Welcome to Amerigroup Kansas! We re happy you re with us. We cover all of your KanCare benefits. Plus we offer fantastic extra benefits to help you and your family stay healthy. Use this booklet to find out all about each of our extra benefits. You ll be able to see which benefits you re eligible for and instructions on how to access them. If you have any questions about the benefits listed, give us a call at 1-800-600-4441 for more information.

Preventive Dental Care With Amerigroup, you can get adult preventive dental care to help you stay healthy. You ll get: Two cleanings every year Scaling and polishing to remove plaque, tartar and stains All Amerigroup members who are 21 or older How to get dental care In order to get preventive dental care, call your dental provider to schedule an appointment. For a list of dental providers in our network, call Scion Dental at 1-855-866-2627. You may get your teeth cleaned once every six months. Stop-smoking Program We can help you quit smoking for free! Our smoking cessation program includes: Unlimited calls to a health coach for tips and advice Online programs and written materials to help you meet your goals Nicotine replacement therapy (NRT) products like: Gum Lozenges Patches All Amerigroup members who are 18 or older How to join the free stop-smoking program There are two ways you can enroll in our stop-smoking program. 1) If you re interested in the program, call Member Services at 1-800-600-4441. Let us know the best way to contact you. A health coach or representative will call you back and give you more information about how to start the stop-smoking program. 2) If you qualify for the stop-smoking program, your care manager or disease manager may ask you if you d like to participate. If you do, we will give you information on how to get started. Healthy Families Program Our Healthy Families program is designed to help families with overweight or obese children live healthier lives. The six-month program will give your family small, simple steps you can take to improve your family s health. You ll learn about the importance of good nutrition and activity with: Access to a health coach you can call to get health tips and advice Educational materials to help your family learn about wellness and healthy living Referrals to local nutrition classes for 6-8 weeks (where available) All Amerigroup families with children ages 7-13 who are one of the following: Overweight Obese At risk for becoming overweight or obese How to start the Healthy Families program If your family qualifies for the Healthy Families program, we may call you and ask if you d like to participate. If you do, we ll give you all the information you need to start the program. If you don t get a call from us and you d like to join the Healthy Families program, call Member Services at 1-800-600-4441. Let us know you d like to be a part of the program. If your family qualifies, we ll help you get started. Real Solutions Healthy Rewards Program You can earn $10, $15 or $25 just for doing things that are good for your health. It s simple. All you have to do is enroll. Then, you ll get dollars credited to your very own Healthy Rewards 3

debit card every time you get certain health checkups or screenings, like: Well-child checkups Cancer screenings Diabetic exams Prenatal care Flu shots You can use your card for approved items from Dollar General and Family Dollar. You can buy a variety of items, such as: Baby care products like food, diapers, bottles, clothes, etc. Children s medicines Diabetic supplies Digestion medicines Everyday goods such as body lotions, mouthwash, shampoos, bath products and more Eye care supplies Family planning products like condoms, fertility kits and contraceptives Grab bars, scales, apnea monitors and other home health items Pain relief medicines Wellness items like nutrition bars, sports drinks and vitamins Women s care needs such as tampons, panty liners and antifungal creams Just take your card to one of the approved local retailers, choose the items you d like to buy and use your Healthy Rewards card to pay for them. All Amerigroup members How to enroll in the Healthy Rewards program To enroll in the Healthy Rewards program, visit myamerigroup.com/healthyrewards or call 1-877-868-2004. Let us know you d like to enroll in the Healthy Rewards program. We ll send you everything you need to start earning rewards. Coupons for Savings at Local Retailers You can get discounts just for being an Amerigroup member! We will send you a coupon booklet that contains some great deals to use at retailers near you. Using your coupons is easy: 1. Look at the list of stores and discounts in your booklet. 2. Cut out the coupon you want to use. 3. Visit the retailer and give the coupon to the cashier when you check out. 4. Enjoy your savings! All Amerigroup members who are 18 or older How to get your coupon booklet If you d like savings at some of your favorite local retailers, call Member Services at 1-800-600-4441. Tell us you d like the coupons. We ll send you the booklet in the mail. You can get one booklet per household per year. Free Mobile Phone Service Amerigroup is partnering with SafeLink Wireless to offer you a special mobile phone service. You can get a free cell phone and up to 250 free monthly minutes to call your family, friends and doctors! Plus, we ll give you these great extra services just for being our member: 200 bonus lifetime minutes Unlimited text messages to or from anyone Unlimited free calls to Amerigroup Member Services Free optional health text messages to give you: Education and tips about your health Reminders about checkups and flu shots for you and your family Weight management information 4

