3 MISSION STATEMENT The school s mission is to train students to pass the state exam and prepare them for entry into the Cosmetology and Esthetics fields. This is achieved through student, graduate, and salon owner surveys, continued education and ongoing improvement program. ADMISSIONS REQUIREMENTS COSMETOLOGY & ESTHETICS Admission is limited to individuals who are least 16 years old and have: a high school diploma, or its equivalent (GED) or A state recognized home school certificate. Those individuals who do not have a high school diploma or GED must obtain one if they wish to apply for financial aid. The State Board requires applicants to be at least 18 years of age at the time the State Board Examination is taken. This requirement must be met before you can apply to take the state board exam. ADMISSIONS REQUIREMENTS TEACHER TRAINING Successful completion of the Cosmetology or Esthetics program. Have a high school diploma, or its equivalent. (GED), Six months salon experience for Cosmetology and Esthetics applicants LANGUAGE All programs are taught in English. ENROLLMENT Students may enroll by appointment at any time during regular school hours. 4

4 COSMETOLOGY COURSE The complete course is 1500 hours in length. It can be completed in approximately one year for those students who attend 30 hours per week. The primary objective of this course is to prepare students for the state licensing examination and to train them in job marketable skills. All units of instruction include theory, practical demonstrations, lectures, and practice. Records of progress are maintained on all students. 7 UNITS OF INSTRUCTION TEACHER TRAINING The following is a table of the units of instruction for the teacher training course. To the right of each unit is the number of hours devoted to each unit. The Senior Studio is a small cosmetology clinic area containing shampoo basins, work stations, and a dispensary. A separate area is provided for skin care and manicuring. The school has a large esthetics clinic area containing a facial room set up with five treatment areas, a wet treatment room, a room for waxing, chemical peels, micro- dermabrasion, skin analysis, hand and foot treatments, a supply room, and a dispensary. The school is equipped with reference libraries, audio and visual aids in the classrooms and a non- smoking student lounge. GRADING SYSTEM COSMETOLOGY & ESTHETICS Written exams are given following each section covered in the theory class. Exams are scored based upon the percentage of correct answers. That score is then applies to the following scale EXCELLENT GOOD AVERAGE 74 0 POOR, FAILING While all practical work is evaluated to see that it is satisfactory; actual grades are assessed to a student s practical work by periodic evaluations. Each service is broken down into various components which must be done correctly in order to receive the portion of points attributed to that component. The student s score is computed based upon the percentage of the total points available that the student receives. The percentage score is then applied to the previous scale. Practical evaluations are done at the end of the freshman program, and prior to graduation. 8

8 GRADING SYSTEM TEACHER TRAINING Written exams are given following each section covered in the theory class. Exams are scored based upon the percentage of correct answers. That score is then applies to the following scale EXCELLENT GOOD AVERAGE 74 0 POOR, FAILING The practical portion of this course is the actual teaching of theory and demonstrating of practical work. It is therefore, this teaching and demonstration that is evaluated for a practical grade. While all classes taught are evaluated to see that they are satisfactory; actual grades are assessed to a student s practical work by periodic evaluations. Each class is broken down into various components which must be done correctly in order to receive the portion of points attributed to that component. The student s score is computed based upon the percentage of the total points available that the student receives. The percentage score is then applied to the previous scale. SATISFACTORY PROGRESS The maximum time frame for course completion is one and one- half times the approved course length. Cosmetology students are evaluated at 450 hours, 900 hours and 1200 hours. Teacher Training students are evaluated at 450 hours and 900 hours. Esthetics students are evaluated at 350 hours for attendance and academic standards. The standard for academic progress evaluation is: The student is encouraged to pass all scheduled tests for the evaluation period with a score of 75% or higher. Students must receive a Grade Point Average of 75% or higher score on their evaluation to be considered passing. The standard for attendance evaluation is: The student must be able to complete the course within the maximum time frame based upon their average weekly attendance. The minimum requirement is 66.67% If a student fails to make minimum satisfactory progress in attendance and/or academic requirements, the student is placed on Financial Aid Warning