(1 of 5) Park Hill Surgery Center (PHSC) Job Description TITLE: Operating Room Registered Nurse JOB SUMMARY Provides nursing care to the patient during the intra-operative phase. Performs nursing activities in the operating room where the circulating nurse is the patient's advocate. Answers patient needs by assisting the physician and person responsible for anesthesia care. RELATIONSHIPS Reports to: Director of Nursing/Clinical Coordinator Supervises: Licensed Practical Nurses, Operating Room Technicians, other members of health care team as assigned QUALIFICATIONS Graduate of accredited school of nursing required Current state licensure required Operating room experience required CPR/ACLS certification required MENTAL DEMANDS Require immediate judgment and response; occasionally in response to disasters or emergencies, actual events or drills. Require language skills adequate for high-level written and interpersonal communication in American English. PHYSICAL DEMANDS Require high-level visual and auditory acuity for timely response and patient care assessment activities. Require the ability to move quickly in response to physician and patient needs, to stand for long periods, occasional overhead and low reaching, and to exert force up to 50 lbs. occasionally, up to 20 lbs. frequently and/or up to 10 lbs. constantly. Patient care activities require the ability to exert force of up to 30 lbs. constantly, up to 40 lbs. occasionally and up to 100 lbs. rarely. OCCUPATIONAL EXPOSURE Category I exposure to blood borne pathogens; frequent exposure to chemical hazards; may encounter radiation hazards. ESSENTIAL FUNCTIONS AND OBJECTIVES 1. Adheres to safety policies and procedures in performing job duties and responsibilities. 5.1. Reports observed or suspected violations, hazards, and noncompliance according to Facility policy and procedure to the Safety Officer or other designated person. 5.2. Observes safety measures in performance of job duties and responsibilities; Corrects immediately and responsibly, safety issues or problems which staff is qualified to address (wet floors, side rail down) 5.3. Uses proper body mechanics while transporting patients to the OR via stretcher and while positioning patient for surgery. 5.4 Responds to emergency situations with competence and composure. Is familiar with alarm sounds and can differentiate alarm tones. Is familiar with the crash cart, its contents, and where to obtain necessary supplies and is knowledgeable of location of eye wash stations and spill kits. 6. Interacts appropriately with various age groups. 6.1. Accurately assesses and interprets age-specific patient data. 6.2. Accurately interprets age-specific patient responses to questions and instructions. Takes

(3 of 5) into consideration age related variables e.g., hearing, visual and motor deficits. 6.3. Involves the patient's family/significant other in decision-making about the patient's care. 6.4. Provides care appropriate to the patient's age group. 6.5. Accurately applies knowledge of growth and development. 6.6. Considers age-specific patient requirements when responding to emergency situations. Provides interpreter or employee for language barrier patients 7. Organizes time, equipment, supplies, and staff to provide effective case management. 7.1. Arrives promptly to designated work area and begins work related activities. 7.2. Supervises and directs patient care in a single operating room. 7.3. Assures OR surfaces are dusted with disinfectant prior to bringing supplies into OR. 7.4. Assures settings are correct on microscope and phaco machine for appropriate surgeon. 7.5. Verifies proper functioning of equipment including zoom on microscope, suction, oxygen and assures clock time on monitor correlates with time on atomic clock. 7.6. Verifies correct lot numbers written on board for the day. 7.7. Prepares BSS bottles with correct amount of epinephrine for surgeon using sterile technique. 7.8. Assures correct ocular meds, intracameral, BSS bottles are in the OR. 7.9. Assures correct supplies are available for scheduled procedure. 7.10. Assures correct equipment in OR for scheduled procedures including surgeon s chair, headlight, lasers, cryo, etc. 7.11. Organizes nursing activities efficiently and effectively delegating activities appropriate to the abilities of available staff members. Utilizes down time effectively by assisting other team members with their preparations for day, assisting pre/post team, and performing tasks that would otherwise wait until the end of the day. 7.12. Considers cost-containment by using supplies economically and in effective turnover time. 7.13. Assists with supply, drug, and equipment inventories to maintain stock level and availability. 8. Addresses patient needs during admission to the operating room with attention to requirements specific to the outpatient setting. 8.1. Verifies correct patient, correct surgery correct site with informed consent on the patient s chart prior to transfer to OR.. 8.2. Confirms allergies and NPO status prior to transfer to OR. 8.3. Checks lab values if appropriate prior to transfer to OR. 8.4. Verifies correct type of anesthesia has been completed, e.g. topical or block anesthesia. 8.5. Obtains record of pre-op vital signs. 8.6. Transports patients to the operating room according to Facility policy, safety requirements, and patient needs. 8.7. Verifies vital signs appropriate to initiate surgery. 8.8. Identifies the patient and correct surgical intervention according to Facility policy and communicates with all surgical team members. 8.9. Informs surgical team of pertinent patient information may affect plan of care, e.g. allergies, disease processes, positioning difficulties, claustrophobia, hard of hearing, monocular, etc. 8.10. Provides comfort and reassurance to the patient; maintains patient's privacy and dignity.

