TENNESSEE OCCUPATIONAL THERAPY ASSOCIATION Student Scholarships Available Prior to each state conference, TNOTA, accepts applications for Student Scholarships. The money used to fund these scholarships is derived from the Silent Auction held at the previous year s TNOTA Annual Conference. Students currently enrolled in entry level OT or OTA programs in Tennessee and who are current members of TNOTA are eligible to apply. Scholarships are awarded based upon the following: Academic performance Leadership skills Professional service Applicants must have had one term or semester of OT or OTA course work completed before applying. The guidelines for applying are included with the Application Form. TENNESSEE OCCUPATIONAL THERAPY ASSOCIATION STUDENT SCHOLARSHIPS Please review these guidelines: incomplete applications will not be considered. 1. Applicants must be current members of TNOTA 2. Applicants must be currently enrolled in an entry level OTA or OT ACOTE accredited program located in Tennessee. Students in developing programs are ineligible. 3. Applicants must have completed at least 1 term of course work. 4. Applicants must arrange to have a recent official transcript sent by their college/university Registrar to the address below. The transcript must include the cumulative grade point average (GPA) from the most recent semester/term the student has completed. Students with a GPA of 3.0 or higher will be given priority. 5. Applicants must submit only 2 professional references with original signatures: One reference must be from a faculty member of the OTA/OT program you are attending. Both references should address applicant s academic achievement, leadership skills and/or professional service. The second reference should be an Occupational Therapist or hold a position of authority and be able to write on the applicant s leadership and professional service. References cannot be family members, immediate household or domestic partners 6. Applicants should read the Terms and Conditions, sign and date the form. 7. Scholarship winner(s) will be notified with formal presentation made at the TNOTA Annual Conference.

Knowingly providing false information will disqualify me from the TNOTA Student Scholarship To receive the scholarship monies I must be enrolled and in good standing in an accredited ACOTE OT/OTA program.
