PROGRAM OVERVIEW Current Topics in Care Management is designed to provide care (case) managers with the enhanced knowledge and skills that can be immediately applied as part of a multi-disciplinary approach to care management and psychosocial support. Developed and led by recognized experts in the field, each interactive workshop provides a unique opportunity for participants to learn and discuss key issues and challenges in supportive patient care. Who Should Attend The workshop series is open to care managers with an interest in enhancing their knowledge and skills in a broad array of supportive interventions for patients with cancer and other chronic conditions. Why Attend Despite the advancements in treating cancer, asthma, and other chronic illnesses, patients and their families are often challenged by significant burdens across the continuum of illness. Current Topics in Care Management will provide care managers with current information and focus on approaches to help patients and their families overcome these barriers through interactive discussions with colleagues and leading experts in the field. Workshop Format Each of the one or two-hour workshops is available for onsite delivery and/or via live webcast. With a blend of presentation, case studies and Q&A, attendees will have the opportunity to address their organization s specific challenges and share best practices. Note: Current Topics in Care Management is not a continuing education activity; therefore continuing education credits will not be issued by Genentech and Genentech may not assist attendees with applying for continuing education credits independently. SUPPORTIVE CARE WORKSHOPS Supporting Members with Advanced Illness: The Role of Care Managers in End-of-Life Care Care managers play a unique role in supporting patients as they transition in care. As many patients with life-limiting illness and their families face key decisions, care managers are often the source of vital information and central to the team in supporting their care. This workshop focuses on the skills and knowledge necessary for the care manager to support patients and their families with establishing goals of care and effectively advocating for those goals. This program reviews communication techniques, discusses the important distinctions between palliative and hospice care, and addresses the management of physical and psychological symptoms that often accompany advanced illness. Identify the challenges that care managers encounter and the skills utilized to improve end-of-life care Distinguish the differences between palliative and hospice care and when each are appropriate Define the unique role of the care manager in advocating and communicating with the healthcare team, patient and family Describe the care manager s role in supporting patients and families to: Identify realistic goals when assessing patient s needs and priorities Transition to hospice care Describe and assess the common grief responses among patients and families Current Topics in Care Management 4

Managing Moderate- and Severe-Persistent Asthma: An Educational Program for Care Managers Asthma is a serious chronic illness that affects nearly 26 million people in the US. 1 Although asthma can be managed with proper treatment, adherence to medication, and other preventative strategies, it continues to be a significant burden on patients, caregivers, and the healthcare system. Through a case study approach, this program will offer information and supportive strategies for care managers to help their patients identify and avoid their asthma triggers, properly use their delivery devices, and monitor their symptoms through an action plan. This program also demonstrates motivational interviewing techniques to enhance the interactions between care managers and their patients to facilitate improved outcomes. Describe the characteristics of moderate and severe-persistent asthma Identify features of asthma control and the factors that contribute to uncontrolled asthma Understand the multifactorial approach for enhancing asthma control Differentiate between quick relief and long-term control therapies Demonstrate accurate technique in the administration and care of inhalational devices Implement a symptom-based or peak flow-based asthma action plan Review supportive strategies for enhanced adherence 1 Center for Disease Control. ONCOLOGY CARE MANAGEMENT WORKSHOPS Cancer care has dramatically and rapidly advanced in the past two decades. It is increasingly important for the care manager to understand how these advances impact members with cancer, their families, and the multi-disciplinary team who deliver care. The oncology-focused workshops provide a unique and comprehensive educational opportunity for care managers to: Understand key challenges facing today s cancer patients Focus on strategic approaches to providing supportive care to their patients Discuss current issues in oncology supportive care with their peers and leading experts Enhance their healthcare team s ability to provide support and care to their patients with cancer Foundations of Oncology Once a diagnosis of cancer is made, patients are often inundated with an abundance of information and face difficult decisions and challenges regarding the treatment and management of their illness. This workshop provides care managers with an understanding of the key aspects of cancer diagnosis, staging and determining an appropriate plan of care. It explores strategies for supporting patients unique physical, emotional, and practical needs through the cancer experience. ONCOLOGY CARE MANAGEMENT WORKSHOPS (cont.) Palliative Care Cancer patients often experience distressing physical symptoms, psychological distress, impaired life function and family stress. Early intervention with palliative care may help alleviate suffering and improve quality of life for these patients. This workshop presents key concepts in palliative care, foundations of palliative care, and interactive case studies to enhance communication and coordination of patient care. Describe the differences between palliative care and hospice care in a variety of clinical settings Identify selected types of palliative care services and models using a case study Select strategies for initiating goals of care discussions with patients and families Identify at least one strategy to accomplish this week to improve coordination of care for people who benefit from palliative care Current Topics in Care Management 7

Psychosocial Issues in Cancer Care Feelings of fear and anxiety, loss and depression, family distress, sexual dysfunction, and job and financial problems not only lower quality of life but can also profoundly affect physiological functioning and response to treatment. This workshop will address the potential impact that psychosocial and behavioral factors can have on quality of life and health outcomes. Care managers will be provided with strategies for effectively identifying and managing these issues. Discuss the concept of distress as it relates to cancer and cancer treatment Describe the importance of psychosocial assessment and intervention for people with cancer Identify common psychosocial issues throughout the phases of cancer Differentiate normal reactions from acute psychiatric/ psychological problems Current Topics in Care Management 8

Cancer Survivorship As the number of cancer survivors in the US continues to grow, so does the need to address the emotional, physical and economic issues experienced by many of the country s nearly 14 million cancer survivors 1. This workshop will help care managers better understand and assess the challenges associated with cancer diagnosis, treatment and long-term survival. Through case examples, the workshop will explore the benefits of a survivorship plan, psychosocial interventions and supporting healthy choices for cancer survivors. 1 American Cancer Society. Addressing Challenges in Oncology Care A Case Study Approach The complexity of cancer care raises a host of issues affecting patients and their families, from decisions regarding treatment to the myriad of psychological, social, and spiritual needs that can impact overall quality of life. For care managers, identifying and addressing their patients unique health needs and concerns are crucial and often difficult steps in the care process. This workshop was developed from issues identified by care managers as their most challenging, including acute symptoms, palliative care, persistent pain, psychological distress, family caregiver burden, cultural barriers, survivorship and end-of-life concerns. Participants will explore effective assessment techniques and other evaluation and management strategies for these areas of support.