(4 of 5) Discourages inappropriate conversations in OR; monitors volume of conversations in the OR to promote safety and professionalism and assure patient s can hear safety instructions. 8.11. Explains intra-operative phase routine procedures and care to the patient. 9. Performs the circulating nurse's functions adhering to approved operating room procedures for the scheduled procedure. 9.1. Prepares operating room according to the procedure to be performed and patient needs. Reviews surgical schedule at beginning of day to prepare for any unusual cases scheduled. 9.2. Assembles and provides properly functioning equipment, supplies, and instruments before start of procedure; positions and adjusts equipment as required during the procedure. 9.3. Anticipates needs of the surgical team; assists anesthesia staff by supporting the patient during administration of anesthesia. 9.4. Assists with the proper and safe positioning of the patient and prepares the operative site as directed by established procedure. 9.5. Is proficient with operation of autoclaves and the removal of sterilized instruments from autoclave. 9.6. Monitors compliance with principles of aseptic technique according to accepted standards. 9.7. Assists with identification and confirmation of operative site. Assures time-out is observed. 9.8. Assures all measures are taken to assure correct surgical site and order prior to draping of patient. 9.9. Verifies correct procedure(s) to be performed. 9.10. Verifies correct implant to be used when appropriate. 9.11. Provides for comfort in the physical environment by monitoring designated parameters. 9.12. Has knowledge of EKG interpretation and reports any changes in heart rhythm, heart rate, blood pressure or O2 saturation to CRNA and / or surgeon when appropriate. 9.13. Provides continuity of patient care by communicating with other perioperative care personnel and documenting records appropriately. Provides a report to the post-op nurse to include procedure, any changes in vital signs, medications given, or additional new post-op orders that may vary from the normal discharge instructions. Helps expedite transfer of patient to recliner by helping with patient possessions, removal of hat, booties and assisting with transfer as needed. Assists patient to recliner and initiates first vital signs until post-op nurse is available to receive report. 9.14. Returns to OR to expedite room turn over when appropriate or transfers next patient to OR safely and efficiently per Facility policy. 10. Responds in a timely manner to meet the needs of the patient and physician. 10.1. Accurately comprehends and interprets verbal orders and direction. 10.2. Documents patient care records and other forms accurately according to Facility policy. 10.3. Accurately advises others responsible for patient care about the patient's status. 10.4. Documents in log book according to Facility policy: patient name, procedure, operative site, method of anesthesia given, OR #, name of surgical tech, name of circulating RN, name of CRNA, and if applicable, any complications or labs that have been sent. 11. Performs end of day functions.

(5 of 5) 11.1. Assures OR is restocked for next surgical day, including lake side cart and drawers. 11.2. Assures OR is cleaned and closed according to Facility P&P. 11.3. Assures all unused equipment and supplies are removed from OR / sub-sterile area and put away in their appropriate areas. 11.4. Assist in pulling cases for next surgical day as needed; asks when unsure. 11.5. Assure medications needed for next surgical day are available. 11.6. Assure daily / weekly duties on checklist have been completed and documented. 11.7. Assists with PM narcotic count as needed. 11.8. Assists other teams as needed to complete all end of day functions.
