That s why we encourage you to choose a personal primary care physician who will guide and coordinate your care. You and your doctor can build a longstanding relationship, and work with each other to help achieve and maintain good health. Having one doctor who knows your medical history helps you get the right care if you need to see a specialist or receive other medical services. You can change your personal physician for any reason. And you can also self-refer to select specialties. Getting started When you join Kaiser Permanente, we ll send you Your Guidebook to Kaiser Permanente Services, which includes important information about where our facilities are located and how to get care. If you d like to get started right away, the following Q&A can help. I don t have my Kaiser Permanente ID card yet. How do I make an appointment? If you have enrolled and haven t received your ID card within 1 working days of your effective date, please call our Member Service Call Center at 1-0-464-4000. Please keep a copy of your enrollment application you can use it as a temporary ID when you need to seek care. Call your group benefits administrator to get your group number and effective date of coverage. You can begin using our services the day your coverage takes effect. Sprain something snowboarding? Banged up while biking? In many areas, we have 24-hour advice nurses who can help assess symptoms and direct you to an appropriate place for treatment. How do I choose a doctor? You can choose your personal physician in a couple of ways: Call the Advice/Appointment number at the Kaiser Permanente facility that s most convenient for you. (Please see pages 26 for phone numbers.) Look up physicians in our online directory at www.kaiserpermanente.org. You can choose your personal physician from family practice, internal medicine, or pediatrics. In some areas, women can also choose a physician from obstetrics/gynecology as their personal physician. If the physician you choose already has a full panel of patients, you might need to select an alternate. You can change your doctor for any reason. What if I m traveling and need care? If you plan to travel, please take along your Kaiser Permanente ID card and your Visiting Member Brochure, which you can get by calling our Member Service Call Center at 1-0-464-4000. If you are in one of our service areas and you need urgent care at a Kaiser Permanente facility, you can usually get a same-day appointment. An urgent care need is one that requires prompt medical attention within 24 to 48 hours.
