HIE Helps Coordinate Care Referrals and transitions of care generate and send care summaries from the HIE (ie, patient discharged from hospital to hospice care) New patient prep - use HIE to search for medical history on new patients to prepare for appointments Results delivery query the HIE or receive results directly into EHR for existing patient test results to better inform clinical decisions Emergency care coordination EMS personnel and hospital emergency dept physicians use HIE to find medication allergies or other pertinent clinical data, such as recent test results or evidence of doctor shopping 8

Use of the CORHIO system helps reduce the need to send information about the transferred patients by more primitive methods such as paper, faxing and email and helps expedite patient care. - Gary Hall, Estes Park Medical Center 22

For patients that are complicated and may not know their entire history, having another source of information is incredibly helpful. If I see a patient that has abnormal labs, for example, and they can t tell me if they ve been in the other hospital, and that information is in CORHIO. What s New at CORHIO Ambulatory CCD Exchange Providers can send care summaries into the HIE new product in development Right now, we are taking them in, we are working on making them searchable and viewable Part of the Colorado Care Connections Program for Medicaid providers and other grant opportunities When this is true exchange and viewable in CHR, ambulatory data will be seen by specialists, vice versa 34

What s New at CORHIO Imaging Results Health Images first non-hospital radiology service provider to join CORHIO Includes locations in Colorado Springs and metro Denver Real-time access to radiology reports and high-quality images when a patient of a CORHIO participant has an MRI, CT Scan or other radiology test performed at Health Images Reports will be available through CORHIO s PatientCare 360 portal or via EHR 35

What s New at CORHIO Colorado Care Connections Program Funded by Health Care Policy and Financing (HCPF) to assist Medicaid providers in connecting to the HIE CORHIO will be assisting practices identified by HCPF with: How to integrate their workflow around the HIE Meeting Meaningful Use Improving their electronic Clinical Quality Measures (ecqms) HIPAA policies and procedures tied to connecting to the HIE 36
