Employee Benefits Summary Medical and Vision Benefits As a Teva employee, you will immediately be eligible for enrollment in one of our three competitive medical plans. You may choose between the Aetna Value with Health Savings Account (HSA), Aetna Core with Health Reimbursement Account (HRA) or the Aetna Premier plan. Both the Aetna Core with HRA and Aetna Premier plans include a $20 copay (primary physician) and $30 copay (specialist). The Aetna Core with HRA covers 80% for in-network services (after the deductible has been satisfied). Both the Aetna Premier and Aetna Value with HSA cover 85% for in-network services (after the deductible has been satisfied). The Aetna Core with HRA has an in-network deductible of $1,500 employee only/$3,000 family. Teva will automatically provide funding to the HRA to help you pay for your deductibles, coinsurance and copays. The Aetna Premier plan has an in-network deductible of $500 employee only /$1,000 family. The Aetna Value Plan with HSA has an in network deductible of $1,750 employee only /$3,500 family. Teva will automatically provide funding to the HSA to help employees pay, and save, for eligible medical expenses. Teva rewards employees who pursue healthier lifestyles. Why? It s a smart decision for them, their families and our company. All three medical plans offer in-network preventive care at 100%. And, for 2015, employees and their spouses/partners can earn additional money in funding from Teva for participating in wellness programs. Vision coverage, which provides one routine eye exam every year, has a maximum allowance of $200 for lenses and frames every two years, per covered member. Prescription Benefits Once you enroll in one of the three medical plans with Aetna, you are automatically enrolled in the prescription benefit with CVS Caremark. As a Teva employee you will enjoy a $0 copay for Teva branded and generic drugs, $5 copay for non-teva generic drugs, $25 Brand Preferred and $40 Brand Non-Preferred at retail. With CVS Caremark, you have the option of receiving your maintenance medications via mail order or through the Retail Maintenance Program. Dental Benefits As a Teva employee you will immediately be eligible for enrollment in Teva s Delta Dental plan. The Delta Dental plan includes in-network preventive care at 100% coverage and additional coverage for basic and major restorative services. In-network orthodontic treatment for adults and children is covered at 50% of the treatment costs, up to $2,000.
401k Plan Teva s 401(k) Plan allows employees to make pre-tax or Roth contributions of up to 75% of eligible income. Teva will match your contributions, dollar for dollar, up to the first 6% of your salary. You are immediately vested in Teva s matching contributions. Paid Time Off Teva offers competitive paid time off benefits includ ing vacation, 13 holidays, maternity and paternity leave, adoption leave and assistance, military leave, jury duty, and bereavement, among others. Defined Contribution Teva will provide an annual defined contribution to your 401(k) account of 3% of your annual salary, even if you do not make contributions. Participants are 100% vested after 3 years of service. Employees must work 1,000 hours during the plan year and be employed on the last day of the last payroll period of the year to be eligible. Employee Stock Purchase Plan (ESPP) Teva s ESPP allows you to invest up to 10% of your eligible post-tax earnings to purchase common shares of Teva Pharmaceutical Industries stock at 85% of market value. All employees are eligible to participate. Teva is committed to the continual development of our employees. Business Travel Accident Insurance When traveling on business, Teva provides an additional AD&D benefit to employees. The policy also includes security evacuation and emergency medical benefits when traveling outside of the home country. This benefit is at no cost to you. Flexible Spending Accounts (FSA) Teva provides a Health Care Flexible Spending Account and a Dependent Care Flexible Spending Account. The Health Care FSA allows employees to contribute up to $2,500 and the Dependent Care FSA allows employees to contribute $5,000 per account on a pre-tax basis for eligible expenses.
Tuition Assistance and Individual Development Teva is committed to the continual skill development of our employees. While the responsibility for professional development rests with each individual, the Company provides reimbursement of approved tuition and associated eligible expenses up to $7,750 per calendar year. The first $5,250 per calendar year is exempt from federal tax. The remainder is subject to federal tax withholdings. Eligibility for assistance begins after 1 year of service. Additionally, Teva has a wide range of learning and development resources to support individual growth and career management. Short-Term Disability Teva s Short Term Disability coverage protects your income in the event of non-work-related injury or illness. The plan pays 100% of your weekly salary up to eight weeks after a seven-day waiting period at no cost to you. During weeks 10 through 13, the plan pays 66.67% of your weekly salary if you are unable to perform the duties of your own occupation solely because of an injury or illness. Long-Term Disability Teva s Long-Term Disability benefits begin at the end of the Short-Term Disability coverage. If you qualify for Long-Term Disability benefits, the plan pays 60% of your gross base earnings, at no cost to you, up to a maximum monthly benefit of $25,000. Basic Life and AD&D Insurance As a Teva employee, you are automatically covered, at no cost to you, under Teva s Basic Term Life Insurance plan. The plan will pay your beneficiary two times your annual salary (rounded up to the next $1,000), up to a maximum of $1,000,000. If your death is the result of a covered accident, the plan will also pay your beneficiary an additional benefit of two times your annual salary up to $1,000,000. In the event of dismemberment in relation to a covered accident, you will receive a benefit based on an established schedule of benefits. Optional Life Insurance Employees may also purchase optional life insurance in increments from $50,000 to $1,000,000 - not to exceed five times your annual salary. Any dollar amount above $300,000 will require Evidence of Insurability. Employees may also purchase Optional Spouse Term Life insurance up to half the amount elected for yourself. Also available, Optional Dependent Child(ren) Term Life insurance for either $5,000 or $10,000 coverage per child. WorkLife/Employee Assistance Program (EAP) Teva s WorkLife/EAP plan provides confidential help for a range of issues including your health, your family s health, stress-related concerns, and even financial planning. This benefit is available whenever you or your family needs it.
