Include participation in Fine Arts, etc. It is strongly suggested all students write a short essay, outlining accomplishments, future goals, community service, and planned area of study. You may also talk about why you need financial assistance. Include any information that will help the reader get to know you. Include this with the application form as it gives organizations who choose the recipient(s), a snapshot picture of the applicant. You only have to complete one general application form to be considered for all awards; however CUPE, THE ROYAL CANADIAN LEGION and the RUTH HARVEY AWARD require additional applications. 8 STATEMENT OF NEED If you are attending school out of town, it is not unreasonable to expect to spend a minimum of $15,000 for your First year. Below, briefly describe any circumstances regarding financial need of which we should be aware. Example: You have another brother or sister in university. The information provided here will be strictly confidential. It is strongly recommended that you include an introductory essay discussing who you are, what you plan on doing, School achievements, extracurricular activities, community involvement, and any extenuating circumstances for financial need. The essay will be copied and included with your application when given to sponsors. The essay is sometimes the deciding factor. You must complete this application and make an appointment (very brief) with Ms. Offutt, Ms. Bryant or Mr. Russell to go over this information to make sure it is complete when you hand it in. NOTES: a) Students need to be aware that if they receive an award administered by Prince Rupert School District #52, they will be required to submit their Social Insurance Number with their proof of registration. This is so award payments are reported on the T4A. b) By signing this application, I am also giving the awards coordinator permission to photocopy my application and provide general grade point averages for sponsors who request it for selection purposes. 9 CHSS 2015 SCHOLARSHIP & BURSARY APPLICATION PROCEDURE The CHSS 2014 Scholarship & Bursary Booklet will be available online at under Grad info by April 10, It is designed to present the financial awards made available by a number of Prince Rupert organizations and service clubs to graduating students who are planning to attend a postsecondary institution. These organizations are to be thanked for their generosity and for their recognition of the real need that exists for all students who embark on further education. The following page of this booklet provides some indication of costs that may be incurred at a post-secondary institution. These figures have been provided by your counsellor and are intended as a guide only. Check post-secondary websites for updated costs. The financial awards described in this booklet have been divided into three categories, as follows: Awards: Bursaries: Scholarships: presented upon fulfilment of specific criteria as outlined in each award description. presented on the basis of fulfilment of criteria and demonstrated financial need. presented on the basis of academic achievement or combined academic, athletic and citizenship achievements. Students applying for bursaries must present proof of financial need (statement of need). Students applying for scholarships must present a transcript of secondary grades as well as a short written description of their athletic and/or leadership qualities when these are specified in the scholarship applied for. 1) All students wanting to be considered for awards must fill out the application form with supporting documents and attach a list of awards for which they are applying. 2) Please make sure to set a time to see Ms. Offutt, Ms. Bryant or Mr. Russell in order to review and clarify all information on your application. 3) Deadline for application is April 29, ) Scholarship, bursary and award recipients will be determined by the Scholarships and Awards Committee or some organizations will choose their recipients and notify the school. 9

15 PRINCE RUPERT BETA SIGMA PHI The Beta Sigma Phi Sorority of Prince Rupert proposes to provide bursaries of $ to high school graduates of Charles Hays Secondary School during the spring convocation. The following are the criteria upon which this award may be granted: 1. The candidate(s) must be a graduate of Charles Hays Secondary School. 2. The candidates must be a daughter or son or legal ward of an "active member in good standing" of a Prince Rupert Beta Sigma Phi Chapter for at least three consecutive years immediately prior to the bursary application. Proof of legal guardianship may be required. 3. The candidates must be proceeding to a recognized University, College or Technical School. 4. The bursary must be applied for by November two years following graduation (eg. graduated in June 2014, bursary must be applied for by November 2015). 5. Confirmation of registration in a recognized Learning Institution (see 3 above) for a minimum of 30 days is required from the institution before monies will be distributed. 6. The bursary will be distributed within 30 days of verification of registration as outlined in Criteria #5. 7. Eligible students may apply for this award by completing and submitting the general application form provided in this booklet. 8. A cover letter is to be submitted along with the general application form. 9. Recipients will be verified by a Beta Sigma Phi bursary committee. NOT OFFERED THIS YEAR 15

