WELCOME TO LAKESIDE AQUATIC CLUB! Lakeside Aquatic Club (LAC) is a year-round swimming club that serves the communities north of the Dallas-Fort Worth Metroplex. We operate out of four facilities in Flower Mound, Keller, and The Colony. Our nationally recognized coaching staff offers programs for swimmers of all ages and abilities. From young children interested in learning to swim to elite swimmers wishing to train year-round, LAC offers a program that's right for you! Included in this packet are: Development Program Description LAC Terms, Conditions and Policies Expectations for Swimmers Expectations for Parents/Guardians LAC Release Form LAC Waiver/Release of Liability Signature Page DEVELOPMENT PROGRAM DESCRIPTION General The LAC Development Program is designed to teach the very basics of swimming to all ages beginning with water safety and concluding with the fundamentals of all four competitive swimming strokes: freestyle, backstroke, breaststroke and butterfly. The Development Program is divided into tiers or training groups: Learn-to-Swim and Stroke School (which is made up of our Red Training Group and Black Training Group). USA Swimming membership is not required for participants in the Development Program. Upon completion of the Development Program, swimmers will be ready to swim with LAC s first competitive training group the Bronze Training Group. Learn-to-Swim Learn-to-Swim (LTS) is the 1 st tier in the LAC Development Program. It is a basic swim lesson program designed for any child 3 years old or older. Instruction is focused on water safety: jumping off the side of pool, kick across the pool, side breathing and the basics of arm stroke technique. At the completion of LTS, swimmers will be able to perform the basic kick and arm movements of all four competitive strokes for half a pool length and are ready to swim with the Red Training Group. Stroke School (Red and Black Training Groups) The Red and Black Training Groups are designed to teach the fundamental techniques of all four competitive swimming strokes. Emphasis is on mechanically sound stroke and kick techniques during a 25 yard swim. Swimmers will be introduced to the basics of competitive swimming through competitions at summer league meets or Friday Night at the Races events. The Red Training Group is the 2 nd tier in the LAC Development Program; the Black Training Group is the 3 rd and final tier. Upon completion of the Black Training Group program, swimmers are ready to advance to the Age Group Program (Bronze Training Group) and become full members of USA Swimming.

LAC TERMS, CONDITIONS AND POLICIES The Lakeside Aquatic Club is a non-profit organization. Our team s success in and out of the water is created by team work, open communication, and the commitment to excellence by our athletes, parents, and coaches. Below are the Terms and Conditions for participation with LAC: 1. Accounts with more than one swimmer, are eligible for discount on the swimmer that has the lower/lowest dues. 2. All fees and dues are to be sent to the LAC P.O. Box or paid through PayPal on our website, unless you are in our Learn-To-Swim program or otherwise notated. 3. Learn-To-Swim Program: Due to swimmers quickly progressing from our Learn-To-Swim Program to our Red and Black Training Groups, you must register for our Learn-To-Swim program on a session-by-session basis. A Waiver/Release of Liability Signature Page and payment must be on file prior to beginning any Learn-To-Swim session. Registration is done through our website at www.swimlac.org. 4. Stroke School (Red/Black Groups): After initial registration and payment is received for swimmers in these groups, subsequent monthly dues will be invoiced, with invoices being sent to the email address provided on the Waiver/Release of Liability page. Monthly dues are paid on the 1 st of each month and no later than the 15 th of each month. A late fee of $10 will be assessed if payments are received after the 15 th of each month. Please contact the Team Treasurer at lac_treasurer@verizon.net if special circumstances exist. 5. All payments are non-refundable should a swimmer leave or be dismissed from the program prior to the end of the month or session. 6. All payments to LAC must be by check, money order, or through PayPal on our website. Cash is not accepted. Coaches will not accept payment of any kind. 7. There is a $25 returned check fee. 8. Parents must notify the Team Treasurer in writing when requesting a leave of absence. The request must be received before the first of the month to avoid paying the monthly dues. Notifying the coaching staff is not sufficient notice. 9. Parents of any Red/Black swimmer must notify the Team Treasurer in writing when quitting the team or suspending their account. A 30 day notice is required, and must be provided to avoid any charges. Notifying the coaching staff is not sufficient notice. No reinstatements will be made until past dues are paid. 10. If you suspend your membership, please note that there is no guarantee that your athlete s spot on the team will be available when you want to rejoin the program. Group openings will be filled as quickly as possible on a first come, first serve basis.

