Benefit Elections You will have 31 days from July 1, 2014 to make your elections for the following benefits: Health Insurance Dental Insurance Vision Insurance Life Insurance Flexible Spending Accounts Your current election is for the 2014 2015 plan year. Your current Healthcare and Dependent Care FSA elections are for July December 2014.
Eligible Dependents Spouse* Same Sex Domestic Partner (SSDP)** Children up to age 26 Eligible children include: Foster children Step children Disabled children Adopted children SSDP s children Legal custody, in a parent/child relationship * $50 per month Spouse/SSDP Medical Charge applies if eligible for coverage through his/her employer. You must complete the certification. ** Proof of their eligibility as a dependent is required required documentation can be found in the Benefits Interactive Guide available online.
Family Status Change Changes can be made each annual enrollment or within 31 days of a qualified Family Status Change. Examples of Family Status Changes include: Marriage Divorce Birth, adoption, legal guardianship or Qualified Medical Child Support Order (QMCSO) Death of a spouse or dependent Start or termination of your spouse s employment Spouse s Annual Enrollment A dependent becomes or ceases to be eligible Please refer to the online Benefits Guide for other common Family Status Changes. Medical Insurance Coverage for you (single) is paid in full by Emory You pay the monthly cost to cover any dependents (Spouse, Same Sex Domestic Partner, Child(ren)) Emory offers one option for medical insurance to Medical House Staff: POS Plan A conventional medical plan that protects you in the event you have significant medical expenses during the year. The Plan allows members to receive services from a national network of providers and facilities on an open access basis. Note: If no election is made for health, employee is defaulted to the POS plan Single coverage ONLY

Flexible Spending Accounts (FSA) Contributions taken on pre tax basis to pay for qualified out of pocket Healthcare and/or Dependent Daycare expenses Contribution Amounts Contribute up to $2,500 to the Healthcare FSA Contribute up to $5,000 per year to the Dependent Care FSA The IRS restricts the amount that can be set aside under the Dependent Care FSA to $5,000 per household $200 minimum contribution under each account The election you will make now will cover expenses from July 1, 2014 December 31, 2014. You will have an opportunity to elect FSA for 2015 in the Fall. Long Term Disability Insurance Paid in full by Emory Provides 60% of salary following a 90 day waiting period if you are unable to perform the material duties of your regular occupation (specialty or sub specialty in which you practice). Benefits are not paid for disabilities that arise from an illness or injury that took place within 3 months before your coverage begins.
Retirement Plan Contributions can be made to Fidelity, Vanguard or TIAA CREF Pre tax contributions can be made to the Traditional 403(b) Retirement Plan After tax contributions can be made to the Roth 403(b) Retirement Plan There will be NO Employer contributions or match while in the Medical House Staff Program. To enroll log on to the Employee Self Service at http://leo.cc.emory.edu Select Benefits and then click 403(b) Savings Plan Election. Complete Vendor Enrollment Online IRS Contribution Limits for 2014 $17,500 if under age 50 and $23,000 for age 50 and over

Retirement Plan elections/changes can be made at any time. Remember to complete the Tobacco and Spouse Certifications by July 31 st. Health, Dental and Flexible Spending Account elections must be made by July 31 st. Mid Year changes can be made within 31 days of a qualified Family Status Change. The changes made to benefits must be consistent with the event.
Conclusion Emory reserves the right to terminate, suspend, withdraw, amend or modify the Plan in whole or in part at any time. Further, Emory reserves the right to terminate or modify coverage for any group of employees, active or retired, and their dependents or a class of dependents at any time.
