Fill out, complete and return the Application for Admission Form. 2. The applicant must be 21 years of age or older. 3. Pay the 50 application fee to be sent in with Admission Application 4. The applicant must provide evidence for 2 years of work experience. 5. A statement of purpose explaining why this degree would enable you to meet your career goals and how this specific program (Master s or Doctoral) enables you to reach those goals. 6. For those applying to a Master s program, the applicant must have a bachelor s degree with a minimum of 2.5 GPA or 2.5 in the last 60 credits. 7. If applying to a Doctoral program, a Master s degree with a minimum GPA of 3.0 is required for admission. 8. The applicant must submit 3 recommendation letters (Students who have completed bachelor s degree from UNAD Colombia will be exempt from this requirement). 9. All foreign degrees must be evaluated, validated and translated. Our preferred servicer is http://unad Florida.jsilny.com. [ ] Monthly Tuition Deferral Plan Students may choose to have their tuition and/or fees automatically charged to their credit, debit, wire transfer on a specific date of each month. Automatic credit card payments are charged after the start date of each course; otherwise, full payment must be received on or before the start date of each course. Credit card or bank information will be requested of students selecting this option in order to establish credit for this plan. Maximum semesters on this plan are four at any one time. TOTAL SALES PRICE The total cost of your purchase on credit including your down payment of WHEN PAYMENTS ARE DUE Beginning on / / and on the same day each (check one) weekly or bi-weekly thereafter [ ] Tuition Reimbursement Plan Tuition Reimbursement may be selected as a payment option if your employer reimburses a portion of your annual tuition to you. Tuition is deferred a maximum of 30 days after grades are received, regardless of when you receive payment from your employer. Applicable fees are due on or before the start date of each course. To qualify, you must submit a signed Tuition Reimbursement Certification and Authorization Form and include a valid credit card number with authorization to charge the card. This card will be charged once the 90 day deferment period expires if any balance remains on the account. Documentation required for the Tuition Reimbursement Plan: Tuition Reimbursement Certification and Authorization Form [ ] Company Direct Tuition Reimbusement Plan If a student works for a company that will pay us directly after students submits grades, we can defer payment until one month after grades are received. If company forfeits agreement, student will have to pay forfeited semester and choose another option for subsequent enrollment. Page 3 of 5

Contracts are not sold to a third party at any time. There are no carrying charges, interest charges, or service charges connected or charged with any of these programs unless stated. CANCELLATION AND REFUND POLICY Should a student s enrollment can be terminated or cancelled for any reason. Students may request a refund in person or via email by filling out a Withdrawal/Cancellation Form. All refunds will be made according to the following refund schedule: 1. All monies will be refunded if UNAD Florida does not accept the applicant, or if the student cancels within (3) three business days after signing the Enrollment Agreement and making an initial payment. 2. Cancellation up to and including the 10th business day after classes start will result in a refund of 100% of the monies paid, with the exception of the application fee. 3. Cancellation on or after the 11th business day after classes have started will result in no refund. 4. Refunds will be made within 30 days of receipt of Cancellation Notice. 5. Any publication acquired by the student through UNAD Florida, including textbooks, is nonrefundable. For courses where digital materials are provided in lieu of textbooks, an 100.00 fee is applied directly to the student's account concurrent with the charge for tuition. The Course Digital Materials (CDM) fee is fully refundable if a student does not attend beyond Week 1 of a course and did not download any materials for the course. After this time, the fee becomes non-refundable. Students are not charged the CDM fee for repeated coursework if previously charged. 6. The Technology Fee is fully refundable if a student does not attend beyond Week 1 of a course. After this time, the fee becomes non-refundable. Students are charged the Technology Fee for repeated coursework. GRADUATION REQUIREMENTS Page 4 of 5
To be awarded an undergraduate degree from UNAD Florida, a student must fulfill the following requirements: Successfully complete his or her chosen program of study as it appears in the catalog Successfully complete the language requirement by passing English Proficiency Exam (if not a native English speaker). Have a cumulative GPA of 3.0 or above. Be under no outstanding financial obligations to UNAD Florida. EMPLOYMENT ASSISTANCE UNAD Florida does not assist its students to obtain employment. However, we do offer advice, send out grade transcripts, and, where appropriate, letters of recommendation written by its instructors. We also send out periodic emails with job fair information. UNAD Florida cannot guarantee job placement. Graduates may encounter employment limitations due to language, market constraints and other economic variables. ACKNOWLEDGEMENT This document and the catalog constitute a binding contract between the institution and the student and no further modification or representation except as herein expressed by both parties will be recognized.
