It consists on: Sensors Are the elements responsible for capturing and monitoring the biometric signals (pulse, temperature, weight, ). Gateways All signals collected by sensors are aggregated into a single point (gateway) for delivery via communication networks to the central processing platforms. Wireless technologies (Wifi, Ant+, Bluetooth, ) Improved usability and better user experience 6

SOLUTION DESCRIPTION HISPANIS HEALTH CONNECT Integration Installations Management Platform HISPANIS HEALTH CONNECT This is the module responsible for processing the signals and embed the layer of administration and management of remote devices. Consists on: Integration platform This is the element responsible for collecting all the captured biometric signals and transform them so that they are understandable and actionable by other systems. The objective of this component is to enable real integration (ensuring interoperability) of telemonitoring systems with the standard work tools of health professionals. Facilities management platform In order to facilitate the management of the system, it is necessary to have an element that allows to monitor the status of remote facilities and act on them if a problem occurs. From the point of view of the patient it offers: Entering data no directly captured by the device (overall sensation, mood, etc.) Receive notifications and reminders based on defined business rules. From the point of view of the professional it offers: Quickly review of all the patients enrolled in telemonitoring programs. Send messages and notifications directly to patients. 8
