ALBERTA PHARMACY PRACTICE RESIDENCY PROGRAMS 2016-2017 ACADEMIC YEAR As of April 1, 2009, the Alberta Cancer Board and 11 other provincial health authorities joined together to form Alberta Health Services (AHS). For more information about health care in Alberta visit the AHS website www.albertahealthservices.ca. The pharmacy practice residency is an amalgamated program, with sites in 4 zones of the province: Red Deer, Edmonton, Calgary and the two Cancer Centres in Alberta. ALBERTA HEALTH SERVICES (AHS) PHARMACY PRACTICE RESIDENCY PROGRAMS GENERAL DESCRIPTION: A pharmacy practice residency is a one-year post-graduate program designed to prepare pharmacists to meet the challenge of pharmacy practice and assume a leadership role in the profession. Through a residency program, the pharmacist will gain experience in all aspects of pharmacy including administration, drug distribution and direct patient care. Problem solving and communication skills are emphasized throughout the residency and research methodology is introduced in the major project. The purpose of a hospital pharmacy residency is: To refine professional competence in direct patient care, through supervised practice under the guidance of model practitioners, in pharmacy operational services and in project management gained during study in an accredited pharmacy professional degree program. To refine personal practice skills. To develop leadership skills that can be applied in any position and in any practice setting All pharmacy practice residency sites offer rotations that will achieve the goals of the program and the competencies in the CHPRB Accreditation Standards. Educational outcomes of a pharmacy practice residency include minimally: Providing evidence-based direct patient care as a member of interprofessional teams Managing and improving the medication-use process Exercising leadership Exhibiting skill in managing one s own practice of pharmacy Providing medication and practice-related education and, Demonstrating project management skills The pharmacy practice residency in Alberta is a general residency providing the resident with a wide variety of patient care experiences. The patient population for each site is different but the experiences and expectations for all residents in Alberta are the same. EXTERNAL ROTATION OPTIONS IN ALBERTA (Subject to agreement between sites) Up to 25% of a resident's program can be allocated to external rotations. Accommodation and travel costs will be the responsibility of the resident. Talk to the Residency Coordinator(s) if you have a specific area of interest that may not be available in Alberta. Rotations at sites within Alberta are not considered as external, but do require planning between coordinators. Talk to your coordinator if you want to complete a rotation at a campus other than your home base. Page 2
SPECIAL EVENTS & PRESENTATIONS: CASE & PROJECT PRESENTATIONS Each resident is required to prepare an evidence-based patient care plan and to present the case at a provincial residency meeting. A second event is presentation of a summary of the resident's research project completed during the residency program. RESEARCH WORKSHOPS Several sessions are planned each year for the residents to discuss issues regarding their research project planning, research methodology, ethics, data collection and statistics, and data dissemination and publication. AWARDS: All awards are coordinated and provided through the Alberta Branch of CSHP. The Alberta Specific awards include the following: 1. Two Alberta Resident awards are for the residents who demonstrate excellence throughout the residency program. The recipients of the awards will be announced at the end of the residency year. 2. Residents are eligible to apply for awards through the Canadian Society of Hospital Pharmacists (CSHP). Contact CSHP (www.cshp.ca and www.cshp-ab.ca) for criteria and application forms for the various awards. These are normally applicable for the projects, and applied for after the residency program is completed. Applicants must be members of CSHP for 12 months prior to application. Page 3
APPLICATION PROCESS PREREQUISITE (S): Bachelor of Science in Pharmacy Degree (or equivalent entry to practice degree) Eligible for the Clinical Pharmacist register in Alberta Member of CSHP Any applicant seeking appointment to a residency program in pharmacy practice must enroll in the Canadian Hospital Pharmacy Residency Board (CHPRB) Residency Matching Service (RMS) by November 1, 2015. Please complete the RMS online at http://www.cshp.ca/programs/residencytraining/applicants/rms _e.asp APPLICATION DEADLINE: APPLICATION FORMS: October 30, 2015. (Completed application forms and all other documents must be emailed no later than 1600 MST on October 30 st ) Forms are available upon request from the Residency Coordinators and from