Meaningful Use Stage 2 - Patient List Creation Patient List Objectives Patient List Measures OpenEMR Assesment Current Status Gap Action Plan Priority 1.Create a list of patients sort, access, and create patient lists. Can the OpenEMR system generate a patient list? Currently part of OpenEMR v4.1.2 2. Generate a patient list based on a sequence or date range date and time. 3. Create a patient list with the ability to generate each one and at least one combination" date and time and based on data from Team # 1 Open EMR Certification Gap Analysis Can the OpenEMR system generate a sorted patient list based on a date range? each one and at least one combination of Medication list, Medication Allergy list, data from two or more of the following Demographics, Laboratory tests and data categories: problems, medications, values/results, and Patient communication medication allergies, demographics, preferences sorted patient list based on a date laboratory tests and values/results, and, range? in the ambulatory setting only, patient communication preferences. Currently part of OpenEMR v4.1.2 Can the OpenEMR system generate at least The OpenEMR database stores one list of patients based on a combination of information from all the outlined data elements from two or more of the categories, this feature is not following categories: the Problem list, currently implemented. 4. Record a patient s communication preference Can the OpenEMR system generate a list based The ability to store the electronically and dynamically set patient on communication preference? communication preferences in Ambulatory setting only. communication preferences for a patient is currently part of OpenEMR v4.1.2, but the ability to search based on a patient's communication preference is not. The ability to sort based on specific date. The ability to search simultaneously based on date range, the Problem list, Medication list, Medication Allergy list, Demographics, Laboratory tests and values/results, and Patient communication preferences. The ability to sort based on specified criteria. The ability to search based on a patient's communication preference. While there are different "Yes/No" style patient communication options in OpenEMR, there is no place to select a specific "preference." We recommend using an algorithm to auto- populate preference (which will be a new data element) based on existing Yes/No answers, and if multiple fields are "Yes" then letting the user of the EMR select one of the options. Another item that will need to be added is the type of provider (ambulatory or hospital) so that you can do the search based on ambulatory online (in case data from hospital is not necessarily included). This should be in a background databsae. 5. Generate a patient list with medical problems date and time and based on structured data defined as medical problems. 6. Generate a patient list with medications defined as medications, medication allergies. 7. Generate a patient list with demographics defined as demographics. 8. Generate a patient list with laboratory test results defined as laboratory tests and values/results. Can the OpenEMR system generate a list based The ability to store the medical on medical problems? problems for a patient is currently part of OpenEMR v4.1.2, but the ability to search based on a patient's medical problems is not. Can the OpenEMR system generate a list based on medications? The ability to store the medications for a patient is currently part of OpenEMR v4.1.2, but the ability to search based on a patient's medications is not. Can the OpenEMR system generate a list based The ability to store the on demographics? demographics for a patient is currently part of OpenEMR v4.1.2, but the ability to search based on a patient's demographics problems is not. Can the OpenEMR system generate a list based on laboratory test results? The ability to store the laboratory test results for a patient is currently part of OpenEMR v4.1.2, but the ability to search based on patient's laboratory test results is not. medical problems. medications. We also recommend adding another data element to pull from other parts of the OpenEMR database the allergy of the patients so you can sort by allergies. demographics. laboratory test results. We recommend adding fields that will pull in the preferred "Lab Test" and "Lab Values". We recommend doing this on the basis of codes for various lab tests (standards are key) rather than what seems to be the current free text option.

Graphical Representation of Patient List for Modification for Meaningful Use 2 Attestation Our current OpenEMR display in HITNY.ORG, this should be incorporated from the Clinical report into the Patient List report: 1 P age

From interpreting the current Report-Clinical Screen we have already the following: Demographics Problem Medications Lab Results 2 P age

We now want to expand the search criteria by adding the following data elements: Add the addition Options: Medical Problems Lab Test: Lab Values/Results: Add a new field Lab Test: and Lab Values/Result: Drug Allergies: Add a new field Drug Allergies: Type: Add a new field Type: 3 P age

The new field is contingent if we can get these values from the current EHR version. A new algorithm to pull in the patient communication preference into the Patient List that is check yes for ambulatory settings only: As for the Ambulatory settings, a new field to be added for new creation of type of Facilities e.g. Inpatient, Outpatient, DPO, Clinic, Ambulatory.: 4 P age
