CHO Award - Application Process The application is evaluated along four themes: Management the organizational aspects of EHR implementation; strategy, planning, project management and governance; Functionality delivered by the EHR to meet the organizational objectives and the needs of patients and end-users; Technology technical design and architecture that enable the EHR to deliver the required functionality and performance; and Value and Return on Investment value-concrete as well as intrinsic value derived from the implementation of the EHR. 10

Davies CHO Award Eligibility Prior to completing the application, applicants are encouraged to validate their eligibility: Test of time Test of EHR-based, technology-enabled care delivery Organization-wide adoption test Infrastructure/sustainability test 11

CHO Award - Application Options Option 1: A single community health organization applies individually Option 2: One or two CHOs that participate with a collaborative entity apply with a description of how the collaborative entity supported each CHO in EHR implementation and adoption Option 3: Three or more qualified CHOs that participate with a collaborative entity apply in partnership with the collaborative entity 12

CHO Application Process: Qualify The CHO must Provide primary medical care and preventive health services for the population served Maintain an open-door policy Accept patients regardless of their ability to pay 13

Davies CHO Application Process: Bare Bones CHO must conform to the following guidelines: Meet the qualification criteria Who Can Apply for the Davies CHO Award? Meet the application pre-requisites Submit an Individual CHO Identification Form Submit a letter of support for the Davies Award application signed by the CEO or Executive Director If a collaborative entity is included in the application, submit the CHO/Collaborative Entity Partnership Identification Form 16

Davies CHO Award Tips & Tricks Applications are reviewed by independent peer group (Davies Committee) with years of experience (former Davies CHO recipients and other subject matter experts) Avoid cutting and pasting functionality from a website Provide insight into how the new EHR functionality is being used to improve patient care delivery 17

Davies CHO Award Tips & Tricks Assemble a dedicated team to develop the application Application team Leader Support for the application from leadership Have all facets of CHO contribute, as this may roll over to the site visit 18

Davies CHO Award Tips & Tricks Application should be well organized & detailed Write the application in the same order and format as the application guidelines Include novel/innovative areas even if they are not addressed in the application (e.g. interface to billing) Be conscientious of style, grammar, spelling, flow 19

Davies CHO Award - Tips & Tricks The Value Section Avoid being vague clearly articulate qualitative and quantitative benefits as well as lessons learned/challenges. Benefits may include but should not be limited to: Safety, Effectiveness, ROI, Efficiency, Regulatory Compliance, Access to Care, User Satisfaction. Please explain what you are presently doing in conjunction with your vendor to prepare to meet CMS s proposed criteria for meaningful use? Public Health Award - Application Process The Award recognizes the use of person- and population-level data for public health issues, and ideally, its impact on population health. It does not recognize care delivery. Vendors can provide assistance to the applicant organization, but cannot apply for the award themselves. Public health informatics researchers should provide assistance in submitting the application. 25

Public Health Award - Criteria An electronic public health system should be capable of Electronically collecting data Distributing data to appropriate program areas Providing data in formats ready for use by public health professionals, and Gathering responses by public health professionals Ability to share data electronically with other appropriate systems in a standardized manner 26

Public Health Application Process: Bare Bones The Davies Public Health Award recognizes excellence in an organization s effective use a public health electronic information system using the following criteria: 1. The system is in routine use and has been incorporated into all phases of the system s operation and practice. 2. The system has been implemented long enough to show strong evidence of measuring and improving the health of a defined population, not individual care. 3. The system provides data currently acted upon by public health officials. 4. The system shares data in a standardized and secure manner with other public health systems. 30