All Amerigroup members are eligible. Members with an existing Lifeline plan will be allowed to opt out of their current service in order get the Amerigroup benefit. Coverage may not be offered in certain remote service areas. How to get your SafeLink mobile phone plan To enroll in the SafeLink plan and get your free phone, call SafeLink at 1-877-631-2550 or enroll online at www.safelink.com. You can call this same number to find out if this benefit is offered in your area. If you d like to learn more about the mobile phone benefit before you sign up, call Amerigroup at 1-877-269-5707. This benefit is available to one member per household. If you are an Amerigroup member who is under 18 and would like to get this benefit, you will need your parent or guardian to enroll for you. Weight Watchers We know it can be hard to lose weight, and we know how to help. We ll give you a Weight Watchers voucher you can use for four weeks of free classes. And we ll pay your initiation fee, too! You ll learn how to manage your weight in fun, new ways. You ll get: Private weigh-ins Helpful tools and tips to help you reach your goals Recipes and meal ideas to help you and your family stay healthy All Amerigroup members who are 18 or older How to get your free Weight Watchers voucher Call Member Services at 1-800-600-4441 and let us know you d like the Weight Watchers voucher. If you are eligible, we will send you your voucher in the mail. Each member may only order one Weight Watchers voucher. Hypoallergenic Bedding You deserve a healthy and comfortable night s sleep. If you have allergies or certain other conditions, we can help you get hypoallergenic bedding. You can get up to $100 for the calendar year to use toward special bedding items, like: Pillows Pillowcases Mattresses Mattress covers Sheets Blankets All Amerigroup members who have been diagnosed with one of the following: Asthma Allergies Chronic respiratory or pulmonary conditions A doctor must recommend/order the product. A care manager s authorization is required. How to get hypoallergenic bedding If you need hypoallergenic bedding to help you sleep, call Member Services at 1-800-600-4441 and let us know. First, we ll check to make sure you are eligible. If you are, we ll ask you to choose which bedding products you want to order. After you decide, we ll let you know when you should expect to get your new bedding. Free Pest Control Amerigroup can help you get rid of unwanted pests. If you qualify for this benefit, we can treat your home for the following pests: Ants Bedbugs Crickets Fleas Mice Rats Roaches Silverfish 5

Spiders Stinging insects like wasps and hornets All Amerigroup members who are in waiver groups and certain members on SSI are eligible. Members must own their own home to qualify. Members residing in ICF/MR, assisted living and nursing facilities, group home or similar settings are not eligible for this benefit. A care manager s authorization is required. How to get pest control treatment If you have unwanted pests in your home, call Member Services at 1-800-600-4441. Let us know what type of pests you have. If you are eligible, we will make sure you get the treatment you need for your home. You may get up to four treatments per year. The maximum benefit is $500 per calendar year. Transportation to Providers for Caregivers If you do not have a ride to eligible medical appointments, your transportation may be covered through KanCare. Amerigroup knows that some of our members may need their caregivers to go, too. We can make sure your caregiver has a ride to go with you to doctor s visits when you need him or her. All Amerigroup members who are in waiver groups and certain members on SSI How to get transportation for your caregiver If you need your caregiver to go to your KanCare covered medical visits, call Access2Care at 1-855-345-6943 at least three days before your scheduled appointment time. You will need to let them know the date, time and location of your doctor s visit. If you qualify, they will let you know what to expect when it gets closer to your appointment time. If you have complex medical needs or need help setting up your ride, tell your care manager or service coordinator. He or she can help you set up transportation for your caregiver. Your caregiver may get up to 24 round-trip rides per year. For emergency transportation, call 911. Transportation for Members to Community Events Amerigroup partners with community organizations, schools, faith-based groups and local businesses to help improve the lives of people in the communities we serve. Community events are a great way to learn about your health, meet people and sometimes even get free gifts. That s why we are offering transportation to members who need help getting to health events nearby. For members in waiver groups and certain members on SSI, we ll give you rides to approved events sponsored by organizations like: Witness Project of Kansas Susan G. Komen American Red Cross American Heart Association American Diabetes Association All Amerigroup members who are in waiver groups and certain members on SSI How to get a ride to a local community event Call Access2Care toll free at 1-855-345-6943 to set up your ride to a health or wellness event in your community. You should call at least three days before the event. The representative will want to know the date, time and type of event you d like to attend. If you are eligible to get a ride, he or she will tell you everything you need to know to set it up. 6

