CARE TRANSITIONS Adventus provides comprehensive in-hospital planning and home follow-up for chronically ill high-risk older adults in an effort to improve care transitions, increase quality of care and reduce readmissions. Patients who received this program were significantly less likely to be readmitted to the hospital. Patients are also more likely to achieve self-identified personal goals around symptom management and functional recovery. Transition coaches using our proven Coleman based model have teamed up with healthcare providers and have managed to lower 30-day readmission rate among Medicare Patients.
