We are here ABOUT COMMUNITY REHAB NQ Community Rehab NQ is a community rehabilitation service for the people of northern Queensland. Services are for people with neurological conditions, such as stroke, brain injury, spinal cord injury, Parkinson s disease and Multiple Sclerosis. People experiencing mild to severe issues or concerns as a result of their neurological condition can be assisted by the service. THE TEAM: Physiotherapists; Occupational Therapists; Exercise Physiologists; Speech Pathologists; Registered Nurse; Dietitian; Neuropsychologist; Social Worker; Rehabilitation Assistant; and Indigenous Rehabilitation Assistant. Together with: University students of these professions. HOW SERVICES ARE OFFERED Individually or as a group. In the Community Rehab NQ Centre and in the community. Rural intensive e.g. every day for two to three weeks. Regular sessions e.g. three days a week for five weeks. Longer term programs e.g. over a three month period. Regular review of progress e.g. annual review and update. Great benefit to community. Well resourced, Friendly and approachable staff. Much needed facility. The information was helpful. Tailor made for individuals. I have learnt so much about my condition and now more capable of managing it appropriately. 6 Individual Rehabilitation Sessions Community Rehab NQ offers individual rehabilitation sessions for people who have a neurological condition who would like to improve their function and quality of life. Sessions focus on activities such as communication, swallowing, balance & mobility, use of the arm in everyday tasks, continence, managing everyday activities, health and fitness, swimming, counselling, health & wellbeing. Individual sessions are available up to 8 hours a day, 5 times a week. Sessions are planned with the participant, to focus on their goals and priorities. Sessions may include assessment, interventions and / or education. A thorough assessment is provided at the start and end of their rehabilitation program. The team assists participants to identify realistic solutions to achieve their goal. A visit to the home or within the community is undertaken if required. Links are made with each individual s local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. Individual rehabilitation sessions are provided by a multidisciplinary team that includes Physiotherapists, Occupational Therapists, Exercise Physiologists, Speech Pathologist, Registered Nurse, Dietician, Psychologist, Neuropsychologist, Social Worker and an Indigenous Rehabilitation Assistant, and by University students of these professions. 8 Use your Arm in Everyday Tasks Community Rehab NQ offers a program for people who would like to improve their ability to use their arm and hand in everyday tasks. The program is designed for people who have upper limb activity limitations as a result of a neurological condition such as Stroke, Brain Injury, Multiple Sclerosis or Cerebral Palsy. The program runs for 3 sessions per week for 1 and a half hours each session. Sessions include physical activity and education. The program focuses on each person s goals and priorities. A thorough assessment is provided at the beginning and end of the program. The team assists participants to identify realistic solutions to achieve their goals. A visit to the home or within the community is undertaken if required. Links are made with each individual s local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. The program is led by a Physiotherapist and an Occupational Therapist with support from the multidisciplinary team that includes Exercise Physiologists, Speech Pathologist, Registered Nurse, Dietician, Psychologist, Neuropsychologist, Social Worker and an Indigenous Rehabilitation Assistant, and by University students of these professions. 9 Balance & Mobility Circuit Class Community Rehab NQ offers a balance and mobility program, in a circuit class format, for people who have a neurological condition such as Stroke, Brain Injury, Parkinson s Disease or Multiple Sclerosis, who would like to improve their ability to participate in everyday life. It is also for people who have had a hip or knee replacement recently and are ready to get out and about. This class is available 5 days a week for 90 minutes each session. Participants can choose how many classes per week they would like to attend. Classes include physical activities that focus on balance and walking. The class focuses on each person s goals and priorities. The team assists participants to identify realistic solutions to achieve their goals. A visit to the home or within the community is undertaken if required. Links are made with each individual s local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. The program is led by a Physiotherapist and Occupational Therapist with support from other members of the multidisciplinary team that includes Exercise Physiologists, Speech Pathologist, Registered Nurse, Dietician, Psychologist, Neuropsychologist, Social Worker and an Indigenous Rehabilitation Assistant, and by University students of these professions. 