Our Center Community Westview Hospital's Acute Rehabilitation Center and programs are specially designed to meet the needs of our patients and their families. We hope that this literature will provide you and your family with a complete overview of our nursing care and rehabilitative therapies. Our Center is a Medicare-certified facility, whose emphasis is on assisting our patients so they can reach their highest level of functional independence. We offer comprehensive, 24-hour inpatient rehabilitation in a 12-bed dedicated rehab unit for patients who demonstrate impairment in activities of daily living (ADL) skills, mobility, self-care, communication, along with medical complexity. Our Philosophy Our philosophy of care is focused on preventative medicine, healthy lifestyles, treating the root cause of illness, and caring for the whole person. Restoring your body, mind and spirit after suffering a serious illness or injury is a great challenge. The process of recovering from a traumatic injury, illness or disability can be long and challenging. We are here to help you overcome these challenges and reach your rehabilitative goals. We provide each patient with three or more hours of individual therapy (no less than five times per week) in a facility of all private rooms, a comfortable activities area and dayroom, with access to a beautiful outdoor rehab courtyard. We provide physical, occupational, speech and recreational therapy programs and an on-site hemodialysis inpatient unit. Our Rehab Team Our rehabilitation team consists of licensed nurses and licensed physical, occupational, speech-language and recreational therapists, and licensed social workers all with rehab experience and skills. Our Center is under the medical direction of a physiatrist, a physician who specializes in physical medicine and rehabilitation. Our goal is to help each patient achieve the highest degree of independence possible. Should you require immediate medical attention during your stay, Community Westview Hospital is directly connected to our Center - with access to 24/7 emergency services, hospital medical and surgical services, and a medical staff consisting of multiple specialties. Our goal is to help our patients rehabilitate, restore and start living The goal of our rehab team is to help you become as strong and independent as possible, while reaching your fullest potential. We attain this goal by providing optimum care, strategies, motivation and the encouragement you and your caretakers need for you to be as self-sufficient as possible. Using input from the entire rehab team, as well as from you and your family, a plan of care is developed that is specifically tailored to meet your needs so that you can rehabilitate, restore and start living. Acute Rehabilitation Criteria Patients may be referred to our center from any setting - a hospital, home, or extended care facility. The referral or request to assess

for inpatient rehab must be made by a physician. Once the order for your rehab assessment or screening is made, the case manager at the hospital, or extended care facility where you are staying, will contact the referral coordinator at Community Westview Hospital s Acute Rehabilitation Center. A representative from our facility will visit you and evaluate your medical condition, ability to perform certain physical activities, your endurance and what your plans would be after discharge following our center. Based on this information, we will be able to determine if our center is the best choice for your rehabilitative care. If you are at home and your physician feels you are a candidate for our center, you or a family member may contact us to schedule a home visit and evaluation. Requirements for Admission To be eligible for admission, you must: Have a physician s order to be evaluated for a rehab program Require 24 hour nursing care Require two or more different therapies Be able to tolerate three or more hours of rehab therapy services no less than five days a week Have the potential to improve function Have a plan to discharge into a family situation (such as home), or one that offers supervision or help with care if needed such as an assisted living environment. You and your family members are also critical members of this team. We believe that patient and family education as well as training is key to rehabilitation success. Family conferences (when needed) provide an opportunity to review the medical history, evaluation results, short and long-term goals, progress and discharge planning. Length of Stay and Discharge Depending on the injury or illness, the average length of stay is 7 to 21 days. Medicare covers most stays as long as you have a qualifying diagnosis (a diagnosis approved by Medicare). The number of days is influenced by your insurance company, your individual needs, the advice of your physician and input from the therapy team. Input from you and your family also help determine your discharge date. Some Medicare and insurance companies may require a co-pay. This means that they will pay a percentage of the stay and you are responsible for the remainder. You will be advised of the co-pay before admission. Your length of stay will be influenced by your own individual needs, progress toward goals and insurance approval. The advice of your physician and the various departments involved in your care, along with input from you and your family will help determine your discharge date. You will be discharged on the written order of a physician. You and your family members(s) will have plenty of notice in order to make arrangements. Representatives from therapy, nursing, dietary and social services will work together to educate you about how to continue the things you have learned in therapy, how to follow your diet and how to take your medications or perform any treatments that may be necessary. Any outpatient therapies, home health care and equipment needs, as well as follow-up doctor visits, will be arranged before you leave.

Rehabilitation Therapies Physical Therapy: The therapist and patient set goals leading toward improvement in strength, endurance, mobility and selfconfidence. Occupational Therapy: O.T. helps functionally link together the activities of daily living learned through the rehabilitation process. Speech & Language Therapy: Provides treatment for patients with language, voice and swallowing disorders, as well as cognitive memory disorders, which can affect one's ability to function independently. Recreational Therapy: Recreational therapy uses functional leisure tasks to facilitate physical and cognitive rehab as well as increase social and emotional well-being. Renal Care: Our nephrologists offer diagnosis and treatment for renal diseases including acute renal failure (kidney failure), fluid and electrolyte abnormalities, acid-base abnormalities, hypertension (benign and malignant), gout, nephrotic syndrome, evaluation of renal stones (kidney stones), evaluation of hyponatremia, hyperkalemia, hypercalcemia and related topics. Hemodialysis: Our center offers people living with kidney failure a convenient treatment option without having to leave our facility. Kidney failure is increasing nationally at a rate of 10 percent to 12 percent each year. In fact, more than 230,000 Americans undergo dialysis each year. Hemodialysis is used to clean the body's waste products from the blood after the kidneys have failed. The blood travels through tubes to a dialyzer, a machine that removes wastes and extra fluid. The cleaned blood then goes back into the body.
