Health Maintenance Reports A Health Maintenance report lists clients that fit a user defined set of criteria. It is used so that a specific activity can be performed and tracked. Once a HM report is generated per provider, letters can be produced and phone calls recorded for the list of clients. When a HM report is run, an alert can be created in the client s chart. Creating Health Maintenance Profiles In order to create a HM report, a profile detailing the search criteria and clinical parameters is created and can be used over and over. To create a HM Profile per provider, go to the Office Actions Setup Health Maintenance Profiles. 1. Click on New Nightingale On Demand Reporting Manual Page 4

2. Provide a name for your profle 3. Select the Add to Patient Alerts checkbox to create an alert for each client that meets the profile criteria. 4. Choose the sex of the clients you are running a report for (Male or Female; Male; Female) 5. Select whether this profile is set for clients of any age or a specifc age and if the later specify the age range. 6. You can narrow your profile search by entering in various other demographic information; shown in previous image. 7. Check the Rostered check box if you only want a report of just your clients flagged as primary care clients. 8. You can specify the Patient Statuses you are searching for by selecting the status on the left and clicking Add. Adding Parameters Health Maintenance profiles offer any combination of the following parameters that the client has or does not have as part of their chart. Activities (appointments) Allergies Diagnosis (in the CPP the problem list) Family History Immunizations Injections Lab Results Medications Procedures Social History Vitals If you select multiple items of the same parameter, you have the ability to define relationships by not selecting the required check box. Items selected from different parameter types are always treated as an and statement. You can add a parameter to your profile by clicking on New Parameter. 1. Select from the drop down of available parameters. Each parameter will change the options available. 2. Select Patient Has or Patient Has Not. 3. Once you have completed your first parameter Click Save. You can add additional parameters by clicking on New Parameter and completing the above 3 steps. Once you have completed adding all parameters you can click Save on your profile. Note: you can not go back and alter your profile once saved, you would need to change the name of the profile and create a copy to alter or update current profiles. Creating HM Reports Health Maintenance reports are run per provider using the Health Maintenance Profiles. In order to create and run a HM report you must have the HM profile created first; outlined in the previous section of this manual. Health Maintenance reports are run by going to Office Actions Administration Work Queue Health Maintenance. Nightingale On Demand Reporting Manual Page 6

1. Click New 2. Provide a name for your report 3. Select a profile for the HM report, you will have a drop down of all the profiles set up for that particular provider. 4. Click Save. Once you save your report it runs and the results are displayed and managed from the Report Results page. Once you run the report that same report can not be run again, you can only review the results for that particular report by clicking on Results. Nightingale On Demand Reporting Manual Page 7

Tracking Client Response to a HM Alert Running a HM report can create a Health Maintenance Alert in the clients chart from Patients Details Summary. The client s response to the particular HM alert can be tracked by clicking on Done then choosing from the drop down if they have accepted; declined; no response. When a patient response is recorded, the HM alert is saved to the Archive Summary page found under Options. EMR Adoption Report This report will run on a per provider basis for a defined time period. The report will provide a summary of the number of clients in the provider's care (based on the provider listed as a client s responsible provider, broken down by gender and age, as well as a summary of activities recorded for those clients. To access this report go to Reports Operational EMR Adoption Nightingale On Demand Reporting Manual Page 8

Ad Hoc Reports The AdHoc reports allow you to create point in time specific reports based on dats from the schedule. You can run these reports from Reports Operational AdHoc Reports. 1. Click New to create a new ad hoc report 2. Type the name of the report 3. Add the desired filters in the Available Filters boxes and click Add. When the Selected Filters box is empty, you are applying ALL the filters. Nightingale On Demand Reporting Manual Page 9

Note: The New and Returning Patient Type filter is pulled from the appointment screen. If the New check box is checked there, the patient will show up as New in this report. 4. Click the Date Range tab at the top of the report to set the following types of dates: Appointment Date select an Appointment Date if you want to see scheduled appointments Booking Date select the date that the appointment was booked. Your options in both sections are: No Filter, Fiscal Year, Fiscal Quarter Range, Date, Generic Range, Current Month, Current Year. Note: If you are going to use the fiscal year date options, first set the Fiscal Year Start and End fields. 5. Select the Report Fields Tab to set the fields to be displayed once the report has run. Select the Choose Field drop down arrow and select the first column in your report. A new dropdown displays. 6. Repeat selecting the fields which will become your columns of the report. Note: the bottom selections: # of Visits, # of Patients and # of No Shows are calculators from within the system. The options that display when selecting one of these three choices are: Total, Count, Average, Min, Max and Percentage. 7. Click Save to save the report and run it at a later time. Click Save and Run report to save and run the report immediately. 8. Click the Calendar icon to set up a schedule for when the report runs. Select when, start and end date and click Save. You can edit Ad Hoc reports by clicking on the name of the report to open it and make changes. Click on the Hx (History) button to see the times and dates on which the report has run. Patient Roster Report There are two reports within this section; one listing all rostered clients per provider; the other is a rostered invitation report that lists all clients per responsible provider that is not marked as rostered (primary care client). For this Patient Roster report choose the provider, enter a date range, and click Search. For the Roster Invitation report choose the provider and click Generate Report. Nightingale On Demand Reporting Manual Page 11
