ANNEX 12: TEMPLATE OF ANIMAL HEALTH PLAN AND ANIMAL TREATMENT PLAN FOR IKB KIP POULTRY FARMS In view of the IKB Kip certification scheme, in relation to Annex 1.2A and Annex 1 to the IKB Kip General Terms and Conditions, in order to compile a template to be used by IKB farms together with vets when drawing up animal health and animal treatment plans, the management of the PLUIMNED foundation adopted on 23 June 2015 the following: TEMPLATE OF ANIMAL HEALTH AND ANIMAL TREATMENT PLAN FOR IKB KIP POULTRY FARMS Animal health plan Article 1 The animal health plan (AWP) involves an analysis of the hygiene and animal health status and veterinary medicinal products used in your farm over a period spanning at least the previous year and entering into agreements to improve the hygiene and animal health status at your farm in the following year. The plan is compiled in co-operation with the veterinarian with whom you have an agreement in terms of the IKB Kip certification scheme, your technical advisers and the owner of the animals (if applicable). You can use the points requiring attention listed in section A.2. in your analysis. In section A.3 you can indicate which concrete measures you will take to improve the animal health at your farm and the time limit you have set yourself for it. You will need to estimate the anticipated reduction in antibiotics use as a result of these measures. Together with your vet and your technical advisers, you evaluate the animal health plan at least once a year. Fill in the results of the evaluation in section A.4. In addition, draw up measures for the following year, based on the outcome of the evaluation. Animal treatment plan Article 2 The animal treatment plan (ATP) consists of: a. an overview of the diseases (bacterial infections), with for each disease, the manner in which your company should tackle it. A distinction should be made between the different stables, if necessary. 1

b. agreements about the way in which a clinical diagnosis is made, about the dissection of animals and about the use of bacteriological investigations and/or sensitivity tests. The vet will compile the animal treatment plan together with you and the owner of the animals (if applicable). You must complete sections B.1 to B.5. Any prescriptions by the vet for antibiotics and administration of antibiotics by the poultry farmer must occur based on the animal treatment plan. The animal treatment plan is based on the most recent version of the Poultry Formulary of the Working Group for Veterinary Antibiotics Policy of the Royal Netherlands Veterinary Association and on veterinary guidelines. Or the foreign equivalent if the IKB company is not based in the Netherlands. You must evaluate the animal treatment plan at least once a year. Please fill in the results of the assessment under B.6. Concluding provisions Article 3 This template is referred to as the Template of animal health plan and animal treatment plan for IKB Kip poultry farms. With the ultimate aim to curb antibiotics use and further confinement of tracking in and movement of illness and contamination List at least three concrete measures to improve the animal health condition at your farm and to curb the use of antibiotics. For each measure, specify the anticipated time limit for implementation. Specify the reduction in antibiotics use you expect to achieve with these measures. Moreover, you will state at least 1 point arising from the hygiene scan. NB. If the use of antibiotics at your company is at the proper level, taking measures regarding animal health status is not obligatory. So far, a target level has only been set for broiler farms.
