Repair Science Building 4th floor due to autoclave faulty water line. Belfor C /1/2012 $ 72, Several hundred gallons of water flooded rooms and halls to (3) floors. Repair failed soffit at Bookstore/Union Building. Soffit support failed to (50) foot section. Failed soffit section was removed and replaced. Failure Belfor C /14/2012 $ 17, due to worn bracket locks. Clean-up mud tracks from off-road vehicle damage; sweep parking lots Belfor C /13/2012 $ 23, and roadways for debris at Concord Campus. Roof replacement at the North Gym due to roof failure during rainstorm. Roof is over main gym floor; teams unable to safely practice; destruction Graham Prewett /11/2012 $ 106, of costly-to-repair/replace hardwood court flooring. Elevator became inoperable during final weeks of spring semester which required elevator tech to run for emergency purposes only. ADA Access is mandatory for all buildings. This elevator is the only ADA access to the upper floor of the building. Emergency repairs and elevator tech made to Excalibur Elevator /15/2012 $ 85, meet ADA requirements. Roof repair/partial replacement. Rotted roof, water/rain damage, Lab School Theater Roof TearOff Project. Davis Moreno Construction /20/2012 $ New dresser couplings and foam insulation after hot water line break flooded Engineering West/Industrial Technology Building areas with hot 5, water. Davis Moreno Construction /20/2012 $ Transite water line on north side of Joyal Bldg damaged due to tree roots, causing flooding. Repair, replace with PVC water line. (Transite is an asbestos-cement product that was discontinued in the 1980's. Since it is no longer manufactured, and is considered a hazardous material, the entire run of pipe had to be replaced rather than just a single piece 7, replaced or repaired.) A number of trees fell due to age and heavy rain into the student housing parking lot just before fall session began, with the threat of more possibly falling if not addressed. Tree trimming and removal of approximately Redwood trees from the slope on the west side of Cypress Residence Hall to the west end of JGC parking lot and up to Sunset Coastal Tree Service /27/2012 $ 48, parking lot. Major storm drain inlet near the LK Wood pedestrian underpass that crossed HSU property, with the point that failed entering CalTrans property adjacent to the freeway. If it was not repaired immediately before any heavy rains, it would have caused a landslide onto the G R Sundberg, Inc /4/2012 $ 12, freeway. Aggreko /17/2012 $ 24, Repair 2500 Amp Generator at Physical Sciences. Hampton Tedder Electroc Co /17/2012 $ 11, Repair power outage at Physical Science 1st and 2nd floor. Clean up water at the Luckman Studio Luckman flooded due to a heavy Emergency Service Restoration /12/2012 $ 6, rainstorm and a roof failure. Two storms and age caused severe damage to boat docks, threatening accessibility, safety and security. Removed damaged/destroyed pile guides. Most of the North dock pile guides had failed entirely. Both the eastern-most and western-most piles of the North dock had worked loose North Coast Divers /12/2012 $ 26, enough to potentially destabilize the entire dock. Emergency modifications to gangway to ensure safety. The gangway from the causeway to the North dock was visibily distorted and the deck North Coast Divers /2/2012 $ 61, was showing signs of buckling and torsion damage. Repair materials for the work performed by North Coast (the only provider Bellingham /5/2012 $ 16, of marine materials with the items in stock). To assist North Coast in their effort to repair the dock in the stormy conditions, Bellingham brought in their available personnel to get the job Bellingham /6/2012 $ 7, done. The North dock and the gangway were in danger of being lost. Services to install gangway ramp. Building interior temperatures reached 80+ degree mark and had the potential to continue heating up. This building is a medical facility which falls under OSHPD regulations. Health and Safety, required immediate action. Reclaimed Pump Station, Primary No. 3 Booster Pump failed caused by badly worn and misaligned shaft assembly rotation, which created high amperage overload motor short out. Without this primary booster pump the Reclaimed Pump Station cannot build reservior storage and supply the campus demand. Hot temperatures threatened university landscaping and agriculture fields and livestock. Immediate remedial measures required to protect property and/or the public's health, safety and welfare. Hampton Tedder /11/2012 $ Restore power lost to High Voltage Feeder 3 at the Electrical Substation Building (#191). Electrical power required to maintain fire and life safety systems per the State Fire Marshal. Fumehoods, exhaust fans and ventilation required per OSHA. Sump pumps are required to prevent water damage; and refrigeration required for instructional labs and perishable items. Outage at Kellogg West, impact to Biotechnology Building (#4) and Kellogg West Buildings (#76 and 77). Health and 80, Safety, required immediate action. Replace (1) 9-ton compressor of the existing air handler unit located at Palm Desert Campus in the Theater Building; AC unit cools audience portion of building, extreme heat; if not replaced shows would have been Couts Heating & Cooling, Inc. SBCMP /8/2012 $ 6, cancelled. Hot water leak in Apartment #5106 of Building 5, Student Housing Unit required plumbing repairs; concrete flooring broken to repair point of leak; Loma Linda Plumbing SBCMP /17/2012 $ 5, carpet replaced as well due to water damage. Repair circuit board, replace filter controller, install flow meter sensor, solenoid assembly and flow switch in heater bypass of campus pool. The chemical controller for the pool went out and needed replacement. It was deemed a Health and Safety issue and could not hold classes or team Knorr System SBCMP /25/2012 $ 6, sports. Repair grounded compressor of AC Unit #4 on the roof of the Oliphant Couts Heating & Cooling, Inc. SBCMP /14/2012 $ 8, Building at Palm Desert Campus. Replace epoxy flooring, base and installation in Health and Physical Education locker room due to damage sustained from UCLA Bruins team during football camp held on campus. Damage caused a Health and Safety issue. The condition became unsanitary and unusable. Regular DFS Flooring SBCMP /6/2012 $ 49, academic class schedules were pending the repair. Repair and replace AC Unit at Palm Desert Campus due to theft of unit Couts Heathing & Cooling, Inc. SBCMP /25/2012 $ 7, located on the Cenic Building. 3/22/2013 Page 3