EXPECTATIONS FOR SWIMMERS Swimmers participating in the LAC Development Program shall abide by the following standards. 1. Swimmers will demonstrate good sportsmanship at all times by following the directions of the Coaching Staff, being respectful of other swimmers, practice good teamwork and put forth their best effort at all times. 2. Abusive language directed at the coaching staff or other swimmers will not be tolerated. 3. Improper or destructive behavior will not be tolerated. Parents/Guardians will be responsible for any damage expense incurred. 4. The possession or use of alcohol, tobacco or any non-prescribed (illegal) drug is prohibited. Swimmers will not participate in any LAC event while under the influence alcohol or illegal drugs. 5. The possession or use of any firearm, hazardous chemical or pyrotechnic device is prohibited. Any violation of the aforementioned expectations in the judgment of the LAC Coaching Staff or Board of Directors is grounds for immediate termination of participation. LAC maintains the right to terminate any membership with or without cause in the interest of our vision, mission, and objectives. EXPECTATIONS FOR PARENTS/GUARDIANS Parents/Guardians of a swimmer participating in the LAC Development Program shall abide by the following expectations. 1. Will practice teamwork with all parents, swimmers, and coaches by supporting the values of Discipline, Loyalty, Commitment and Hard Work. 2. Will not coach or instruct the team or any swimmer at a practice (from the stands or any other area) or interfere with coaches on the pool deck during practice. 3. Will not step onto the pool deck unless you the parent/guardian are registered with USA Swimming (per USA Swimming guidelines). If registered with USA Swimming, will only step onto pool deck with the intent to discuss LAC work-related business. 4. Will demonstrate good sportsmanship by conducting yourself in a manner that earns the respect of your child, other swimmers, parents, officials, and coaches. 5. Will understand that criticizing, name-calling, making inappropriate gestures, or using abusive language at practices, or towards the athletes, coaches, LAC board members, or officials will not be tolerated. This includes face to face interactions, phone calls, text messages, social media and emails. Any violation of the aforementioned expectations in the judgment of the LAC Coaching Staff or Board of Directors is grounds for immediate termination of participation. LAC maintains the right to terminate any membership with or without cause in the interest of our vision, mission, and objectives.

LAC RELEASE FORM Indemnify and Hold Harmless By registering and paying, and in consideration thereof, the parent or guardian, for themselves, and their children or wards, hereby agrees to release, waive, indemnify, defend, and hold harmless the Lakeside Aquatic Club, all and each of its officers, directors, agents, and employees from and against any and all demands, claims, causes of action, suits, liability, losses and/or damages to persons or property, including reasonable attorney's fees, from or relating to use of the property, facilities, and/or services of the Lakeside Aquatic Club. Risks and Assumption of Risk By registering and paying, and in consideration thereof, the parent or guardian, for themselves, and their children or wards, acknowledges and understands that swimming and the use of equipment and/or facilities provided by Lakeside Aquatic Club as with any activity, involves some risk and danger, including but not limited to property damage and bodily injury, which may include permanent disability, paralysis and possibly death. These risks may result from participation in the program and specifically in training as a competitive athlete, and/or the use of the equipment or facilities, the acts of others or from the unavailability of emergency medical care. Additionally, the undersigned parent or guardian, for themselves, and their children or wards or their guests, voluntarily assumes all risks that may arise out of or result from the use of the equipment or facilities, and/or the services of the Lakeside Aquatic Club. This clause does not apply to any injuries caused by the gross negligence, or willful or wanton misconduct of any officials, officers, directors, employees, agents, or volunteers of Lakeside Aquatic Club. Photography and Video LAC believes our swimmers are our best Ambassadors. We often update our website with photos and/or videos of swimmers at events. Photos/videos are used for LAC business only and only for publicity, training, or other business purposes. LAC will not notify you prior to using photos/videos of your swimmer.

Waiver/Release of Liability Form PLEASE READ CAREFULLY BEFORE SIGNING. THIS IS A RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS. I,, the parent/guardian of the participant agree and understand that swimming is a HAZARDOUS activity. I recognize that there are risks inherent in the sport of swimming, including but not limited to, paralyzing injuries and death. The participant hereby agrees to participate in the LAC Development Program and hereby agrees to release and indemnify and hold harmless Lakeside Aquatic Club, its coaches, officers, directors, agents and employees against any liability resulting from any injury that may occur to the participant while participating in the LAC Development Program. The participant also agrees to indemnify Lakeside Aquatic Club for any damages incurred arising from any claims, demand, action or cause of action by the participant. The parent/guardian of the participant states that the participant is in good physical health. The participant authorizes any representative of Lakeside Aquatic Club to have the participant treated in any medical emergency during their participation in the LAC Development Program. Further, the participant and/or parent/guardian agree to pay all costs associated with medical care and transportation for the participant.
