1 Information & Directions This year we are going to give back to the community and really make a difference! Each student or group of students will decide on a project, complete the project (taking pictures along the way), and share their project with the class. Below you will find all of the information you need to complete your Community Service Project. 1) Choose a service project. 2) Have your parents and your teacher sign the Community Service Project form. 3) Think of a name for your organization. For example, if you were collecting coats for people, you could call your organization Global Warming, Inc. 4) Design a logo for your organization. We will do this in class, but you may want to start thinking of a logo now. 5) Set a goal for yourself. 6) Try to reach your goal! Do your project. 7) Keep a journal about what you are doing to help the community. Each day that you work, write about what you did. 8) Have someone take pictures of you as you complete your project. 9) Using a tri-fold presentation board, create a presentation describing your service project. You will use your journal and pictures for the report. 10) Give yourself a pat on the back! You are an extraordinary citizen! Return the signed portion of this paper to your teacher.... I have received and read the Service Project Information and Directions sheet. Parent Signature Student Signature

5 Final Presentation Congratulations on finishing your service project. You are an OUTSTANDING CITIZEN! I m sure you ve made a lot of people very happy! Now it s time to share your project with the world (and our class). On a tri-fold board, create a display to explain what you did to help the community. Add drawings, cut-outs and photos to jazz it up. You can write the information in your neatest handwriting or type it on the computer. What was/is your project? Why did you choose that project? What is the name of your organization? Why? Tell about what you did (every detail!). If you do this again, what will you do differently? How did helping the community make you feel?
