Faculty of Nursing Graduate Programs PROGRAM RECOMMENDED AWARDS APPLICATION 2015/1016 Academic Year APPLICATION DEADLINE: April 6, 2015 at 12:00 pm Completed applications must be sent as an email attachment to nursgrad@ucalgary.ca Instructions: Donors endow the Faculty of Nursing with funding for awards, and determine the criteria which are defined in the terms of reference for each award. You must meet these terms when applying for each award and you can apply for as many as you qualify. How to apply: 1) Please indicate how you meet the terms of the award(s) for which you are applying by checking off each of the terms of reference for each award. 2) If an additional statement is required as one of the terms, please enter the statement in the box provided. 3) If a criterion requires that you be a resident of Alberta or Calgary, you must be such a resident during the tenure of the award. That is, if you are from another province and you will be moving to Calgary to attend studies, then you are eligible to apply for the award. 4) Have your supervisor review your application for his/her guidance on how to strengthen it. 5) Indicate on the application that your supervisor has reviewed the application. Please allow the supervisor at least two weeks notice, paying heed to the application deadline. If you are applying for admission to the program and do not have a supervisor, please send your application to nursgrad@ucalgary.ca and the Graduate Program Office will have a faculty member review your application for input. 6) Complete the main essay at the end of the application. It is recommended that you read the full terms of reference for each award, to assist you with the content of your essay.