16 Bob Quast Memorial Award The Bob Quast Memorial Award has been established to provide an award of $ annually to the successful candidate. Criteria: 1) The candidate must be a graduate of Charles Hays Secondary School. 2) The candidate must have enrolled in a post-secondary institution and the award will be provided upon receipt of registration at a recognized university, college, technical school, or emergency services academy (firefighter, police, paramedic). 3) This award must be used within 24 months of graduation. 4) The candidate must have achieved a C+ or higher in every grade 11 & 12 course in which they were enrolled (candidates for this award are disqualified if they have dropped any grade 11 or 12 course more than halfway through its completion). 5) The candidate must be strongly dedicated to at least one extracurricular activity. 6) The candidate must possess an exemplary record with respect to attendance and punctuality. 7) The candidate must display excellent citizenship at the school and/or community level. 16

17 FILIPINO-CANADIAN ASSOCIATION OF PRINCE RUPERT The Filipino Canadian Association of Prince Rupert will provide $ in award money to a student or students (divided equally if more than one is eligible) graduating from Charles Hays Secondary School. The following conditions must be met before this award may be granted: 1. Preference will be given to the son, daughter, or dependent of a member in good standing and active in the association and with the residency of three consecutive years. 2. The candidate/candidates must have enrolled in a post-secondary institution and submit a copy of the registration and receipt of tuition to the Association within a year. 3. The school officials will recommend students and the Board of Directors of the Association will decide on the recipient/recipients based on the pre-set criteria. 17

18 HOSPITAL EMPLOYEE S UNION AWARD The Prince Rupert Local of the Hospital Employee s Union is sponsoring a $ award at Charles Hays Secondary School. Criteria: 1. Preference will be given to dependents of H.E.U. members. If no dependent applies, then graduating students entering post-secondary training in the health care field will be considered. 2. The candidate must be a graduate of Charles Hays Secondary School or Pacific Coast School. 3. Payment of this award will be made upon confirmation of enrollment and proof of registration in a post-secondary institution. 4. This award must be used within twelve months of graduation. 5. The Prince Rupert Local of the Hospital Employee s Union will be involved in the selection process. 6. This award must be used within 24 months. Eligible students may apply for this award by completing and submitting the general application form available with this booklet. 18

19 KNIGHTS OF COLUMBUS COUNCIL 5282 Knights of Columbus Council 5282 wish to advise that they will award two bursaries to the graduates of Charles Hays Secondary School for Two - $500 bursaries (five hundred dollars) Pre-requisites 1. Applicant will graduate from Charles Hays Secondary School in Prince Rupert. 2. Successful graduation from Grade Applicants will continue their education in a recognized post-secondary institution. Classification Criteria 1. Evidence of service to local community (1-10 points) 2. Priority to families of the Knights of Columbus in good standing (5 points) 3. Priority is given to members of the Annunciation Parish, Prince Rupert (5 points) 4. Academic achievement. (1-10 points) 5. A 300-word essay outlining personal career goals and two personal references (including at least one teacher) - (1-10 points) 6. Extraordinary hardship due to lack of financial support will be considered Conditions 1. The Bursary scholarships to be transferred in the name of the successful applicant warranted upon completion of the first term of post-secondary study. 2. 20 LOYAL ORDER OF MOOSE #1051 The Loyal Order of Moose of Prince Rupert proposes to provide four awards of $ each. These awards are the "Michael Budinick Memorial Award" and the "Mel Mathews Award". The following are the criteria upon which these awards may be granted: 1. The candidate must be a graduate of Charles HayS Secondary School or Pacific Coast School. 2. The candidate must have exhibited good citizenship within both the school and the community. 3. Economic need will be considered if all other factors are deemed equal. 4. The candidate must have enrolled in a post secondary institution, preferably college or university. 5. Preference will be given to the son or daughter of a member of the Loyal Order of Moose. 6. The recipient of the Mel Mathews award must be involved in school or community athletics. 7. The money will be disbursed for the second semester of course work once proof of registration is supplied. Eligible students may apply for this award by completing and submitting the general application form provided in this booklet. 20

