Initial Evaluation: Special Services Evaluation Procedures Initial, Re-evaluation, In State and Out of State 1. Student is brought to CMT meeting to discuss overall concerns 2. Parent is contacted regarding the overall concerns 3. Parent is contacted to schedule the evaluation planning meeting 4. Background information is collected from the parents 5. The evaluation team consults with each other on an on-going basis during the evaluation 6. Reports are reviewed including: o Medical o Psychological 7. The following are conducted: o Observations o Evaluations including: Behavior Cognition Information Processing Adaptive Skills Functional Skills Functional Behavioral Analysis Autism School Psychologist Special Education Case Manager or School Psychologist School Psychologist School Psychologist School Psychologist School Psychologist Licensed School School Psychologist 1 of 6

8. The following are also conducted: o Evaluations including: Assistive Technology Transition Checklist Academic Assessment 9. Sensory information is collected and added to include: o Vision o Hearing o Other 10. All data entered by members of the team is reviewed 11. The following is written and reviewed: o Eligibility determination o Summary of educational needs 12. Meeting is scheduled to review evaluation results and written notice is sent 13. Eligibility determination meeting is facilitated 14. Paperwork is placed into the special education 15. Re-evaluation: 1. Pre-planning is completed for re-evaluations for the school year o Re-evaluation due dates o Coordination of IEPs and reevaluations 2. The following is completed prior to reevaluations: o Student needs are discussed at CMT o Student history is reviewed o Current present levels of academic achievement and functional performance is obtained from classroom teacher and case manager 3. School records are reviewed including: o Progress in school (grades) o State and district assessments o Past evaluations for special education o Referrals to SAT o Other reports in file 4. Re-evaluation meeting is held with the team in order to gather information to develop the evaluation plan School Psychologist Case Management Team School Psychologist Case Manager School Psychologist 5. Evaluation plan is developed Case Manager 6. Background information is collected and reviewed; parent concerns section is completed Case Manager 7. Conduct observations School Psychologist Special Education Teacher General Education Teacher Social Worker Case Manager 8. Conduct evaluation of: o Behavior o Cognition o Information Processing School Psychologist 3 of 6

In State Evaluation Procedure 1. During CMT the team reviews (as soon as records are received) the current evaluation. a. Meets MN criteria: proceed to step #7 b. Does not meet MN criteria: complete steps below 2. Evaluation plan is developed, parents involved in process of developing plan and permission to evaluate. School Psychologist Case Manager Other parties *as needed Case Manager School Psychologist 3. Complete evaluation plan requirements. IEP team 4. Meeting is scheduled to review evaluation results and written notice is sent. 5. Eligibility determination meeting is facilitated to include: 6. Out of State Evaluation Procedure 1. During CMT the team reviews (as soon as records are received) the current evaluation. School Psychologist Case Manager Other parties *as needed 2. Evaluation plan is developed, parents involved in process of developing plan and permission to evaluate. Case Manager School Psychologist 3. Complete evaluation plan requirements. IEP team 4. Meeting is scheduled to review evaluation results and written notice is sent. 5. Eligibility determination meeting is facilitated to include: Case Manager Case Manager 6.