If you have complex medical needs or need help scheduling your ride, tell your care manager or service coordinator. He or she can help you set up the transportation you need for health events in your area. You may get up to 24 round-trip rides per year to community events. For emergency transportation, call 911. Amerigroup will only pay for rides to events that we approve. Transportation to the following event types will not be covered: Festivals Concerts Sports games Entertainment events Car shows County fairs Markets Mail Order Over-The-Counter (OTC) Drug Benefit Our OTC drug benefit makes it easy to get what you need to stay healthy. If you qualify, Amerigroup will give you up to $10 each month in OTC products. All you need to do is order the items you want, and we ll have them delivered right to your doorstep. You can get products to meet a variety of needs, like: Acne treatments Anti-itch products Cough and cold medicines Dental care Vitamins Pain relievers All Amerigroup members who are in waiver groups and certain members on SSI How to order your OTC products If you are eligible for this benefit, we will send you a catalog in the mail that lists all the products you can order and instructions on how to get them. Once you get your catalog, choose the product(s) that meet your needs and place your order. Order Online log in to our member website at myamerigroup.com/ks Order By Phone call Member Services at 1-800-600-4441 (TTY 1-800-855-2880) If you think you may be eligible for this benefit and you don t get a catalog in the mail, call us at 1-800-600-4441. Our Member Services representative will be able to tell you if you qualify. If you do, we ll send you an OTC catalog. Respite Care For members in the Frail Elderly waiver group, Amerigroup will give up to 56 hours of respite care per year. Respite care is extra caregiver support so that your regular caregiver or personal care attendant can take a break. Respite care may be used if your caregiver is ill or has a family emergency. That way, he or she can take some time off, and you can keep getting the care you need. All Amerigroup members who are in the Frail Elderly waiver group. Excludes: Members living alone Members residing in ICF/MR, assisted living and nursing facilities, group home or similar settings How to get respite care Please tell your service coordinator or care manager if you would like the respite care benefit. He or she can give you the information you need to set up respite care to help you and your caregiver. Extra Respite Care Respite care is extra caregiver support so that your regular caregiver or personal care attendant can take a break. Some members may already be eligible to receive respite care through KanCare. Amerigroup 7

wants to make sure you are getting the care you need. We offer extra respite care on top of what KanCare covers. Members in the Intellectual or Developmental Disability waiver group get: 15 extra days of overnight respite care per year Members in the Autism waiver group get 24 extra hours of respite care per year. All Amerigroup members who are in the Intellectual or Developmental Disability or Autism waiver groups. Excludes: Members living alone Members residing in ICF/MR, assisted living and nursing facilities, group home or similar settings How to get extra personal assistant services Please tell your service coordinator or care manager if you would like the extra personal assistant services benefit. How to get extra respite care Please tell your service coordinator or care manager if you would like the extra respite care benefit. He or she can give you the information you need to set up respite care to help you and your caregiver. Extra Personal Assistant Services For members in the Intellectual or Developmental Disability waiver group, Amerigroup will give three extra days of personal assistant services to help support members health and well-being at home and in the community. Personal assistant services means getting help from personal assistants with tasks such as: Activities of daily living like bathing and grooming Independent activities of daily living like shopping, housecleaning and meal planning Support services like community and recreational activities All Amerigroup members who are in the Intellectual or Developmental Disability Waiver group 8