and will be given opportunity to bring performance up to the compliance level in the following evaluation period. A student may not be placed on FAW for consecutive periods. At the end of the FAW period the student must be meeting the published attendance and academic standards to remain eligible for financial aid. If the student is not making Satisfactory Progress, he/she may appeal and if the appeal is granted the student will be placed on Probation. If the appeal is granted, financial aid is reinstated for the subsequent payment period only. GRADUATION REQUIREMENTS COSMETOLOGY, ESTHETICS & TEACHER TRAINING Students must have completed the required clock hours, met all practical requirements, passed all regular examinations (minimum passing score is 75%) and pass both the written and the practical finals with at least an 85% average. Tuition and fees must be satisfied. Upon completion of the course and satisfaction of all graduation requirements the student is awarded a diploma that certifies that all of the above is listed requirements have been met. 9

9 LICENSURE REQUIREMENTS TEACHER TRAINING The Professional Licensing Agency requires the completion of additional work experience in a salon in addition to 1000 hours of teacher training course work in a beauty college. To receive an Instructors License in the state of Indiana, you must have a high school diploma or GED. WITHDRAW POLICY The student has the right to withdraw at any time. If a student wishes to withdraw, a signed dated withdrawal notice indicating the date of withdrawal should be delivered to the school. Once the student is determined to have withdrawn, the school will calculate and remit any applicable refund. RETURN OF UNEARNED TITLE IV FUNDS Federal regulations have been enacted which state that students may forfeit a portion of their federal student assistance if they fail to complete the program of study in which they were enrolled. This policy affects students who: a) received or were eligible to receive federal student financial assistance authorized under Title IV of the Higher Education Act (HEA), i.e. Federal Pell Grants, Federal SEOG awards, or Stafford or PLUS loans Awarded under the Federal Direct Student Loan (FDSL) programs, and b) who, on or after October 7, 2000, withdrew or were terminated from the institution during the first 60% of any payment period or period of enrollment. Payment periods are defined as (a quarter, trimester, semester, or X number of clock or credit hours). Eligible Title IV aid recipients who fail to complete over 60% of a payment period or period of enrollment are considered to have not earned all of the federal aid that may have previously been awarded for that payment period or period of enrollment. Accordingly, a required calculation will be performed to determine the portion of the unearned federal student aid that must be returned to the U.S. Department of Education. This calculation will be done before a tuition refund calculation is performed in accordance with the institution s refund policy. In many cases, the Return of Unearned Title IV Funds calculation will result in the student owing tuition and fees to the institution that would otherwise have been paid with federal student aid. this policy may also result in the student owing a refund of unearned federal aid to the Department of Education. DESCRIPTION OF SCHOOL S OWN REFUND POLICY Refunds on behalf of Student Financial Assistance recipients must be distributed in the following manner 1. Unsubsidized Federal Direct Stafford Loans 2. Subsidized Federal Direct Stafford Loans 3. Federal Direct Plus Loans 4. Federal Pell Grants 5. 10 INSTITUTIONAL REFUND POLICY A. Any monies due to the applicant or student shall be refunded within 30 days of official cancellation or withdrawal. Official cancelation or withdrawal shall occur on the earlier of the dates that: 1. An applicant is not accepted by the school. This applicant shall be entitled to a refund of all monies paid. 2. A student (or in the case of a student under legal age, his/her parent or guardian) cancels his/her contract and demands his/her money back in writing, within three business days of the signing of the contract regardless of whether the student has actually started training. All monies collected by the school shall be refunded. 3. A student cancels his/her contract after three business days after signing, but prior to entering classes. In this case, he/she shall be entitled to a refund of all monies paid to the school less an application fee if applicable, and registration fee of $ for Cosmetology, Esthetics, and Teaching Trainee. 4. A student notifies the institution of his/her withdrawal. 5. A student on an approved leave of absence notifies the school that he or she will not be returning. The date of withdrawal determination shall be the earlier of the scheduled date of return from leave of absence or the date the student notifies the institution that the student will not be returning, 6. A student is expelled by the school 7. In type 2, 3, 4, or 5 official cancellations or withdrawals, the cancellation date will be determined by the postmark on written notification, or the date said information is delivered to the school administrator/owner in person. B. Any monies due a student who unofficially withdraws from the institution shall be refunded within 30 days of a determination by the institution that the student has withdrawn without notifying the institution. Unofficial withdrawals for clock- hour schools are determined by the school through monitoring clock hour attendance every 14 days. C. For students who enroll in and begin classes, the following schedule of tuition adjustment will be considered to meet the minimum standards for refunds. 11