10 Walking Group Community Rehab NQ offers an afternoon walking group. The walking group is suitable for people who would like to walk regularly. It is designed to assist people who have walking difficulties resulting from a neurological condition such as Cerebral Palsy, Stroke, Brain Injury, Parkinson s Disease or Multiple Sclerosis. It is also for people with mobility issues associated with ageing. The walking group runs 3 sessions per week for 1 hour each session. The group focuses on each person s goals and priorities. A thorough assessment is provided at the beginning and end of the program. The team assists participants to identify realistic solutions to achieve their goals. Links are made with each individual s local services and facilities. The program is delivered by an Exercise Physiologist and/or a Physiotherapist, and by University students of these professions. 11 Communication Group Community Rehab NQ offers a program for people who would like to improve their communication. This includes people who have difficulties with talking, understanding others, reading and writing, as a result of a neurological condition such as Stroke, Brain Injury, Parkinson s Disease or Multiple Sclerosis. This program is available 4 days a week for 1 hour each session. Participants can choose how many times per week they would like to attend. The program focuses on each person s goals and priorities. The team assists participants to identify realistic solutions to achieve their goals. A visit to the home or within the community is undertaken if required. Links are made with each individual s local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. The program is led by a Speech Pathologist with Speech Pathology students and with support from other members of the multidisciplinary team which includes Occupational Therapists, Psychologists, Physiotherapists, Exercise Physiologists, Speech Pathologist, Registered Nurse, Dietician, Social Worker and an Indigenous Rehabilitation Assistant, and by University students of these professions. 12 Everyday Activities - Workstations Community Rehab NQ offers a group program for people who wish to improve their ability to perform everyday activities. This program is run in a workstation format and is designed for people who have a neurological condition such as Stroke, Brain Injury, Parkinson s disease or Multiple Sclerosis. It is also for people having difficulties with everyday activities due to ageing. This program is available 4 days a week for 90 minutes each session. Participants can choose how many classes per week they would like to attend. The program focuses on each person s goals and priorities. 13 Singing Group Community Rehab NQ offers a social singing group for people with communication difficulties. The aim of the singing group is to encourage participation and interaction in a comfortable and supportive environment. Singers of any ability are welcome. The singing group runs for 1 session per week for 1 hour each session. The group incorporates posture, breathing and vocal exercises along with singing. Participants are provided with a folder with education resources and handouts for warm-up activities and song lyrics. The singing group is delivered by a Speech Pathologist, Occupational Therapist, and University students of these professions. Support persons / partners / family members are encouraged to attend. 14 Fit and Well Group Community Rehab NQ offers a health and fitness program which includes education about a healthy lifestyle as well as physical activity sessions. The focus is on being fit and well through a healthy diet and a physically active lifestyle. The program runs for 2 sessions per week for 1 hour each session. Each session involves education and physical activity. The program focuses on each person s goals and priorities. A thorough assessment is provided at the beginning and end of the program. The team assists participants to identify realistic solutions to achieve their goals. A visit to the home or within the community is undertaken if required. The program is led by an Exercise Physiologist and Dietician with support from the multidisciplinary team which includes Physiotherapists, Occupational Therapists, Speech Pathologist, Registered Nurse, Psychologist, Neuropsychologist, Social Worker and an Indigenous Rehabilitation Assistant, and by University students of these professions. 15 Adapted Physical Activity Program (APAP) Community Rehab NQ offers an individually tailored physical activity program for people who have a disability as a result of a neurological condition such as Stroke, Brain Injury, Parkinson s Disease or Multiple Sclerosis. Physical activity means any movement of the body that results in burning calories e.g. walking the dog, going shopping, and working out at the gym. The program runs for 1 session per week. Sessions include physical activity and education. The program focuses on each person s goals and priorities. A thorough assessment is provided at the beginning and end of the program. The team assist participants to identify realistic solutions to achieve their goals. A visit to the home or within the community is undertaken if required. Links are made with each individual s local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. 