1 Primary Health-care, Adult Services and Children Services in Westminster In Westminster over 4,000 people receive Supporting People services provided by 44 different organisations. The Supporting People programme aims to improve the quality and life for a diverse range of vulnerable adults and young people in Westminster who at times may need to access Primary Care, Adult and Children Services in Westminster. A conference was held on 8 th May to discuss issues around accessing the range of statutory and healthcare services by providers of housing related support and this contact list has been designed to assist housing related support service providers to access social care services and the range of primary health-care services in Westminster. Also included are details of Primary Care Trust services discussed at the conference, in particular the Westminster Rehabilitation and Rapid Response Service. There are a range of services across Westminster and are generally divided into North and South with access to these services covered by geographical areas. This information should be made accessible for front-line workers to ensure referrals can be made to services where a tenant s health deteriorates or where advice is sought to meet the needs of vulnerable people in Westminster.