Any monies due to the applicant or student shall be refunded within 30 days of formal cancellation by the student as defined in item #2, or formal termination by the school, which shall occur no more than 14 days from the last day of physical attendance, or in the case of a leave of absence, the documented date of return. The tuition a student owes the school accounts for any absence time that occurs during the student s enrollment period. D. Enrollment fee, textbooks, kits, HFBKBC support material and locker fee are not included in refund computation. Tuition fees are refundable. E. Student and school have a complete description of services school is to furnish for which student is to pay. Description as to classes and hours, conduct and schedules may be augmented by bulletins and other notices furnished to the student by the school. The school is operated on a continuous basis throughout the year. Classes commence at 9:00 AM and run thru to 5:00 PM. Tuesday thru Friday and 8:00 AM to 4:00 PM on Saturday. F. If a student withdraws from classes or their enrollment is terminated by the school a termination/ withdrawal fee of $ will be charged to the student by the school. G. If a school is permanently closed and ceases to offer instruction after a student has enrolled, and instruction has begun, the school must make arrangements for the student., The student shall be entitled to a pro rata refund of tuition. H. If a course is cancelled subsequent to a student s enrollment but before instruction has begun, the school shall at its option: 1. Provide a full refund of all monies paid; or 2. Provide completion of the course. I. If a school cancels a course and ceases to offer instruction after students have enrolled and instruction has begun, the school shall at its option: 1. Provide a pro rata refund for all students transferring to another school based on the hours accepted by the receiving school, or 2. Provide completion of the course, or 3. Participate in a Teach- Out Agreement, or 4. Provide a full refund of all monies paid. 12

12 J. Collection procedures reflect good taste and sound ethical business practices. Collection correspondence regarding cancellation and settlement from the institution itself, collection agencies, lawyers or any other third party representing the institution clearly acknowledge the existence of the withdrawal and settlement policy. EMPLOYMENT ASSISTANCE Placement assistance is available at the time of graduation or at any time in the future. Students may contact the school at any time for information regarding open positions. Students are assisted by Jim Galloway with preparation of a resume upon request. A copy is provided; additional copies are the responsibility of the student. Placement is not guaranteed. Graduates are assisted in locating prospective salon; finding the correct contact person setting interview and interview follow up as necessary. Assistance is provided to graduates until it is no longer needed. Instructional staff is available during regular school hours to provide assistance to students as needed. SCHOOL CALENDAR New classes start the first and/or third Tuesday of every month. The school reserves the right to change the school calendar and/or add additional start dates if it should become necessary. The school observes and closes for the following holidays: New Year s Day, Fourth of July, Thanksgiving, Christmas Eve, and Christmas Day. (These dates change from year to year based upon when the holiday falls on the calendar.) The exact dates are posted for the students. 13 STUDENT RECORDS The FERPA is a federal law designed to protect the privacy of a student s educational records. The law applies to all schools, which receive funds under an applicable program from the U.S. Department of Education. The FERPA gives certain rights to parents/guardians of dependent minors regarding their children s education records. These rights transfer to the student or former student who has reached the age of 18 or is attending any school beyond high school level. Students and former students to whom the rights have transferred are called eligible students. Parents/guardians of dependent minors or eligible students have the right to inspect and review all of the student s educational records maintained by the school. Schools are not required to provide copies of material in educational records unless, for reasons such as great distance, it is