1 Mrs. Amy Denney, Principal Mrs. Kim O Shea Asst. Principal April 9, 2018 Read-A-Thon!!! Thank you for supporting our Read-A-Thon, which supports our school s web based literacy resource, iready. Students are able to utilize iready at home to support literacy growth and development in all areas of reading and math. Our Read-A-Thon supports our desire to encourage healthy reading and math habits at home each day and will help to sustain our use of iready into next school year. Reading just minutes each night at home promotes strong fluency, vocabulary, and comprehension skills, which will help your child develop into a strong reader. Successful Kindergarten & TK Registrations We welcomed over 110 incoming kindergarten students and 40 incoming TK students at our registration day last week. It was great to meet all of our new Prairie Dogs and welcome them. If you were unable to attend our registration and have a student who will be 5 years old by September 15, please call the school at to get them registered. Violent Intruder Preparedness We will be refreshing our skills for the threat of a violent intruder in our schools. The Sioux City Community School District is working with Chad Sheehan to implement the S.A.V.E. Yourself strategies. All teachers will be discussing these strategies with our students this week and we will have another drill next week. Just as we practice for fire and tornado emergencies, we must also practice for this type of emergency. Final Weeks of School There are just a little over seven weeks remaining in our school year. We find each day to be filled with learning and ensuring that each student has learned the necessary standards to be able to succeed at the next grade level. Please continue to encourage your students to do their best work, demonstrate positive behavior and make sure they are getting adequate sleep each night. Iowa Assessments We want to thank you for all you did to help your child in grades 2-5 do their best on the Iowa Assessments. We are confident that the students did their best and we look forward to receiving the results in May. APRIL 10 ~ 2nd Gr. to Opera Ia. 1st & 2nd Gr. 2 Nondiscrimination Statement Time To Start Planning We will soon be in the process of scheduling our students into their classrooms for next school year at Perry Creek Elementary. Please let us know if you have you moved or are you planning to move from the Perry Creek district? If you have moved from our district & would like to keep your child at Perry Creek you will just need to request an In District Transfer Request Form from the school office & return it as soon as possible. If you are planning on sending your child to another school in the district, please call the Barb in the office and let her know. SCHOOL SAFETY MEETINGS Join us for an in-depth discussion about school safety. In this meeting, you will learn how staff and students are trained for crisis, what precautions we take to try and prevent crisis, and the everyday safety practices currently in place. The first meeting will be held on Tuesday, April 10 at 6 pm in the North High Auditorium. The second meeting will be held on Thursday, April 12 at 4 pm in the West High Auditorium. The final meeting will be held on Thursday, April 12 at 7 pm in the East High Auditorium. Please join us for one of these three meetings. SNOW MAKE-UP DAYS Because of the snow days we have had, we will be having school on April 27 & May 18. Our last day of school will now be May 31. A Message from our P. E. Department This year the Sioux City Physical Education teachers are supporting the Live Healthy Iowa Kids track and field championship instead of Sioux City Relays. Unlike SCR this event is free to spectators, and has much more to offer. There are different running events that include relay teams, and field events like long jump. We hope this will be much more engaging and a better use of our families time. It also includes our younger students the ages run from age 7-14, and they have the opportunity to qualify for a state championship track competition. Follow the directions to register your student athlete today. 1. Go to 2. Track Championships 3. Find Competition 4. Scroll down to Sioux City and click Here to register, your child must be registered before the day of the meet to compete. We are still in need of volunteers for this year s field day. Perry Creek s will be on Friday, May 11. There are 2 sessions, the first is from 9:00-11:30 for the 3rd 5th grade classes, and the second is from 12:30-3:00 for the 1st, & 2nd grade classes. Clark s will be on Friday May 25 from 9:00 11:00. You may sign up by ing Mrs. Hensley at hensleh@live.siouxcityschools.com We will take help from any family member that would like to come and play some fun games with the kids for a few hours or the whole day! 4 STATE OF IOWA HEALTH REQUIREMENTS Kindergarten and 3 rd Grade Students According to Iowa law, all children enrolling in kindergarten and 3 rd grade are required to evidence of a vision screening no earlier than 1 year prior to enrollment and no more than 6 months after the date of your student s enrollment. Vision screening forms are available through eye care professional, from your school nurse, or at the following website, userfiles/88/cert%20of%20vision%20scrndec2015%20wfacts% 20Fillable_FINAL.pdf 7 th and 12 th Grade Immunization Requirements Iowa Immunization law requires that all students entering 7 th grade have both a Meningococcal vaccination and a Tdap (Tetanus, Diphtheria, and Pertussis/whooping cough) vaccination before school starts in the fall. Iowa law requires that all students entering 12 th grade have a Meningococcal vaccination. The requirement is 2 doses of meningococcal (A,C,W,Y) vaccine for students in grade 12, if born after September 15, 1999; or 1 dose if received when students are 16 years of age or older are required before school starts in the fall. The Tdap requirement has been in place since the 2013/14 school year and requires a booster dose of Tetanus, Diphtheria, and acellular Pertussis containing vaccine for students entering 7 th grade, regardless of the interval since the last Tetanus/Diphtheria containing vaccine. Pertussis (whooping cough) is a very contagious disease that causes violent coughing fits that make it hard to breathe. The meningococcal vaccine has been required since the start of the 2017/18 school year. Meningococcal disease is a life threatening illness that is caused by bacteria that infects the brain, blood, and spinal cord. It easily spreads in crowded settings. The vaccine is required prior to both the 7 th and 12 th. What does this mean for you? You will need to call your physician s office to make an appointment for these vaccines. You will need to check your home records or call your physician s office to see what your student s status is. If you do not have insurance, you can contact the Siouxland District Health Department at Please send a copy of your student s updated immunization record to the school nurse prior to the first day of school. Thank you in advance for your cooperation in this matter. Kindergarten & 9th Grade Students According to Iowa law, all Iowa children enrolling for the first time in Kindergarten or 9th grade must have a dental screening on file. It s required per Iowa law! According to this law: Screening must occur no earlier than 1 year before enrollment and no later than 4 months after enrollment. If you think you have already sent this form to school, please double check with your student to make sure that they have turned it into the nurse s office. Screenings can be performed by dentists or dental hygienists. Here is a link to the dental form. uploads/2015/03/certificate_of_dental_screening_ pdf Please have this form filled out by your dental provider and turned in to your high school.
