited will be goal-orientated and be dependent on the severity of the injury, the amount of rehabilitation or recovery already made and the service that fits client s personal circumstances. Patients may be admitted and their needs met informally, under DoLS or the Mental Health Act. Underlying all care, there remains a clear focus on privacy, dignity and quality of life. Patient care is managed by a full MDT, including the local GP, RMNs and RGNs, and a range of therapeutic disciplines.
