Helping to ensure a smooth transition Prior to discharge, Valley View makes every effort to ensure that discharging residents are fully prepared for the challenges they will face when returning home or moving to an alternate level of care. Our therapists may provide a plan of exercises and guidelines for safely performing daily routines. If needed, social workers will arrange for wheelchair assistance or other assistive devices. Nutritionists are available to provide counseling on dietary issues, and nurses will provide instruction and literature on wound care and medication management, if applicable. The Valley View rehab department will also evaluate the resident s home environment to determine if physical modifications are needed in order to optimize safety and security. It s all part of our continuing effort to ensure that each resident s road to optimum recovery and independence will be a safe and successful one.

The Valley View Center for Nursing Care and Rehabilitation has been serving the health care needs of Orange County residents for more than three generations. Rooted in a tradition of excellence in long-term nursing care, today s Valley View is also one of the area s premier rehabilitation and subacute care centers. A special healing environment Valley View s modern, 14,000-square-foot rehabilitation unit features more than 50 state-of-the-art treatment modalities in the areas of physical, occupational and speech therapy. Separate and distinct from our main facility and general population, it is dedicated exclusively to residents in need of short-term, post-hospital rehab. Special construction, interior design and furnishings help to create a unique homelike environment and sense of community, conducive to optimum resident privacy, comfort and recovery. Headed by licensed physical therapist, Mr. Sam Nagappan, our highly trained staff of 10 full-time and three part-time therapists, including a prosthetic specialist, enables us to handle even the most complex rehabilitative cases. An experienced team of nurses provides an added measure of clinical support. An interdisciplinary approach to care Valley View s rehab programs are physiciansupervised and specially tailored to meet each patient s individual needs and goals. Depending upon the specific injury and the number and types of functional abilities that need to be addressed, a typical integrated rehab program may include several distinct types of therapy and related specialized services, including gait and balance training, pain management and wound care, to name just a few. Likewise, the interdisciplinary rehab team may consist of Physical therapists to help restore flexibility, strength and mobility Occupational therapists focusing on restoration of daily living skills Skilled nurses to provide on-going nursing care, pain management, wound care and other important clinical services Psychologists to help deal with the long-term emotional and psychological implications of a particular condition Recreational therapists to help enhance physical and cognitive skills through group and individual activities Therapeutic specialists trained in restoring gait and balance, hand strength and fine motor function Social workers to act as liaisons for family, home and community services Medical equipment specialists to assess and fit assistive devices such as wheelchairs and walkers Adaptive training specialists to help train patients in the use of prosthetic and orthotic devices Individualized care and programming At Valley View, rehabilitation programs are designed around each person s specific needs and goals. Individual capabilities and limitations are carefully evaluated and used to establish realistic objectives and optimum care plans. Therapies are age-appropriate and may take the person s occupation, lifestyle and even his or her favorite sports or activities into consideration. It is also important to note that unlike many rehab programs based in nursing facilities, Valley View offers six days of therapy per week. For those who are able to safely participate at this more aggressive level, the results are often measured in faster recovery times.
