As Delaware s community health record, submission of information to Public Health (e.g. immunization registry information) must go through DHIN, as the functionality becomes available.

DHIN & MU Attestation Support, Core Items Immunization: Successful ongoing transmission of immunization data Immunization Registry DHIN Immunization Submission Process Practices, pharmacies, hospitals and other organizations may now submit their immunization records to DPH electronically, directly from their EMR, through DHIN. Since program launched in October, over 55K immunization records from 25 organizations have been transmitted through DHIN to the DPH Immunization Registry, or 33% of DPH immunization registry updates. Another 45 organizations are in the process of being on-boarded to this new immunization delivery process. 15
