The two purposes of CPS intervention are to protect the safety of children and to provide services to help families with any difficulties that cause the safety of their children to be at risk. While every attempt is made to preserve the family, if a child's safety cannot be controlled, then court intervention, including removal of the child, may be necessary. 2 l Page

The primary goals for the program are for participants to eliminate substance use and abuse, improve social functioning, build strong families, and develop a comprehensive support network within one s family and community. It includes individual, group, and family counseling within a highly structured environment. The recovery program assists in the positive development of personal problem solving, decision making, and general coping skills for patients with addictive behaviors. Choctaw Children and Family Services Indian Child Welfare program works on behalf of Choctaw children and families in accordance with the Indian Child Welfare Act (ICWA) of 1978. Our social workers join with state workers and the courts in working to insure safe and healthy homes for Choctaw children removed from their homes. This includes finding Choctaw foster and adoptive homes to prevent placement in non Indian homes. Youth Outreach 219 North Broadway (877) 285 6893 or (580) 326 8304 The Youth Outreach program was established in 2005 and our goal is to assist young Choctaw students in finding solutions for hardships that may occur throughout adolescent and teen years. The program also assists in goal setting, plan development and preparation for the future. Range of services include: mentoring, after school activities, college preparation and home and school visits to clients. Women, Infants, & Children Program (WIC) Durant Office P.O. The nutrition program provides foods that are high in iron, protein, calcium, vitamin A and vitamin C. WIC sites are located throughout the Choctaw Nation. 4 l Page

Health Services Behavioral Health (800) 349 7026 The Department of Behavioral Health concentrates on the short and long term care and treatment of clients who have mental health and substance abuse issues. Common treatment issues include depression, anxiety, panic, addiction, abuse and other challenges to living a stable and full life. Main clinic is located in Talihina but services are offered in other satellite clinics within the Choctaw Nation. It is the center of the Nation s health care services covering 10 ½ counties of southeastern Oklahoma. There are four out patient clinics and a diabetes wellness center in/or near the hospital. Boasting a multi specialty medical staff, the hospital renders care through both in patient services and 150,000 out patient visits annually. Outreach Services Adolescent Family Life Project 219 North Broadway (877) 285 6893 or (580) 326 7758 The program provides services to pregnant and parenting teens under 19 years of age, their infants, male partners, and families who reside in the 10 ½ county service area of the Choctaw Nation. The project provides comprehensive and integrated services to reduce repeat pregnancies, improve health outcomes for mother and infant, increase healthy parenting skills, provide opportunities for education and job training, and increase healthy relationship skills. 5 l Page

Being a community based program, services are taken directly to the community and assistance is given there where families are more comfortable. The mission of the program is to help Choctaw families find permanent solutions to long term problems by initiating change, setting goals, making a plan and following through. The program s primary goal is to assist Choctaw families in locating resources to help meet immediate needs. Community Health Representative (CHR) 219 North Broadway (877) 285 6893 ext. 6094 This program is designed to assist Choctaws and other Native Americans maintain the health of themselves and their families. The CHR Program can do many things to assist clients such as taking vitals, glucose monitoring for diabetes, hypertension and cholesterol diet education, patient education, maternal and child health, drug and alcohol abuse counseling, patient assessments, medical referral follow up care, as well as the provision of many other health care services. Faith Based Counseling/Services for Victims of Crime 219 North Broadway (877) 285 6893 or (580) 326 8304 Program provides faith based counseling to restore the balance and harmony in the lives of crime victims. Through the collaboration of spiritual leaders, victim service providers, and traditional healers, the program works to overcome the violence, and abuse within our native families who reside within our 10 ½ county service area within the Choctaw Nation. to provide effective legal aid to victims who are seeking relief in legal matters arising as a consequence of abuse or violence. Project C.H.O.I.C.E.
