Upgrades Included With Care Connect, you will benefit from automatic upgrades to servers and software, including upgrades to ICD-10 (anticipated for 2013) and ICD-11 (potentially in 2014). Increased Patient Satisfaction With additional options such as MyCare, your patients will benefit from easy, online access to current health information and lab results. Ability to Achieve Meaningful Use Care Connect is a certified EMR enabling you to earn up to $64,000 if you enroll in Medicaid or up to $44,000 if you enroll in the Medicare program. GRIPA will assist Care Connect users in achieving Meaningful Use. Simplified, Less Costly Hardware Management With Care Connect, servers are maintained by RGHS, eliminating the need to purchase your own servers which can be complicated and expensive. Maintain Your Practice Autonomy All practice financial data remains private and confidential. Automatic E-prescribing Functionality Both RGHS and URMC have selected the Epic EMR. This means that up to 80% of patients in Monroe County could potentially share the same electronic medical record system. As part of our continuing efforts to build One Great Health System, RGHS is implementing Care Connect, an Epic Electronic Medical Record (EMR) system, across the organization. The Epic system is one of the most highly regarded electronic tools available in the healthcare industry today. In partnership with GRIPA, RGHS is pleased to be among the many health systems in the country to extend this best-in-class EMR and Practice Management (PM) offering to our RGHS-credentialed community physician practices as well. The intent is to build a highlyintegrated EMR network with our affiliated community practices who subscribe to the service. This unified EMR system will provide all users with instant access to a broad range of consistent, accurate and up-to-date patient information creating a powerful connection between physicians, patients and RGHS hospitals and medical facilities. Due to regulations permitting hospitals to subsidize the cost of certain EMR technology components, Care Connect can be offered to you on a subsidized basis to offset the significant investment typically required for a practice to implement such a robust EMR system. This offering is another important example of the unwavering commitment of RGHS and GRIPA to support physicians and the community at large in our shared objective to advance the health and well being of the communities we serve. An important benefit of installing an electronic medical record is the ability to re-examine current care and operational processes that will become a part of the EMR. This exercise has proven to be a strong catalyst for process improvements in many practices that result in better patient care and increased patient safety, operational/billing efficiencies, enhanced staff collaboration and stronger clinical integration, to name just a few key benefits. Your practice and staff can realize numerous efficiencies and improvements through the Care Connect Community Physician Program including: Immediate, automatic availability of your patients latest health records and RGHS test results as soon as they are completed Simpler, faster and more accurate documentation of patient encounters through the use of consistent templates Better practice management made possible with the help of many standard report tools available in the system Integrated Practice Management Applications (Registration, Scheduling and Billing) that streamline your administrative processes Partner Criteria To participate in the Care Connect Community Physician Program, interested practices must first fully understand the commitment, and their readiness, to undertake such a dramatic transformation of their care delivery processes. To assist with this process, our Care Connect Support Team will work with you to conduct an initial assessment of your practice and answer your questions. Practices that would like to be considered for one of the pilot sites will be asked to supply additional information for selection purposes. Partner Suitability A suitable partner will demonstrate leadership commitment, financial commitment and capacity, operational readiness and technical competence. They will also demonstrate beliefs and goals consistent with the mission and vision of RGHS and GRIPA. Leadership Commitment Strong physician leadership and commitment are required for success. Both the practice manager and a designated physician champion are needed to lead all practice staff to full adoption. The physician champion must have a good understanding of practice operational controls and work processes. This leader will need to embrace change, including disruption of office flow and work processes during implementation, as the entire practice moves forward into the digital world of an EMR.

Well Suited Practice Types The Care Connect Community Physician Program includes easy to follow EMR workflows and computer screens developed for a broad array of practice types including, but not limited to: Asthma/Allergy/Immunology Infectious Disease Pediatrics Cardiology Internal Medicine Plastic Surgery Cardiothoracic Surgery Medical Oncology Podiatry Dermatology Neurology Psychiatry Endocrinology Neurosurgery Pulmonology ENT Obstetrics/Gynecology Rheumatology Family Medicine Ophthalmology Sports Medicine Gastroenterology Orthopedics Urology General Surgery Pain Management Vascular Surgery Financial Commitment and Capacity The financial investment in the Care Connect Community Physician Program is greater than the obvious project costs. Implicit costs include system set-up and learning activities, as well as the soft cost of reduced productivity during the initial learning curve. All members of the practice must understand and accept that this learning curve can only be overcome through persistent effort. Operational Readiness For a successful implementation, the practice should be fully staffed, running smoothly and not undergoing any significant changes. Leadership and staff will need to be adaptable to substantial changes in the clinical workflows that are inherent in implementing an EMR. Technical Competence The computer and network technology must meet the Care Connect requirements and be well supported by a qualified technician(s). Pilot Sites and Sequencing Several practices will be selected as initial pilot sites to implement Care Connect. The pilot sites will be asked to provide candid feedback about their experience and the implementation process to the RGHS and GRIPA care community. The physician champion at the pilot sites may be called upon by other Care Connect participants as a reference or to offer guidance and advice. Specific pilot site criteria are under development and further information will be provided as soon as it s available. For a limited time, Care Connect can be offered to eligible practices on a subsidized basis to offset the significant investment typically required to implement such a robust EMR system.* Special Considerations Possible Tax Implications Accepting the subsidized offering may have tax implications to you or your practice. When considering subscribing to the Care Connect Community Physician Program, you should consult with your tax or accounting advisor. Subsidies Expire The Care Connect subsidy will expire on 12/31/2013 in accordance with the sunset provisions of the Stark Exception. All fees will then be increased to the current fair market value. We will provide a good-faith estimate of those fees in our proposal to you, though these fees cannot be guaranteed. * Practices currently using an EMR may not qualify to receive the Care Connect subsidy. This provides the ability to increase patient-provider communication via an online patient portal. Practices using this tool have reported significant reductions in the time spent managing patient requests, and dramatic increases in their patients satisfaction levels.