the web page www.cshp.ca/programs/residencytraining/applicantsinfo_e.asp An applicant may request consideration at one or all zones by submitting separate applications to each zone. Completed application forms are to be sent to the Residency Coordinator(s) by email at each institution. SELECTION PROCESS: Selected applicants are interviewed by the Residency Coordinators. Applicants are evaluated on the basis of individual merit. Each Residency Coordinator chooses the applicants to be interviewed. All applicants, including those not selected for interview, will be contacted regarding the status of their application by December 18, 2015. The applicants will be ranked through the CHPRB residency matching service. Programs and applicants submit a Rank Order List to the CHPRB RMS. AHS, Calgary Zone Hospital: Calgary Zone Acute Care Hospitals 4448 Front Street SE Calgary, AB T3M 1M4 Type of Program: General Residency Number of Residents: 3-4 Starting Date: July 1. PROGRAM DESCRIPTION: A Residency program has been available in the Calgary zone since 1978.This program offers a wide range of patient care experiences involving the 4 adult acute care hospitals and the Alberta Children s Hospital. The residency program features a balanced participation in the principles and practice of clinical, distributive and administrative activities, with an emphasis on patient care. 2. ROTATIONS/ACTIVITIES/TIME SPENT: Orientation Distribution and Sterile Products Direct Patient Care Precepting Management/ Leadership Information Project Orientation to the organization, department, residency program, clinical program and other initial activities Involves all areas of distribution including order review, computerized profiles, unit dose, traditional distribution, outpatient dispensing, use of investigational and study drugs, manufacturing inventory control and Intravenous admixture. Development of skills and knowledge in the provision of direct patient care. Under the guidance of clinical pharmacists, the resident provides pharmaceutical care to patients; serves as a clinical resource person; provides drug information; participates in multidisciplinary rounds and discussions. Case presentations, journal clubs, inservices and/or short written assignments are expected. Mandatory rotation experiences include cardiology, general medicine, critical care, infectious disease, ambulatory care and age related (either pediatrics or geriatrics). The resident may fulfil the criteria of the mandatory rotation experiences through rotations in various areas including, but not limited to, mental health (adult or adolescent), nephrology/transplant, palliative care, oncology, neonatal intensive care, anticoagulation management services, general or vascular surgery and neonatal intensive care., Ambulatory Assist in teaching undergraduate pharmacy students. Under the supervision of an experienced preceptor, the resident will be involved in the day-to-day activities of these students. Introduction to leadership concepts as they apply to pharmacy. Residents are exposed to daily managerial functions, including planning and developing departmental functions, and attend various meetings during a one week rotation with a manager in Pharmacy Services. A small administration project will be completed. Provide drug information to nurses, pharmacists and physicians. Evaluation of information sources, and formulation and communication of the information will be stressed Completion of a pharmacy research project. This learning experience in focuses on research methodology, data collection and evaluation skills. Topic is based on individual s interest and selected in conjunction with research advisor. Special Activities: Various educational opportunities such as medical rounds, journal club, inservices, and interdepartmental visits and meetings are encouraged. Joint education endeavours with the Alberta Pharmacy Practice Residency Programs Committee are mandatory. 4. ADDITIONAL COMMENTS: The Calgary Zone Pharmacy Residency Program is challenging and practice-oriented. The program focuses on the individual needs of the resident, while providing a variety of experiences in major referral centres. An interdisciplinary approach involving medical and allied health team work and teaching is used. 5. PROGRAM DESCRIPTION: The Residency at the Cancer Services offers an emphasis on oncology pharmacy at each of the two tertiary sites in the province. Each of the residents will have a primary rotation site in either Calgary or Edmonton; however, the resident must be prepared to spend time at the alternate site in order to be exposed to a broad range of different aspects of cancer care. 2. ROTATIONS/ACTIVITIES/TIME SPENT: Orientation Distribution and Sterile Products Information Management /Leadership Direct Patient Care Precepting Project Orientation to the