impossible for parents/guardians of dependent minors or eligible students to inspect the records personally. The school may charge a fee for copies. Parents/guardians of dependent minors and eligible students have the right to request that a school correct records believed to be inaccurate or misleading. If the school refuses to change the records, the parents/guardians of dependent minors or eligible student then has the right to a formal hearing. After the hearing, if the school still refuses the correction, the parents/guardians of dependent minors or eligible student has the right to place a statement in the records commenting on the contested information in the records. Generally, the school must have written permission from the parents/guardians of dependent minors or eligible student before releasing any information from a student s record. However, the law allows schools to disclose records, without consent, to the following parties: - school employees who have a need to know; - other schools to which a student is transferring; - parents when a student over 18 is still dependent; - certain government officials in order to carry out lawful functions; appropriate parties in connection with financial aid to a student; - organizations doing certain studies for the school; - accrediting organizations; - individuals who have obtained court orders or subpoenas; - persons who need to know in case of health and safety emergencies; - state and local authorizes to whom disclosure is required by state laws adopted before November 19, 1974 You have the right to file a complaint with the U.S. Department of Education concerning alleged failures by the school to comply with the requirements of FERPA. The student records his hours, grades, and services daily and has them verified by an instructor. The school personnel are responsible for totaling and recording each student s hours at the end of every week. All grades, services, and hours to be credited in each subject are kept on file and forwarded to the State Board, as required, with the student s application to take the licensing exam. 14

14 SCHOLARSHIPS Scholarships are offered to students in the Teacher Trainee program. The awards are based upon their performance in the Cosmetology program they have already completed. Financial need is also taken into account. A student may apply for a Hardship Scholarship if a waiver fees is necessary and all other options have been exhausted. An application is available in the financial aid office upon request. ADVISING Advising is available to students during all phases of training. OTHER AVAILABLE SERVICES The school provides career counseling upon request. Housing is not available. A list of professional assistance agencies is provided to students at orientation. 15 Written exams for state licensing are given at various locations throughout the State of Indiana. All tuition and fees must be paid or satisfactory arrangements must have been made and all tests passed and progress sheets completed before taking exam. The school gives the final tests prior to registration for the state exam. The school works very closely with the state guidelines to prepare you for your state licensing examination. Remember, anything is easier if you are prepared. Requirements are simple. They are checking to see that you are trained to not pose a threat to the health and safety of the public, and you can carefully read and follow instruction in the use of chemicals. Certificates of Accreditation and Licensure are on display in the schools and may be reviewed any time during normal business hours. BE A PROFESIONAL A student must be dedicated and agree to abide by the Code of Ethics (Copies of the Code of Ethics maybe found in the School s Information Packet and Posted in the School) established by the profession, in order to maintain the high standards sought by Hair Fashions by Kaye Beauty College. Even through a college degree is not required, you are expected to conduct yourself in a professional manner. SUPPLIES Students may use their copy of their contract for admittance to professional supply houses. CLIENTS Clients are taken on number basis; first come, first served. An all- out effort must be made to service as many clients as necessary in order to achieve the fullest educational experience. TRANSFERS If a student wishes to terminate his/her course, a transcript of hours is available. Tuition and fees must be paid up to date. See addendum for tuition and fees. If a student has attended another school and wishes to enroll at our school, we will accept hours only in whole one hundred hour increments. The student must supply our school with a certified transcript from the school they previously attended. The student must be able to complete the program in the 3 year period as required by the state of Indiana. A written practical exam is given to evaluate the student s ability prior to enrollment. DAILY ROUTINE 1. Clock in at 8:50 A.M. 2. Sign in on the appropriate Sign- in sheet. 3. Check the sanitation duty for the day on the Sign- in sheet. Sanitation duties are done throughout the day. 4. Go to class. 5. At lunch time, clock out and clock back in. 6. At the end of the day, finish the sanitation duty assigned to you for the day. Clean your own work area throughout the day. 7. Clock out before going home. 16

