The group meets quarterly to network, share news of new and innovative projects and hear from guest speakers. Dementia specific information is distributed electronically on a regular basis. Hours will vary) Free ADDITIONAL NOTES HACC Funded DASS is a statewide service providing assistance to people living with dementia, their loved ones and service providers to further develop knowledge and skills in dementia care. Advisors work in consultation with the individual to provide support in the changes they may be experiencing, with the goal of enhancing independence and choice. Based on individual care needs, personalised dementia services may include access to: Face to face or telephone support Identification and assessment of needs Assistance to overcome challenges Support to rediscover independence Links to other support services Provision of information to achieve a greater understanding of living with memory loss and/or dementia Help with care option Delivery of person centred education in the community West Moreton/South Coast region covers from Gold Coast, Logan, Boonah, Beaudesert, Laidley, Ipswich, Lowood, Esk to Toogoolawah. 4 Ipswich West Moreton Dementia Network Service Directory
5 Dementia Specific Services Ipswich Multi Service Centre, Alzheimer s Queensland Caylie Field ipswich@alzheimersonline.org 84 Chermside Road, Ipswich QLD hours a day / 7 days a week $25 per day / $50 per night Reduced rates can be discussed ADDITIONAL NOTES Alzheimer s Queensland provide: A 24 hour Dementia Helpline Education for both professional groups and carers. Community Awareness programs Residential Care Support Groups AQ Rehab Physiotherapy, Speech Pathology, Occupational Therapy, Dietetics and Nursing services in the setting of your choice (home, clinic, workplace or other facility) Disability Driving Assessments The service is provided from an old Queenslander cottage that is affectionately referred to as Rosemary Cottage. The service is available to those people over 65 who have Dementia (or symptoms of Dementia) or are frail elderly. For people under 65 (or 50 for Indigenous visitors) there must be a diagnosis of Dementia. The service is funded through HACC and NRCP. The services offered include: Centre-based day respite (offering social and recreational opportunities that are provided in the cottage as well as out in the community) Overnight centre-based respite In-home social support Home maintenance Alzheimer s Queensland also run an Ipswich Carer Support Group on the 3rd Wednesday of the month, 10am-12 noon at the Calvary Baptist Church (83 Chermside Road). We offer a range of services for people with dementia, family carers, industry professionals and anyone who wants to know more about memory loss, dementia and good brain health. Alzheimer s Australia has launched a national Younger Onset Dementia Key Worker Program to provide individualised support and services to improve the quality of life for people with younger onset dementia, their families and carers. A key worker is the primary point of contact and assists people with younger onset dementia to locate and link into the service and support options appropriate for their individual needs. The principle underpinning the key worker model of service delivery is consumer directed care. We are client-centred and consent based, and essentially work with older people and their carers who have contact with Ipswich Hospital to promote enhanced health management and quality of life. Many of these people are experiencing varying levels of cognitive impairment +/- formal dementia diagnoses. We also provide social work support to outpatients of the geriatrician clinics, including a number of people living with the effects of dementia. We believe that everyone should be able to choose to live in his or her home for as long as possible and maintain their self esteem through independent living. We provide support for: Older people needing assistance at home with meals, shopping, cleaning. Anyone needing help at home after medical treatment and needing recuperation Those needing regular transport to doctors and hospitals People who wish to remain independent. Respite Care Our mobile respite care service is offered as a support for those caring for others in their own homes and in some of our retirement villages. Respite care offers temporary care, allowing the primary carer a little time out for appointments, family commitments or a much deserved holiday. 27 Assessment Services If you: Require Assessment to access services Would like to enquire about any aged care services in the region or Would like the most current aged care service information Please contact: My Aged Care To contact the Aged Care Assessment Team (ACAT) in Ipswich, please phone The Aged Care Assessment Team undertakes assessments for frail older people and, in some circumstances younger people with a disability, to determine their eligibility for Commonwealth Subsidised services including: Home Care packages Residential Respite Care; and Permanent Residential Care People referred to the Aged Care Assessment Team must: Consent to the referral Have care needs that cannot be addressed through HACC funded services; and Have capacity to make informed lifestyle decisions. If a person lacks capacity to make lifestyle decisions evidence must be provided from an appropriately qualified medical practitioner. Eligibility for services: Over 65 (or 50 for ATSI) with health conditions that impact on their ability to maintain independent living at home. Under 65 with a moderate to severe disability. Carer of the above. Cost: Client contribution and available services will vary. Contact individual service providers for details and to discuss your situation. Community Home Care Packages (Levels 1-4) & Consumer Directed Care Packages (CDC) Increasing levels of support services to enable eligible clients to remain living independently in their own home in the community. ACAT assessment and approval is required. Contact My Aged Care for assessment. Cost: Client contribution and available services will vary. Contact individual service providers for details and to discuss your situation. Overnight Respite Community Respite No ACAT assessment required.
