Today s Key Points 1. DHIN offers a solution for any Meaningful Use measure involving moving data from an EHR to Public Health, another provider, a patient, etc. 2. As Delaware s community health record, submission of information to Public Health (e.g. immunization registry information) must go through DHIN, as the functionality becomes available.

DHIN & MU Attestation Support, Core Items Summary of Care: Provide summary of care documents for more than 50% of transitions of care and referrals with 10% sent electronically & at least one sent to a recipient with a different EMR vendor or successfully testing with CMS EMR Community Health Record DHIN/ProAccess, V6.X, Release Q1 2014 Access to up to 6+ years of medical results, ADT s and transcription summaries. CCD s available on the current version. 14

DHIN & MU Attestation Support, Core Items Immunization: Successful ongoing transmission of immunization data Immunization Registry DHIN Immunization Submission Process Practices, pharmacies, hospitals and other organizations may now submit their immunization records to DPH electronically, directly from their EMR, through DHIN. Since program launched in October, over 55K immunization records from 25 organizations have been transmitted through DHIN to the DPH Immunization Registry, or 33% of DPH immunization registry updates. Another 45 organizations are in the process of being on-boarded to this new immunization delivery process. 15

DHIN & MU Attestation Support, Menu Items Syndromic Surveillance: Successful ongoing transmission of syndromic surveillance data Syndromic Surveillance DHIN Immunization Submission Process Organizations are now submitting their syndromic surveillance records to DPH electronically through DHIN. 16

DHIN & MU Attestation Support, Menu Items Imaging Results: More than 20% of imaging results are accessible through certified EMR technology Community Health Record DHIN/ProAccess on MU Certified Technology Nanticoke and Bayhealth expected to launch with a link from image result reports to the actual image (non-diagnostic quality) this summer. MU certified software launch Q1 14. DHIN EMR Integrations 13 Certified EMR Integrations Current DHIN EMR integrations include Allscripts, STI, Cerner, GE Centricity, ecw, GEMMS, NextGen, ADS, Info Quest, Bizmatics/Prognosis, Care360, Connexin, Micro MD (Henry Shein). DHIN & MU Attestation Support, Menu Items Specialized Registry: Successful ongoing transmission of data to a specialized registry Newborn Screening DHIN Newborn Screening Submission Process Working closely with DHIN member hospitals to establish electronic newborn screening interfaces for submission to DPH registry. Aligning process with DPH efforts in this area. 18
