Opt-in Schemes - a unique concept Opt-in Schemes from General & Medical have been designed to complement our existing range of company paid Private Health Insurance Schemes. Opt-in gives employees and members of associations access to our wide range of covers and our first class, personal service but at discounted rates. By offering your employees or members the opportunity to opt-in to healthcare, you can enhance your overall employee benefits package at no extra cost to your organisation. You can offer the benefits of discounted health insurance and enjoy the rewards of a happier, healthier workforce whilst attracting and retaining the best people. It is in your interests to promote the Opt-in Scheme through your website or intranet. We will provide you with a branded link to your own area of our website. But remember, our Opt-in Schemes place no administrative or cost burden on you. All you have to do is complete an application form and leave everything else to us! Opt-in is flexible, allowing you to offer discounted private health insurance and generate an income for your organisation in the form of a commission for every policy taken out. For example, many sports clubs have set up Opt-in Schemes and a popular use of this revenue stream is to spend it on youth related activities to encourage the next generation of players. Similarly, we work with charities that value the income as additional donations. It is easy and free to set up, requiring no administration on your part. Additionally, Opt-in Schemes do not give rise to PIID benefits, as each employee is responsible for paying their own premiums. Equs Range - essential affordable cover The Equs range is designed to provide the benefits of private healthcare, at the lowest cost. Because of this, the Equs range has proved very popular with individuals and families, who may not have considered private healthcare before. Equs Key Equs Key provides an essential level of cover. Members have fast access to an initial consultation and diagnostic tests to find out very quickly whether something is wrong. They ll also be covered if they need to be admitted to a private hospital for surgery of an acute condition. Equs Key may suit you if you are looking for an essential level of in-patient cover only. Equs Equs has all the benefits of Equs Key but it includes an essential level of cover for the treatment of cancer that increases for each of the first five years of membership, if the member doesn't make a claim for cancer. In addition, within certain limits, there is out-patient cover for therapies, complementary medicine and cardiovascular conditions. Equs Plus Equs Plus offers more protection. There are higher limits for out-patient benefits and more comprehensive cover for therapies and complementary medicine. Perhaps more importantly, Equs Plus provides in-patient cover for cardiovascular conditions. There is also a higher level of cover for the treatment of cancer that increases for each of the first ten years of membership, if the member doesn't make a claim for cancer. 5 here to help at www.generalandmedical.com

Altus Range a comprehensive choice The Altus range is our most popular range of schemes. The range is designed to provide an excellent choice of comprehensive cover at a price to suit most budgets. Whether the member needs to go to hospital as an in-patient or a day-patient, or to see a specialist as an out-patient, they are guaranteed access to some of the best care available. Altus Key Altus Key provides fast access to initial consultation and diagnostic tests, so members can find out very quickly whether something is wrong. Comprehensive cover is also given for admission to a private hospital for surgery of acute conditions. This includes a level of cover for cancer treatment that increases for each of the first ten years of membership, if the member doesn't make a claim for cancer. Altus Altus may be the right choice for you if you need comprehensive cover for in-patient care and you are happy to accept out-patient benefits which are subject to certain limits, in return for a lower monthly premium. Of course, cover is provided for cancer treatment including radiotherapy and chemotherapy. In addition, with access to our GP advice line 24 hours a day, members will be able to discuss any health concerns at any time, without having to trouble their own doctor. Altus Plus Altus Plus provides all the benefits of Altus and more. There are higher limits for out-patient benefits but there are also useful additional benefits such as cover for a private ambulance and home nursing. A life cash benefit is also included and there are cash benefits for personal accident, temporary disablement and critical illness. Importantly, Altus Plus also provides members with medical cover outside of the UK. If they are taken ill or injured overseas, Altus Plus is designed to meet the medical costs and the cost of transportation back to the UK for urgent medical attention. Sports Range winners in the field Our range of Sports covers is one of the most comprehensive on the market. In fact, our experience in the sports health insurance arena is second to none making General & Medical an increasingly popular choice with sports clubs and players, up and down the country. Our Sports Healthcare range has been specifically designed for amateur and professional sports people. In addition to cover for sports injuries, players and clubs can benefit from unique features such as an additional team-patient benefit and cover whilst touring as well as complementary services such as our treatment booking service for non-eligible treatment. Years of experience in the sports healthcare market has enabled us to hone the range of schemes we offer to ensure the benefits are useful and realistic whilst remaining affordable. We price our schemes responsibly to make sure that premium levels are sustainable for the long term. Unlike some other Sports Healthcare insurers, we offer the full range of underwriting options