16 Aquatic Program Community Rehab NQ offers an aquatic program for people who have a neurological condition such as Stroke, Brain Injury, Parkinson s disease, Multiple Sclerosis or Cerebral Palsy. It is also for people who have had a hip and knee replacement recently and wish to get out and about. The program is designed for people who would like to return to swimming or who could benefit from activity in a reduced load-bearing warm environment. This program is available 3 days a week for 45 minutes each session. Participants can choose how many times they attend per week. The program focuses on each person's goals and priorities. A thorough assessment is provided at the beginning and end of the program. The team assists participants to identify realistic solutions to achieve their goals. A visit to the home or within the community is undertaken if required. Links are made with each individual's local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. The program is led by a Physiotherapist and an Exercise Physiologist with support from other members of the multidisciplinary team that includes Occupational Therapists, Speech Pathologist, Registered Nurse, Dietician, Psychologist, Neuropsychologist, Social Worker and an Indigenous Rehabilitation Assistant, and by University students of these professions. 17 My Story Community Rehab NQ offers a program called My Story for Indigenous people who have a disability as a result of a neurological condition such as Polio, Cerebral Palsy, Stroke or Parkinson s disease. The program is delivered by the Indigenous Rehabilitation Assistant. 'My Story' picture cards are used to help the participant to tell their own story and to assist in overcoming barriers that could be caused by language and culture. The 'My Story' picture cards are used to identify what really matters to the individual so that they can work on these things whilst at Community Rehab nq. A program is designed with the participant, tailored to their goals and priorities. Each participant compiles a journal to guide them to work towards their goals at home. Family members are welcome to attend. My Story is led by the Indigenous Rehabilitation Assistant with support from the multidisciplinary team. Where: 16 Ryan Street in Belgian Gardens. 18 Have a goal, have a plan, have a go! This program runs in conjunction with all other programs. Participants are guided to set their goals, plan how to achieve their goals and then supported to 'have a go at their goal. Participants review their progress, to identify both opportunities available and barriers to overcome, to achieve their goals. Barriers may include difficulties with motivation, thinking, reasoning or remembering, communicating, walking or getting from place to place. The program runs for 1 hour per week. The program focuses on each person s goals and priorities. Participants generally participate in a range of other programs to work towards achieving their goals (e.g. communication mobility, everyday activities, health and fitness). A positive approach is used to help overcome any needs/barriers identified. Links are made with each individual s local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. The program is delivered by a multidisciplinary team that includes Physiotherapists, Occupational Therapists, Exercise Physiologists, Speech Pathologist, Registered Nurse, Dietician, Psychologist, Neuropsychologist, Social Worker and an Indigenous Rehabilitation Assistant, and by University students of these professions. 19 Get Up & Go The Get Up and Go program is designed for people who are ready to get out and about in the community following hip or knee replacements. The program runs in conjunction with all other programs. Participants are guided to set their goals, plan how they will achieve their goals and then supported to 'have a go at their goal. Participants review their progress to identify both opportunities available and barriers to overcome to achieve their goals. The program runs for 1 hour per week. 20 Keep it Up Community Rehab NQ offers the Keep it Up Club for people who would like to practice their activities independently but in a group environment. It is designed for people who have been provided with their own program to work on independently. A staff member is available to assist participants as required, so that they are able to continue to drive their own recovery in a safe environment with minimal supervision. The program is available for 2 afternoon and 2 morning sessions per week for 1 hour each session. The program is supervised by a Rehab Assistant, with a Physiotherapist, and/or Exercise Physiologist, and/or Occupational Therapist being available as needed. 