organization, department, residency program, clinical program and other initial activities The resident will develop competency in all aspects of the drug distribution system, from receipt of drugs through to the administration of the medication to the patient. Legal requirements, drug distribution systems, order review, investigational and special access drugs, dispensing and incident reporting are some to the topics to be covered. The resident will develop skills required for the safe and efficient preparation of intravenous admixtures, total parenteral nutrition and chemotherapy. Provide drug information to nurses, pharmacists and physicians. Evaluation of information sources, and formulation and communication of the information will be stressed. Introduction to leadership concepts as they apply to pharmacy. Residents are exposed to daily managerial functions, including planning and developing departmental functions, and attend various meetings during a one week rotation with a manager in Pharmacy Services. A small administration project will be completed. Using the philosophy of pharmaceutical care, the resident will provide patient care in the oncology and general medicine settings. Core rotation experiences include general medicine, pain and palliative care, acute care oncology, blood and marrow transplant, and hematological oncology,. Elective rotations include breast cancer, neuro oncology, pediatric oncology and infectious disease. Supportive care for complications of chemotherapy treatment will also be addressed as well as treatment of oncologic emergencies. Assist in teaching undergraduate pharmacy students. Under the supervision of an experienced preceptor, the resident will be involved in the day-to-day activities of these students. Completion of a pharmacy research project. PROGRAM DESCRIPTION: A Residency Program has been offered in Edmonton since 1978. The residency program features balanced participation in the principles and practice of clinical, distributive, administrative and scholarly activities, with an emphasis on patient care. Residents have opportunities in several different sites in Edmonton. 2. ROTATIONS/ACTIVITIES/TIME SPENT: Orientation Information/ Use Evaluation Direct Patient Care Management/ Leadership Project Precepting Distribution and Sterile Products Orientation to the organization, department, residency program, clinical program and other initial activities Provide drug information to nurses, pharmacists and physicians. Evaluation of information sources, and formulation and communication of the information will be stressed. Introduction to DUE principles through formulary process. Rotation includes preparing a Utilization Formulary Review. Provision of pharmaceutical care to patients in a variety of interprofessional settings. Mandatory rotation experiences include cardiology, general medicine, critical care, infectious diseases, ambulatory care and either pediatrics or geriatrics. Optional rotations include cardiology, infectious diseases, critical care, ambulatory care (cardiology, HIV, women s health or homecare), surgery, pediatric oncology, pediatric or neonatal intensive care. Daily discussions with preceptor and regular case presentations to staff are expected. Presentations such as journal club or inservice as well as attendance at rounds and seminars with input will also be expected. Introduction to leadership concepts as they apply to pharmacy. Residents are exposed to daily managerial functions, including planning and developing departmental functions, and attend various meetings during a one week rotation with a manager in Pharmacy Services. A small administration project will be completed. Completion of a pharmacy research project. It is a learning experience in research methodology, data collection and evaluation skills. Topic is based on individual s interest and selected in conjunction with research advisor. A presentation of the research will occur at year end and a manuscript suitable for publication is expected. Assist in teaching undergraduate pharmacy students. Under the supervision of an experienced preceptor, the resident will be involved in the day-to-day activities of these students. Involves all areas of drug preparation, quality control and distribution including interpretation and dispensing of medication orders, and provision of drug information. Technical aspects of aseptic technique and equipment, and preparation and administration of parenteral products will be addressed. During the year the resident is expected to attend staff meetings and selected meetings involving the Pharmacy department. 