so it s easier and fairer, for those already insured elsewhere, to switch to General & Medical. Our claims teams are also tuned in to the specific needs of sports players and clubs. This includes knowledge of the hospitals and consultants throughout the country, renowned for providing the best facilities and treatment for sports injuries. Participating Hospitals - more choice A vital part of any private medical insurance scheme is the available medical facilities, in which treatment may be given. With General & Medical, whichever level of cover is chosen, members have access to over 450 of some of the best private hospitals and medical facilities throughout the UK. Hospital Choices At General & Medical we have one of the largest selections of hospitals and medical facilities compared to any UK health insurer. We call these our Participating Hospitals and they include the majority of the leading national private hospital groups and the NHS private patient units, giving access to the most modern facilities available in the UK. Most of our Participating Hospitals provide a single private room with en-suite facilities for in-patient accommodation, together with a choice of menus, flexible visiting times and other facilities consistent with a modern private facility. This gives members timely access, privacy and the dedicated care and comfort they deserve. The cost of treatment between different facilities can vary enormously, so we have grouped our Participating Hospitals to give three levels of choice. In this way, we are able to reflect the lower cost of treatment at certain facilities by reducing premiums according to the hospitals chosen. First Choice Our First Choice Hospitals are a selected group of over 150 private hospitals at locations throughout the UK. Those who choose our First Choice Hospitals benefit from our lowest monthly premiums. Freedom Freedom Hospitals give access to many additional hospitals throughout the UK, both NHS and private. Also, where Freedom Hospitals are chosen we provide members with a cash benefit of 100 for each night spent in one of our First Choice Hospitals. If treatment is undertaken as a day-patient at one of our First Choice Hospitals, we will also pay 100 for that day. Premium Premium Hospitals are specialist facilities that due to their reputation and location are able to charge substantially more for their medical services. Implementing New Opt-in Schemes and the support we give you Implementing an Opt-in Scheme for your organisation is a very straightforward process. There are no awkward complications for Accounts and Human Resources departments and the scheme does not give rise to a P11D benefit for members. All you need to do is inform your employees about the availability of the scheme. To ensure awareness of the scheme we will support you with a range of branded media, at no cost to you. For example: 1 Posters for display in communal areas. 2 Promotional leaflets or emails that can be circulated throughout your organisation. 3 Company intranet hyperlink or image linking directly to General & Medical s Opt-in website. The Opt-in service is delivered through an online quotation and application process so all information about the scheme is available to members through a special area on the General & Medical website. There are no brochures to circulate or forms to fill in. The discount agreed for members will automatically apply to quotes generated. Any information including key facts and application forms are available for printing from the General & Medical website, so it s easy for people to apply for cover. After an application has been processed and accepted, all policy documentation will be sent direct to the applicant. Finally, as part of the implementation process, you will be sent log in details to a secure area of our website where you can: 1 Tell us about any changes to your contact details. 2 Access statistical information about the scheme such as the number of people who have taken out cover and the type of cover applied for. 3 View the number of people who have visited the Opt-in site to obtain quotations. 9 here to help at www.generalandmedical.com

Frequently Asked Questions Will I have to pay the monthly fees and then reclaim the money back from my staff or member? No. When a member joins the scheme the contract is between General & Medical and the member. Will I be responsible for distributing policy documentation? No. General & Medical will send all policy documentation direct to the member. What age do people have to be to join the scheme? Individuals can join the scheme between the ages of 18-75. However, once on cover there is no upper age limit. Anyone below the age of 18 can only be accepted as part of a family membership. What will I need to do to allow my employees to join the scheme? After your Opt-in Scheme is set up. All you need to do is inform employees that the scheme is now available. We can supply branded posters and leaflets to help you as well as advertisements for your website or intranet. Must all employees or members apply for the same type of cover? No. Each employee or member may choose any level of cover. The only restriction is that each 'family group' must have the same level of cover. Do my employees or members have to have a medical to qualify for cover? No. They do not need to undergo a medical or fill in a complicated medical questionnaire. They must, however, have been registered with an NHS GP in the UK for a minimum of 60 months. What are the main benefits for my organisation? The main benefits are that you will be able to offer an excellent benefit to your staff or members and you may also earn an income stream based on membership levels. Will I have to get involved if there is a problem with a member's or employee s policy? No. All queries about claims or membership will be sorted out directly between General & Medical and the member. Will staff or members have to stop their health insurance policy if they leave my company or organisation?