21 Cerebral Palsy Program Community Rehab NQ offers a self-management program for people who have Cerebral Palsy and who would like to improve their function and quality of life. This program runs in conjunction with all other programs. Participants are guided to set their goals, plan how they will achieve their goals and then supported to 'have a go at their goal. This program consists of an 10 week package of services consisting of 2 12 hours a week. The package is designed with each participant, tailored to their goals and priorities. All participants attend self-management sessions for 2 hours a week. All participants are offered a range of other activity and education sessions that include communication, mobility, everyday activities and health and fitness. The team assists participants to identify realistic solutions to achieve their goals. A visit to the home or within the community is undertaken if required. Links are made with each individual s local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. 26 Brain Injury Program Community Rehab nq offers a self-management program for people who have an Acquired Brain Injury (ABI). This program, titled Skills To Enable People and Communities (STEPS), is a Queensland-wide program run through the Acquired Brain Injury Outreach Service (ABIOS). The program involves group discussions. This program runs in conjunction with other programs. The STEPS program runs 1 session per week for 6 weeks for 2 hours each session and is augmented by other programs as required. All participants are offered a range of other activity and education sessions that include communication, thinking and memory, mobility, everyday activities and health and fitness. Each participant receives a manual to work through. The program is co-facilitated by a health professional and a person who has experienced an ABI. Support persons / partners / family members are invited to attend and participate. Topics include: Understanding brain injury. How I look after myself, including stress management, managing risks and goal setting. How I live in the community, including relationships, activities in the community, and working with services. Participants are invited to join a Townsville based social network following the program. Where: 16 Ryan Street in Belgian Gardens. When: Friday from 10.am to 12 noon. 27 Post-discharge Program (Intensive rehabilitation for people recently discharged from hospital) Community Rehab nq offers tailored assessment and intervention programs for people who have been recently discharged from hospital, particularly people who have had a Stroke, Brain Injury, Spinal Cord Injury or Guillian Barre. This program includes individual rehabilitation sessions as well as group programs. A comprehensive assessment is provided by the multidisciplinary team. A 3 5 week intensive program is tailored to the individual's goals and priorities (with the option to extend if indicated). Sessions are provided Monday Friday and may include assessments, individualised training, education and self-management strategies. The team assists participants to identify realistic solutions to achieve their goal. Links are made with each individual's local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. The program is delivered by a multidisciplinary team that includes Physiotherapists, Occupational Therapists, Exercise Physiologists, Speech Pathologist, Registered Nurse, Dietician, Psychologist, Neuropsychologist, Social Worker and an Indigenous Rehabilitation Assistant, and by University students of these professions. Arrangements are made for participants and a companion to stay in accommodation nearby. The program is for people who have a disability as a result of neurological conditions such as Polio, Cerebral Palsy, Stroke or Parkinson's disease. Participants are guided to set their goals, plan how they will achieve their goals and then supported to 'have a go at their goal. A comprehensive assessment is provided by the multidisciplinary team. A 2 3 week intensive program is tailored to the individual's goals and priorities (with the option to extend if indicated). Sessions are provided Monday Friday and may include further assessments, individualised training, education and self-management strategies. The team assists participants to identify realistic solutions to achieve their goals. Links are made with each individual's local services and facilities. Each participant compiles a journal to guide them to work towards their goals at home. Out and about sessions occur after lunch and include visits to the home or community, such as to the shops, art centres, jetty, or beach, riding a bike, swimming or working out at the gym. The purpose of the out and about sessions is to help people with disabilities to participate in family and community. Outreach services to Palm Island run every Tuesday for 5 weeks, in April and October. In between April and October, services are provided every 2 nd Tuesday of the month. Sessions include physical activity, memory and thinking, talking and listening. The 'out and about' sessions focus on what really matters to each person. A thorough assessment is provided at the beginning and end of the program. Links are made with each individual's local services and facilities. Each participant compiles a journal to help them continue to work towards their goals within their home and community.