4. ADDITIONAL COMMENTS: The Edmonton Zone Pharmacy Residency Program is challenging and practice-oriented. Our zone draws on a large number of advanced practice pharmacists to offer rotations. The clinical vision of Edmonton Zone Pharmacy Services emphasizes quality patient-centred care in a dynamic educational environment. The program structure focuses on the individual needs of the resident, while providing actual problem-based experiences in progressive teaching and community hospital environments. PROGRAM DESCRIPTION: A General Residency Program originated in Red Deer in 1982. The residency program features a balanced participation in the principles and practice of clinical, distributive and administrative activities, with an emphasis on patient care. This program offers a variety of rotations primarily based out of the Red Deer Regional Hospital Centre. 2. ROTATIONS/ACTIVITIES/TIME SPENT: Orientation Distribution and Sterile Products Direct Patient Care Precepting Management/ Leadership Information Project Orientation to the organization, department, residency program, clinical program and other initial activities Involvement in all areas of drug preparation, quality control and distribution including interpretation and dispensing of medication orders and provision of drug information. Development of an understanding of the principles of unit dose versus other systems, IV admixtures, chemotherapy and TPN. Technical aspects of aseptic technique and the preparation and administration of parenteral products will be addressed. Development of skills and knowledge in the provision of direct patient care. Through the mentorship of role model clinical pharmacists, the resident provides pharmaceutical care to patients; serves as a clinical resource person; provides drug information; actively participates in multidisciplinary rounds and discussions. Case presentations, journal clubs, in-services and/or short written assignments are expected. Mandatory patient care experiences include cardiology, general medicine, critical care, infectious diseases, ambulatory care and age related (either pediatrics or geriatrics). Achievement of these experiences is completed through direct patient care rotations in various areas including, but not limited to: general medicine, critical care, cardiology, nephrology, palliative care, heart function clinic, emergency medicine, psychiatry (located at CCMHBI), anticoagulation/stroke prevention clinic, pediactrics. Assist in the precepting of undergraduate pharmacy students. Under the supervision of an experienced preceptor, the resident will be involved in the day-to-day activities of these students. Introduction to leadership concepts as they apply to pharmacy. Residents are exposed to daily managerial functions, including planning and developing departmental functions, and attend various meetings during a one week rotation with a manager in Pharmacy Services. A small administration project will be completed. Provide drug information to nurses, pharmacists and physicians. ADDITIONAL COMMENTS: The Red Deer Pharmacy Residency Program is challenging and practice-oriented. The program structure focuses on the individual learning needs of the resident, while providing actual problembased experiences in a dynamic community hospital environment. The majority of rotations will occur at the Red Deer Regional Hospital Centre. However, there is an optional psychiatry rotation which is located at the Centennial Centre for Mental Health and Brain Injury (CCMHBI) in Ponoka, AB. As well, the drug information rotation is only available at either the Calgary or Edmonton Information Centre. Other specialized electives may be available offsite and can be arranged upon request if required to meet the personal learning goals of the resident. 5. APPLICATION PROCEDURES FOR PHARMACY PRACTICE RESIDENCY PROGRAMS IN ALBERTA A. Information to be sent to the Residency Coordinator at the hospital. The following information is to be completed and sent to the hospitals of choice (via email) no later than 1600 MST October 30, 2015: 1. Application form. 2. Personal resumé (include work experience, volunteer activities, professional membership and participation in professional organizations). 3. Written statement (as described below). 4. An original transcript from the university (for pharmacy degree university training only). should be sealed and sent by mail directly from the university to the Residency Coordinator. (must be received no later than October 30) 5. Three references utilizing the Confidential Evaluation Form. Email this form to the persons providing the reference (evaluators). The referee shall email the completed form to the coordinators directly at all Alberta sites you are applying to. ALL three references should be from professional, pharmacy, or academic personnel; or those who can support your potential for a residency position. Carefully choose the people who you ask to support your application through the reference form. Encourage them to submit their forms well in advance of the application deadline. B.
