5 Evaluate your health with the Total Health Assessment Our online total health assessment tool helps you examine the many factors affecting your health and prioritize lifestyle changes that can help you live healthier. You can also save a summary of your results to your electronic health record so you can discuss next steps with your doctor. 1 Manage chronic pain with Care for Pain Everyone s pain and tolerance levels are different. 6 Tilapia with Chunky Tomato Sauce INGREDIENTS 2 (14-oz) cans no salt added diced tomatoes, drained ¼ cup chopped green onions, including green stems ½ tsp. sugar (optional) 1 Tbsp. finely chopped fresh basil (2 tsp. dried may be substituted) 2-3 cloves garlic, minced 2 tsp. fresh lemon juice Salt and freshly ground pepper to taste ¼ tsp. crushed red pepper (or to taste) 1 Tbsp. olive oil 1 lb. tilapia fillets 2 Tbsp. chopped fresh parsley DIRECTIONS Place all ingredients, except oil, fish and parsley in a bowl and gently combine well. In large skillet heat oil over medium high heat. Place fish in skillet. Top with tomato mixture, cover and simmer over medium low heat for 8-10 minutes or until fish is cooked. Garnish with parsley and serve. Makes 4 servings. Elegant Cheesy Spinach Cakes INGREDIENTS 12 oz. fresh baby spinach ½ cup part-skim ricotta cheese (low-fat cottage cheese may be substituted) ¼ cup shredded Romano (Parmesan cheese may be used) 3 large eggs (can substitute 2 large eggs, plus 2 egg whites, beaten) 2 cloves garlic, minced Freshly ground pepper to taste Canola oil cooking spray DIRECTIONS Preheat oven to 400 degrees. Use food processor and pulse spinach until it is finely chopped, but don't overdo it. Place spinach in bowl. Add cheeses, eggs, garlic and pepper. Stir to combine well. Coat 8 "cups" of a muffin pan (½ cup size)* with spray. Spoon in spinach mixture. Bake about 20 to 22 minutes. Remove pan from oven and let stand 6 or 7 minutes to allow spinach cakes to firm up. Loosen sides with knife and gently lift out. Kaiser Permanente s online cancer resource kp.org/ cancer is a one-stop shop for reliable, expert cancer information. Based on feedback from our cancer patients and oncologists, the site has recently been updated and expanded, making it an invaluable resource for those navigating a cancer diagnosis. At the newly updated kp.org/ cancer, you will find information about various types of cancer, clinical trials, and support to help you manage your health. You can also learn about strategies to help you cope with cancer and enhance your quality of life, including: easy-to-understand information about cancer, conventional and complementary cancer care, recurring and advanced cancers, eating well during treatment, coping with side effects, and support groups and online communities, including whatnext.com. In a recent survey, 82% of our members living with cancer would recommend kp.org/cancer to a friend or relative. If you or a loved one needs cancer information, turn to this great resource. 9 Asthma management Do you often have episodes of breathlessness, chest tightness, wheezing, and constant morning or evening coughing? If so, you could have asthma. The good news is that you can still live an active and healthy life with asthma, if it is controlled properly. You can take an active role in managing asthma by working with your doctor to develop an asthma action plan. The following are resources and tools you may use to help you learn more about asthma. 10 Behavioral health and mind/body health topics You can get treatment for mental illness, emotional disorders, drug abuse, and alcohol abuse without a referral from your personal doctor. You can arrange for services directly by calling our behavioral health access unit, where licensed behavioral health clinicians (social workers and nurses), and scheduling representatives can assist you in arranging for appropriate services. Also ask about any ongoing classes that could support you and your family. 11 Diabetes management InSTEP with Diabetes I & II Strive to be healthy and feel great living with diabetes. Learn skills for healthy living including nutrition/ food choices, physical activity, testing your blood sugar, emotional support and recommended care. All members with diabetes should attend both InSTEP I and II. InSTEP with Diabetes III the next step You must attend InSTEP 1 and InSTEP 2 before attending InSTEP 3. Having trouble managing your diabetes? Refresh your skills and knowledge of diabetes by taking this class. You can review nutrition, physical activity, pattern management, coping skills and other lifestyle skills learned from InSTEP 1 and 2. This class is a great opportunity to ask questions about your habits and learn how to make the right behavior changes. Want to know more about diabetes? 12 Heart failure management Successful living with heart failure If your doctor told you that you have heart failure, we encourage you to attend this class. You will learn what heart failure is; the causes of heart failure; how to treat it with medicine, healthy eating habits and lifestyle changes; heart failure and your emotional health; and how to know and act on symptoms when they happen. (Check with your provider if you are not sure of your heart failure diagnosis.) 