Suite #5 Reno, NV Phone: PE Options Packet Washoe County School District (WCSD) high school students are permitted to earn physical education credit by participating in instructor-led group classes at an approved fitness facility as predetermined by Extended Studies. Students must complete 60 hours (3,600 minutes) of instructor-led group classes (the instructor may not be the student s parent or close relative) within the semester to earn.5 credit. Classes under 1 hour may not be rounded up. For example, a 45 minute class does not count as 1 hour. Students may count up to 2 hours of instructor-led group classes per day. Students may earn a maximum of 2 credits (physical education or elective credit) through PE Options. S/U grades are awarded for PE Options and do not affect the student s GPA. The purpose of this packet is to provide the information and forms needed for students to successfully complete the course. The requirements of the course are listed below. 1. The following must be completed at time of registration: Parents need to complete a registration form and pay the $75.00 registration fee for the student to participate in the course. Registration for the course can be completed online or at Extended Studies. Please note that only.5 credit may be earned through PE Options per semester. 2. The following must be submitted to Extended Studies Office within 5 business days after registration has been completed. THESE FORMS MUST BE TURNED IN BEFORE HOURS COUNT. Students must submit proof of gym membership. The gym must be an approved facility. See the approved facility list found on the Extended Studies website. Students must submit the Manager/Supervisor Acknowledgement form to Extended Studies. The Statement of Understanding form must be signed by the parent and student and submitted to Extended Studies. 3. The following must be completed and submitted monthly to Extended Studies. PE Options Attendance Card must be turned in monthly to Extended Studies. The PE Options Attendance Card must be signed by the class instructor after every group class attended and by the manager/supervisor at the end of the month. Originals only. No faxes or scanned s. 2 A group class schedule from the gym must be attached monthly to the PE Options Attendance Card. The classes that the student attended must be circled. Seniors: You must submit the following items listed in #4 and #5 below, three (3) weeks prior to the end of the Spring semester in order to have your grades posted in time for graduation. 4. The following must be submitted two weeks before the end of the semester to Extended Studies. Students must complete and submit a final project. The final project must be submitted two weeks before the end of the semester. Students may choose to turn in an essay (3 pages, MLA format, double spaced, 12 point font size, 1 margins) or a Power Point presentation (a minimum of 10 slides including supporting evidence) demonstrating what they have learned about the activity, himself/herself, and how their experience working out has affected him/her. Student must demonstrate correct grammar, punctuation, and spelling. 5. The following must be submitted by the Thursday before the end of the semester to Extended Studies. Students must have completed all 60 hours and submitted the final monthly PE Options Attendance Card. 2

3 Registration Registration may be completed at the beginning of the desired semester either online, in office, or via mail. Students must register and pay $75.00 registration fee for every.5 credit. Registration must be completed before sessions count. Before registering, ensure you understand all information contained in this packet. Please follow the appropriate steps below for registration: 1. Determine which approved fitness facility you will attend (see website or Extended Studies for a list) and pay the associated membership fees. 2. To pay for the course online: I. Go to click Register, and follow the directions to log in using your WCSD student ID #. II. Find PE Options using the navigation pane on the left hand side of the screen. III. Read the waiver, click the I Agree button, download the required documents, and purchase the item. IV. Payment of $79.05* ($75 + $4.05 non-refundable convenience fee) is due at time of registration via Visa, MasterCard, American Express, or Discover. OR To pay for the course in office or by mail: I. Go to click Register, click Office/Mail Registration, and print and complete the appropriate PDF registration form. This form may also be obtained in office. II. Submit the registration form either in office or by mail and payment of $75* via cash, check, or credit card (Visa, MasterCard, or Discover.) 3. Registration for the course is not complete (and you may not start counting hours) until you submit the following documentation to Extended Studies: a. Proof of gym membership (membership card, bill, etc.); b. Manager/Supervisor Acknowledgement form; and c. Expectations & Signatures page. All documentation must be submitted either in office or via mail (5450 Riggins Ct. Suite #5 Reno, NV 89502). We do not accept faxes or s. Originals only. 4. A 20% processing fee will be assessed on all refunds if cancellation is made within 1 week of registration. No refund will be given if cancellation is not made within 1 week of registration. All refunds are issued via check within 2-3 weeks of request. 3

4 Statement of Understanding PE Options Please initial each item below and complete the information at the bottom of the page to indicate your understanding. This form is to be submitted to Extended Studies within five business days after registration. I understand that BEFORE HOURS COUNT, the registration form, payment, this Statement of Understanding page, Manager/Supervisor Acknowledgement form, and proof of gym membership must be submitted to Extended Studies. I understand 60 hours (3,600 minutes) of participation in an instructor-led group class (the instructor may not be the student s parent or close relative) is required to earn a.5 credit in PE Options. I understand that I must attach a class schedule with my monthly PE Options Attendance Card with the classes attended circled and that a maximum of 2 hours a day may be counted on the PE Options Attendance Card. I understand that if I become injured and therefore am unable to continue with participation in PE Options for a period of time, I must contact Extended Studies to make appropriate arrangements. A doctor s note is required. I understand the WCSD or Extended Studies is not liable or responsible for the student s payment of any fees associated with fitness facility membership. I understand that due dates are based on the WCSD Balanced Calendar and even if I am an Incline High School student, I must meet these due dates. I understand that ALL HOURS are due 1 week before the end of the semester & FINAL PROJECTS are due 2 weeks before the end of the semester. ALL requirements must be completed and submitted to Extended Studies in the semester in which application is made. SENIORS to have all materials and final project in 3 weeks before semester ends. I understand that the final grade will be posted to the transcript at the end of the semester. I understand that the final project must be original and that plagiarism will not be tolerated. I understand that habitually submitting course requirements late, or neglecting to turn them in at all, will result in the issuance of an unsatisfactory grade (U) and no credit will be earned. I understand that I must submit original signed documents; therefore faxes and s are not accepted. I understand there is absolutely no tolerance for abuse of the course and that any student who falsifies their participation in PE Options or forges a signature will earn an unsatisfactory grade (U) and no credit. I understand that students who drop this course after the 11 th week in fall and spring semesters or the 4 th week in summer semester will earn a U. No record of the course will be posted to the student s transcript if course is dropped prior to the 11 th week in fall and spring or the 4 th week in summer. I understand that a 20% processing fee will be assessed on all refunds if cancellation is made within 1 week of registration. No refund will be given if cancellation is not made within 1 week of registration. All refunds are issued via check. Students are only allowed to participate in instructor-led group classes. The Instructor who leads the group class will be required to sign the student s PE Options Attendance Card at the end of the class (the instructor may not be the student s parent or close relative). Students can only count up to 2 hours a day. Before a student can submit his/her PE Options Attendance Card to Extended Studies, it must be signed by the gym manager or supervisor. Students are also required to attach a class schedule to their attendance card and submit the forms once a month to Extended Studies. I understand there is absolutely no tolerance for abuse of the course and that falsifying a student s participation in PE Options will result in the student earning no credit (U) and the facility will be removed from the approved facility list.
