Be able to read, write, and speak English proficiently. 2. Have completed a Highline College Application for Admission New Highline students must complete a Highline College Application for Admission You can fill out the application in-person in building 6. You can also fill out the application online (please print off the confirmation email). http://admissions.highline.edu/admissions.php Nursing Assistant Certified Application Steps: 1. Complete the attached Nursing Assistant Certified application. 2. Provide evidence of a clean Washington State Patrol criminal background check. (see below) 3. Provide evidence of a negative Tuberculosis (TB) test reading (within the past year) or current chest x-ray/symptom check reading (within the past 2 years). 4. Provide payment in full for the course upon submission of application. If you are receiving funding, you will be required to submit a letter from the funding source stating you have been approved for tuition assistance. All application materials should be submitted to the Continuing Education office in building 99, room 101. Incomplete or illegible applications will not be considered. A reading/math assessment will be administered upon application submission. Criminal Background Check: Students cannot enter the Nursing Assistant Program without providing a clean Washington State Patrol background check. If you have questions about the types of convictions that would exclude you from the program, call Washington State Patrol customer service at (360)705-5100 or check the list online at http://app.leg.wa.gov/wac/default.aspx?cite=388-113&full=true. 10/17/13 Page 1

Clinical sites will perform an additional national background check that includes any previous addresses within the last 3 years. Failure to pass this additional background check will prevent you from completing the program. To obtain your Washington State Criminal Background Profile, choose one of the following options: 1. Go to http://www.wsp.wa.gov/crime/crimhist.htm to request a Criminal History report. Print your results. The cost is $10.00 (price may vary) payable online (requires a credit card). 2. Pay $16.60 (price may vary) to Highline Community College Cashiers office and deliver the receipt to the Campus Security office with a Criminal History Search request form. Include self-addressed stamped envelope and the search results will be mailed to you OR arrange for pick-up at security. Tuberculosis (TB) test: A primary care physician or community health center can provide this service for a fee. The fee will vary depending on the provider. Highline College Nursing Assistant Certified (NAC) Training Program FAQ s The NAC course prepares students to take the state exam. The state exam is NOT given at Highline College. After successfully completing the course, students will need to apply for the state exam (additional fee required). Information regarding this process will be provided in class. Who can apply for the Nursing Assistant Certified Training Program? Anyone over 16 years of age can apply for the Nursing Assistant Certified Training Program. You must complete an Underage Student Waiver Form and submit it to Highline College s Continuing Education Department in Building 99, Room 101. However, to be employed as a Nursing Assistant, you must be 18 years of age or older and have a high school diploma or a high school diploma equivalency. What are the costs associated with the NAC program? SELF PAY OTHER FUNDING SOURCES $595.00 $776.60 ( for in-state tuition)** Certificate of Completion 7 College Credits and Certificate of Completion Both the $595.00 and $776.60 fee include the required $57.35 Medical/Liability Insurance, Nursing Supply Fee and Building Fee. **Out-of-state tuition is based on residency status and 7-credit equivalency. Nursing Assistant Supplies (Refer to pages 5-6) Other associated testing & licensing fees (optional-see below) $110 NACES WA State Testing Fee $48 WA DOH License Fee What is the next step after successfully completing the Nursing Assistant training course? The next steps are optional, but highly recommended. You may choose to take the WA state test to become certified as a Nursing Assistant. Your training is valid for one year. Your instructor will discuss this process during week three of class. In order to register for the NAC test, each student will complete a WA NACES application and submit their money order of $110 to NACES plus Foundation. Once you submit both your paperwork and payment to NACES and that paperwork has been processed, NACES will assign you a test date. If you pass the test successfully, you will be considered certified. 10/17/13 Page 3

We will also assist you with The Department of Health Nursing Assistant registration. The cost to register with the Department of Health (DOH) is $48. Paperwork and the $48 money order will be mailed and processed by DOH. This registry will allow employers to verify your Nursing Assistant credentials. What types of financial assistance/funding are available? The Nursing Assistant Certified Training Program does not qualify for traditional Financial Aid. Students can use other funding sources to pay for the class if eligible. Non-Highline funding sources may be eligible for the $595.00 self-pay fee (those wanting college credit must pay $776.60**). Highline related funding sources must pay $776.60**. What supplies are required for the NAC Program? Burgundy uniform top/bottom 2 sets are required. There are various places you can purchase scrubs just a few examples are the Highline bookstore, Uniform Destination in the Auburn Supermall, and Burlington Coat Factory. Gait Belt the gait belt can be purchased in the Highline bookstore (building 8), second hand stores like Goodwill or Salvation Army, or any medical supply retailer. Continuing Education Nursing Assistant Certified Training Program Funding Application Steps 1. Do you have a Nursing Assistant Application Packet? 2. Nursing Assistant Packets are available online or in person at: nursingassistant.highline.edu Or in Building 99, Room 101 Continuing Education Department Workfirst (TANF) orientations are every Friday in Building 10, Room 101 from 12PM-1PM. WES orientations are every Thursday in Building 10, Room 103 from 1pm-3pm. (Effective 07/16/15) *It is required to attend an orientation session to be eligible for Workforce or Workfirst funding. 3.