15 Get 24/7 access to your child's health information at kp.org Imagine using your personal computer to review your child s vaccine information, your child s doctor or schedule a routine appointment. No matter how busy you are, we have tools to help you better manage the health of your kids. With kp.org s My Health Manger, you can safely check your child s* health information online when you have the time. If your child gets care at a Kaiser Permanente medical center, you can use kp.org s Act for a Family Member anytime to: your child s doctor View most lab test results View medicine information and refill prescriptions Make, cancel or review routine appointments or well child visits View and print a health summary for your child Check on the status of referrals Review ongoing health conditions Review past office visit information, including any follow up steps View immunizations and allergies medicine Check eligibility and benefits To use kp.org s Act for a Family Member feature, the parent or guardian must be a Kaiser Permanente member and registered at kp.org. To register, go to kp.org/registernow. You ll be asked a few questions to check your identity. Once your identity is confirmed, you will get immediate access to wide range of healthy living tools and My Health Manager features on kp.org. Or if you prefer, you can request that a temporary password be mailed to your home. Once you are registered on kp.org, you can Act for a Family Member for your child. To get started, go directly to the Act for a Family Member page at kp.org/ actforfamily. Follow the directions to add your child s account to your My Health Manager. You will be asked to provide some information about your child and agree to the terms that confirm you may make health decisions for your child. Once you are approved to access your child s information, you can easily view the personal health record of your child 24/7. If you have any problems or questions when you are registering, contact the kp.org Web Manager at Monday through Friday, 9 a.m. to 10 p.m. or Saturday and Sunday, 10 a.m. to 6 p.m. * This feature automatically ends on a child s thirteenth birthday. 16 Nutrition Living well with pre-diabetes Are you interested in learning about pre-diabetes or has your doctor said you may be at risk? Join our Living well with pre-diabetes class where you can learn ways to help prevent diabetes through lifestyle changes and blood sugar and lipid control. The class will talk about what is pre-diabetes, what puts you at risk, self-care behaviors, such as healthy eating, weight loss, and physical activity, and how to create an action plan for change. Nutrition consultation Do you need nutrition advice about a specific health concern? A nutritionist can help you modify your current diet so you can better manage your medical condition. A referral is required by your doctor and you may have a co-pay for the visit. You can also attend one of the nutrition classes, either online or in the classroom, without a referral. These services are open to Kaiser Permanente members only. Nutrition for cholesterol control Learn to control your cholesterol through healthy food choices. Nutrition for weight control Learn to achieve a reasonable body weight through sensible eating and regular activity. Call an appointment representative, Monday through Friday, 8:30 a.m. to 5 p.m. at (703) or , toll-free, to register for nutrition classes or to book a nutrition consultation appointment. Weight loss surgery information meeting Attend this meeting to learn more about weight loss surgery. You will have an opportunity to meet bariatric surgeon, Dr. Brian Cantor. If you are a candidate for weight loss surgery, you must attend three informational sessions. The sessions are offered every second Tuesday of each month at 6 p.m., at Holy Cross Hospital. To get more information or to register, please call (301) Weight loss surgery support group Ongoing support is the key to long-term success in the journey of weight loss surgery. You can benefit from this group whether you are still trying to decide if this option is right for you, are in the process of preparing for the surgery, or have had weight loss surgery and need motivation and encouragement to stay on track. Get support and advice from others who may be going through the same kinds of life challenges as you. All sessions are FREE to attend and do not require registration. 19 Chronic disease self-management Healthier Living: Managing ongoing health conditions workshops Become a lay leader for the Chronic Disease Self-Management Program, also known as Healthier Living: Managing ongoing health conditions, and help make a difference in people s lives. Are you, or someone you love, living with a chronic health condition? Would you like to learn skills to manage your ongoing health condition and get the most out of life? Healthier Living Workshops are for adults living with one or more ongoing health conditions. If you have a heart or lung condition, diabetes, multiple sclerosis, arthritis, or any other ongoing health condition and would like to help others learn how to take control of their health, consider being a volunteer leader for the Healthier Living: Managing ongoing health conditions workshops. Leaders help others to: Fight fatigue and frustration Communicate better with their health care team Manage stress Deal with depression Develop an exercise program Set goals and solve problems to make positive changes in their lives Workshops are great support for physical and mental health. Who can become a lay leader? Anyone who is willing to attend a leader s training and is able to lead at least one workshop a year may apply. Having a chronic condition is not a requirement to teach the workshop. The workshops meet for 6 consecutive weeks, 2 ½ hours each session, and are led by trained lay leaders, many who have a chronic health condition. The workshops are also for family members or caregivers. Contents of the classes will not conflict with any current treatment plan from the doctor.
