Wellbeing Assessment The Wellbeing assessment is an online questionnaire that measures your wellbeing in 6 categories: life evaluation emotional health physical health healthy behaviors work environment basic access to health/life resources This gives a complete picture of current wellbeing, risks, and areas for improvement for you. When you complete the assessment, you will receive a personal wellbeing report and an action plan. 20
