4 The Need Marijuana In 2013, 35% M-DC high school students and 10% M-DC middle school students used Marijuana at least once in their life. 20% of M-DC high school students have used Marijuana in the past 30 days. 7% of M-DC high school youth report they tried Marijuana for the first time before the age of 13, while 4% of M-DC middle school students report trying before the age of 11. 4

6 The Need Tobacco In 2013, 28% M-DC high school students and 14% of M-DC middle school students tried cigarette smoking, even if it was one or two puffs. 8% of M-DC high school and 2% of M-DC middle school students smoked cigarettes in the past 30 days. 25% of M-DC high school and 12% of M-DC middle school students reported that they bought their own cigarettes in a store or gas station during the past 30 days. 4% of M-DC high school students smoked a whole cigarette for the first time before the age of 13, while 3% of M-DC middle school students reported trying a whole cigarette before the age of 11. 6

8 What s the Point? 1 in 2 children who have their first drink of alcohol by age 13 will have an alcohol dependency problem as a adult, compared to 1 in 11 who don t have their first drink until age 21 or later. Those using Marijuana by age 14 have six times greater potential for adult drug dependency/abuse than those using after age 18. Serious mental illness is double that for those who first used Marijuana by age 14. Underage drinking and substance abuse are precursors of multiple negative youth behaviors and outcomes. The EARLIER the first exposure to substance abuse, the GREATER the risk for LIFELONG CONSEQUENCES of dependency. 8

9 THE COUNTER MEASURE: SBIRT-CRAFFT Miami Focus: Pilot an approach to regularly screen youth (12-18) most at risk for substance abuse Tools: Promote SBIRT-CRAFFT screening protocols, expanding screening through evidence-based practice Engagement: Engage youth and health providers to adopt protocols through outreach via a neighborhood based delivery system Facilitation: support expansion implementation through provision of ongoing Training and TA 9

10 SBIRT Screening Protocol SBIRT: Screening-Brief Intervention & Referral to Treatment A nationally recognized SAMHSA supported screening PROTOCOL with a strong research base demonstrating effectiveness It s relatively low-tech and brief 5 to 10 minutes It s universal anyone can be screened as part of a standard intake process It targets alcohol and other drugs 10

11 Project Screening Tools Reliable Validated Standardized 11

12 S2BI: Screening to Brief Intervention Developed by the Center for Adolescent Substance Abuse Research (CeASAR), Children s Hospital Boston, in affiliation with Harvard Medical School Frequency screen that triages adolescents into risk categories based on the DSM-5 diagnoses for SUDs Frequency questions are recommended for screening adults and are used in the NIAAA Youth Alcohol Screen. A small study at BCH also suggested they are also accurate with teens. 12

14 CRAFFT Screening Tool CRAFFT is an evidence based six question behavioral health screening TOOL for youth under 21 Developed by the Center for Adolescent Substance Abuse Research (CeASAR), Children s Hospital, Boston, in affiliation with Harvard Medical School Recommended by the American Academy of Pediatrics Committee on Substance Abuse for adolescents Available for use in multiple languages in either a selfadministered or clinically administered version 14

17 Sustainability SBIRT-CRAFFT protocol is low-tech/low cost SBIRT-CRAFFT protocol is easily integrated into existing intake procedures SBIRT-CRAFFT protocol is JHCAO recommended and may soon become required SBIRT-CRAFFT protocol is increasingly recommended by youth and substance abuse prevention funders SBIRT-CRAFFT protocol is a billable service requires Florida Medicaid code and appropriate staff SBIRT-CRAFFT protocol is proven effective 17