21 MAKING HEALTHY CHOICES - AWARDS for DRUG-FREE YOUTH Community Prevention Education Continuum (CPEC) and the Prince Rupert Sikh Missionary Society have joined to provide $1000 in award money to two Grade 12 graduating students ($500 each) in the Prince Rupert area who have made the decision to be drug, tobacco and alcohol free during their high school years. CPEC is designed as a sustainable community-lead, police assisted, comprehensive youth drug prevention strategy (da.bc.rcmp.ca). The smart and healthy choice to avoid the use of illegal substances, focus on academics and healthy school and community activities is a choice worthy of commendation. To be eligible for these awards, students must meet the following criteria: 1. Have not used alcohol, tobacco or illegal drugs during their lifetime; 2. Write a one page essay (type written and double spaced) that describes specifically how you have lived and modeled a drug free life; why you made the choice to be alcohol and drug free; and what role youth can play in drug free prevention efforts; 3. List in detail specific drug prevention activities you have been involved in and list your extracurricular activities both in school and in your community; 4. Complete the general application included in this awards booklet which specifies post-secondary schools you have applied to, program of study, and current contact information for you and your parents/guardians; 5. Include three references (one is a parent/guardian) who can affirm your drug free lifestyle and Healthy choices as stated on your declaration of being drug free. (Statement of Support on page 8); 6. Provide proof of registration in a post secondary school, have 30 days of full time attendance at a post secondary school, and a tuition receipt before the award money will be disbursed; 7. 23 MICHAEL TOMESCU MEMORIAL AWARD An award in the amount of $ is being given in memory of Michael Tomescu. The following criteria must be met before this award is given: 1) The candidate must be a graduate of Charles Hays Secondary Secondary School or Pacific Coast School. 2) Preference will be given to a student who is planning to pursue a degree in Political Science. 3) Payment will be made upon proof of registration in the program. Eligible students may apply for this award by completing the application form provided in this booklet. 23

24 THE ROGER OBATA ACHIEVEMENT AWARD ( ) The Friends of Roger Obata Committee (Toronto, Ontario), is pleased to sponsor one award of $ , to be given for the next ten years to one CHSS graduating student. Different recipients are to be selected each year if the criteria is met. Mr. Obata, a Canadian of Japanese descent, was born in Prince George, British Columbia, in 1915 and was raised in Prince Rupert. He never forgot his early education and friendships made and often mentioned them in his memoirs, interviews and conversations. Mr. Obata, a successful businessman, received a degree in engineering from the University of British Columbia, and served with the Canadian Army during the Second World War. He is known for his outstanding contributions to the Japanese Canadian community over a period of 50 years, especially in the area of human rights. He was a valued member of the strategy team that signed the historic Redress Agreement with the Government of Canada in Mr. Obata was named to the Order of Canada in 1990 for his human rights activities, and was an example for all Canadians who love justice and freedom. Criteria 1. Recipients must be a CHSS graduating student. 2. Student must provided evidence of outstanding service to school and/or community. Human rights advocacy/social action may strengthen the application. 3. In addition, students who have overcome personal obstacles and achieved graduation through perseverance may be considered. A brief written statement explaining what obstacles and how the student has persevered must accompany the application. 4. The recipient must provide proof of acceptance at an accredited post-secondary school before money will be disbursed. The proof should be a tuition receipt and a timetable. 36 Prince Rupert Chapter Canadian Parents for French-Prince Rupert Chapter (CPF-PRC) provides bursaries for School District #52 Grade 12 French Immersion student graduates and also students who have successfully completed Core French 12, and who are entering post-secondary university, college, trade, vocational or technical education. BURSARY AMOUNT: dependent on the available funds and on the number of applicants. The maximum is $500 per student. Candidates must: 1. Currently be a member or a dependant of a current member of Canadian Parents for French (CPF) prior to bursary application. Length of CPF membership and contribution to CPF activities will be factors in prioritizing recipients for available funds. 2. Be a Grade 12 French Immersion Program graduate, or have completed Core French 12, in School District # Complete and submit the general bursary application form in the School District #52 graduation booklet in the designated month of your Grade 12-graduation year. 4. Provide a covering letter stating why you should be considered for a bursary, including your goals and aspirations and how you have worked to promote the French language within the community. Demonstrate financial need in your cover letter. 5. The selected student will have taken one or more of the following courses: Drama 9 and/or 10 Theatre Performance 11 and/or 12 Directing & Scriptwriting 12 Peer Tutoring (in a drama class) The selected student will have participated in two or more of the following: The school musical/major fall production Drama festival Fine arts revue Community theatre This award will be given to a student who plans to continue his/her education in a post-secondary institution (including college, university, performing arts school, technical or trade school) within two years of graduation, and who is likely to continue to be involved in theatre in the future. The recipient will be chosen by the drama teacher/theatre director at Charles Hays Secondary School. In the event that two students are chosen, the award can be shared. *This award is a part of the in-district award process. 39
