College Application Worksheet Instructions: Students, complete this College Application Worksheet and bring it with you to your school s College Application Day. You may need to ask your family, school counselor, or another adult you trust to assist you in answering all of the questions. Your English teacher, school counselor, and other adults you trust are good resources to assist you in getting your essays to a final version. You should bring a final, proofed, electronic copy of your essays and/or personal statements on a flash drive to the event, or email them to an email address that you can access from a school computer. Test Scores Use the worksheet below to list the scores for tests you have already taken. If you have not yet taken the SAT or ACT, speak with your school counselor about upcoming test dates. This could include clubs, organizations, volunteering, or sports. List your activities below, including your position and whether you received an award or recognition related to the activity. It would be good to include volunteer information here as well: College Application Information Many colleges will ask for information regarding your college plans. Be sure to do research prior to your school s College Application event regarding whether the colleges you are interested in have the major you want to study. If you don t know what you want to study, that s okay! You can work with your school counselor while you re still in high school to do career exploration and there will be resources on college campus to assist with this as well. You can likely apply with undecided, but do your research before College Application Day to make sure that is an option at the schools to which you plan to apply. Regardless, you can also change your major once you are enrolled in college too. Students who were eligible for fee waivers to college entrance exams, such as the SAT or ACT, are usually also eligible for college application fee waivers. Some Indiana colleges

Page 5 offer free applications to all students. Visit www.learnmoreindiana.org to find out what application fees are at each of Indiana s colleges public and private (non-profit) colleges. Not all colleges accept fee waivers. Of those that do, the most common are: ACT fee waiver (if you were eligible for a fee waiver for the ACT; your counselor can fill out the form at www.act.org/aap/pdf/act-userhandbook.pdf) College Board fee waiver (if you were eligible for a fee waiver for the SAT; your counselor can learn more at http://professionals.collegeboard.com/guidance/applications/fee-waivers or by calling 888-SAT-HELP (728-4357).) National Association for College Admission Counseling (NACAC) fee waiver (if your counselor agrees that you one of the requirements such as family income, receiving public assistance, being an orphan or ward of the state, or meeting other criteria; you or your counselor can learn more at www.nacacnet.org/studentinfo/feewaiver/pages/default.aspx) 21 st Century Scholar fee waiver (if you are a 21 st Century Scholar; almost all public colleges accept this fee waiver and some private colleges do as well.