16 POLICIES REGARDING TARDINESS, ABSENCES, CONDUCT, TERMINATION AND OTHER RULES AND REGULATIONS A student s enrollment will be terminated if they are absent for more than 14 consecutive calendar days. Students are considered tardy weekdays at one minute past 9:00 a.m., and one minute past 8:00 a.m. on Saturdays. We count on 1/4 hour at the end of the week. The school does not recognize or identify absences as excused or unexcused. Students are to take their textbook and pen or pencil to class. Students are not to take magazines to class. Students are not allowed to eat, smoke, or chew gum in class. They are not to talk, sleep, lounge, or lay their heads on the desk during class. Proper conduct is expected from each student. The following is a list of reasons for suspending or expelling a student: 1. If a student is absent more than 14 consecutive calendar days, his/her enrollment will be terminated. 2. Failure to follow school and state sanitation requirements. 3. Clocking or signing another student in or out. 4. Not making tuition payments according to contract. 5. Failure to clock out when leaving for lunch. Clocking back in before you are ready to go back on the clinic floor or to class. 6. Cigarette smoking in a non- designated area, or while clocked in. 7. Leaving the building without permission. 8. Leaving school before the student s contracted time unless there is special permission. No permission to leave early on Saturday will be given except in case of illness or emergency. 9. Receiving personal services without first getting permission from the clinic floor director. Both the student doing the service and the student receiving the service are accountable. 10. Removing time cards or progress book from school premises. 11. Refusal of an assignment or direction. Students are to accept any assignment be it clinic floor, theory, freshman area, classroom, sanitation, or paperwork that is given to them by school personnel. The following actions are considered to be refusing an assignment: a. Not answering the call at the front desk. b. Disagreeing or arguing with the school personnel about the services or procedure or the client. c. Making any comment or expression that would embarrass the client. d. Refusing to take their lunch break at the assigned time. e. Failure to perform a service that is written on the client s ticket or performing a service that is not written on the client s ticket. 12. Not following the school dress code or staying in dress code the entire time they are on the clock. This includes while studying, working on a mannequin, giving or receiving a service, etc. 13. Using foul or inappropriate language on the clinic floor, freshman area, or classroom. 14. Being in school under the influence of illegal drugs or alcohol. 15. Having illegal drugs or alcohol in the student s possession. 16. Any conduct that would disrupt the school s ability to carry out the school s responsibility to students. 17. Cheating on an exam. 17

17 18. Any unsubstantiated claim of wrong doing, such as claims of theft against the school, faculty, or fellow student by a student or his agent(s). 19. Theft. Theft from the school or another student will result in immediate termination of enrollment. 20. Racial or sexual harassment, to include name calling implying sexual orientation, will not be tolerated between students, clients, or personnel. If any act of racial or sexual harassment is confirmed by a staff member this will mean immediate termination with no chance of re- entry. 21. Failure to have equipment and books in school. 22. Failure to attend school as contracted. 23. Failure to provide and/or sign required material for financial aid. 24. Any threat of violence (actual or implied) toward another person (student, school personnel, or client) or school property will mean immediate termination with no chance of re- entry. 25. Gossiping or causing discord. 26. Arguing with another student or staff member about anything. 27. Unprofessional conversation or distracting another student from their assignment. 28. Weapons of any kind are not allowed on school premises. Possession of weapons of any kind will result in immediate termination of enrollment with no chance of re- entry. MAKE-UP WORK POLICY Theory tests may be made up upon receiving permission from the appropriate staff member during any available time, except on Saturdays. Most of our practical classes are on DVD and are available for students if a class has been missed. Making up missed work is the responsibility of the student. LEAVE OF ABSENCE Leaves of absence up to 180 days are available. METHODS AND TERMS OF PAYMENT Financial Aid Students A $ enrollment fee is due at the time of enrollment. If sufficient eligibility has been established the enrollment fee may be paid from Financial Aid proceeds as needed. Students must complete and provide all forms and material necessary to determine financial aid eligibility and process awards. This information must be provided prior to starting class. Cash Pay Students For each of the courses offered there is a cash down payment of 25% of the total cost due at orientation. The remaining balance is paid in equal monthly payments beginning thirty days after start date. Other payment schedules may be established for the convenience of the student, subject to administrative approval. FINANCIAL AID The following types of Federal Financial Aid are available to those who qualify. Pell Grants Wm. D. Ford Federal Direct Loan Program The Students are assisted in applying for Federal Financial Aid by the financial aid officer present in the school. Prospective students may make an appointment, at their convenience to speak with the school financial aid officer 18

18 or enrollment advisor to begin the application process. Applications for Federal Student Aid Programs are available at the school. Student eligibility requirements are: U.S. citizen or National Permanent resident of the United States Must be beyond the age of compulsory school attendance (age 16 in the State of Indiana) Must have a high school diploma, or its equivalent (GED), or a state recognized home- schooling certificate. Must meet minimum satisfactory progress requirements. Must be enrolled at least 20 hours per week Must not owe a refund on any Title IV Grant or Loan Must demonstrate financial need, if applicable Criteria for determining award amount: With the exception of the unsubsidized Stafford loan program and Parent loan program for undergraduate students (Plus Loans), the student must demonstrate financial need to receive aid from the financial aid programs. Financial need is the difference between the students cost of education and the student s ability to pay the cost. The amount of aid awarded to the student is determined by the student s Pell Grant index, cost of education and family contribution amount, using methods prescribed by the U.S. Department of Education. Any student who meets the student eligibility requirements will be awarded financial aid, based on their demonstrated need beyond their ability to pay (with the exception of unsubsidized Stafford loan program); and provided that they follow the school s procedure for applying for assistance. For all programs offered by Hair Fashions by Kaye Beauty College students must be continue to meet all eligibility requirements and maintain satisfactory progress in the course in order to maintain eligibility for financial aid. Financial aid payments are made in the following manner: Pell Grants Pell grant funds are made payable to the school on behalf of the student; and credited to the student s account by Hair Fashions by Kaye Beauty College financial aid office. Students can qualify for a maximum of 2 Pell payments per grant and a maximum of 2 grants during the 1500 hour cosmetology course. Students in the Teacher Training course normally qualify for 1 Pell Grant. Pell Grant payments are approved for disbursal at 450 hour increments for Cosmetology & Teacher Training and 350 hour increments for Esthetics, provided the student is making satisfactory progress. Direct Loans Proceeds from Direct Loans are disbursed to the school from the Department of Education. The loan is disbursed in two parts. $ per month for an independent student and $ per month for a dependent student is the average typical room and board cost for students attending Hair Fashions by Kaye Beauty Colleges. $ per month for an independent student and $ per month for a dependent student is the average typical transportation and personal expense cost for students attending Hair fashions by Kaye Beauty Colleges. MORE ABOUT LOANS Student loans have several unique features. The loan officer or financial aid administrator will discuss these in detail upon applying for a student loan. Of particular importance to you should be the repayment schedule you will need to follow in repaying the loan. To give you an idea of the total amount you will need to repay, we have prepared the following chart for loans with an 8% interest rate. 20 OTHER SOURCES OF FINANCIAL AID In addition to federal and state programs, certain students may be eligible for other programs, which provide assistance for educational costs. These benefits, like grants, do not have to be repaid. Vocational Rehabilitation: The Division of Vocational Rehabilitation provides services and financial assistance for education to students with certain disabilities. Further information can be obtained from the Division of Vocational Rehabilitation. A list of this agency is found in the white pages under the name of the State, then Rehabilitative Services or Vocational Rehabilitative Services. Job Training Partnership Act (JTPA): The Department of Labor makes funds available through local agencies for training persons meeting certain criteria. For example, some agencies concentrate their efforts on displaced housewives, whereas others might concentrate on minority youths.
