1.0 Introduction The following details set out OMC s partners experience and contains extracts from OMC documented procedures to undertake surveying, sampling and assessment of ACM s. 1.1 OMC is a limited liability company and is independent of any other company, (OMC certificate of Incorporation N 7364856 and VAT registration N 138 0145 36). 1.2 OMC is a UKAS accredited laboratory and is accredited to ISO 17020 and ISO 17025 for testing and Site Inspection. In addition, OMC hold SafeContractor accreditation, Construction Line and CHAS accredited. 1.3 OMC s Directors have been providing independent safety and environmental management solutions for a wide range of industries and have in excess of 47 years combined experience in Asbestos Surveying, testing and Asbestos Management. 1.4 OMC s senior partners, have diverse experience of working alongside Clients in all industries and markets. OMC prides itself on delivering a totally comprehensive solution. All procedures are based on those within the OMC Quality Manual and Technical Training Procedures. The Manual is part of the company's comprehensive quality control system. Copy of OMC s Quality Manual and Technical Training Procedures shall be made available upon request. 1.5 OMC business activities are in Asbestos Surveying, Analytical Monitoring & 4-Stage Clearance Testing, Asbestos Project Management and providing advice. 1.6 It is the policy of OMC to employ technical staff on a direct basis and as employees of the company. 1.7 OMC carries Employers, Public and Professional Indemnity insurance as detailed below: Employers Liability 10,000,000.00 Public Liability 5,000,000.00 Professional Indemnity 5,000,000.00 (See appendix I) 1.8 OMC is not associated with any asbestos removal contractors but are independent specialists in Asbestos Survey, Project Supervision and Expert Witness services. 1.9 All OMC staff employed to undertake Asbestos Surveys and Reporting are experienced in the disciplines of surveying and materials sampling. Survey staff holds either the BOHS Certificate of Competence in Asbestos S301 and/or BOHS Proficiency Certificate in Building Surveys and Bulk Sampling of Asbestos P402. 1.10 OMC Directors have previously had experience in the compilation of detailed asbestos abatement and removal specifications including full Project Management ranging in value from 500 to 1 Million. 1.11 OMC are participants in the Regular Inter-laboratory Counting Exchange Scheme (RICE). 1.12 OMC are participants in the AIMs (asbestos in materials proficiency scheme). 4.0 Applied Standards Survey Requirements Independent and impartial asbestos consultancy as required by HSG264 OMC Qualification OMC is a privately owned limited company with in excess of 47 years experience in Asbestos Surveying, testing and Asbestos Management. OMC is not associated with any other laboratory, removal contractors or their trade associations. OMC partner s experience, knowledge and reputations in the marketplace are as leading consultants for the provision of Independent specialist asbestos surveying, project management, and In providing expert witness services. Management and Refurbishment and Demolition Surveys OMC are a UKAS accredited laboratory for the following aspects of its surveying in accordance with HSG 264 and documented Quality Manual and in-house Technical Procedures. - Management Surveys - Refurbishment & Demolition Surveys - Material Assessments - Risk Assessments - Re-Inspections Sampling ACMS in accordance with HSG 264, HSG 248 and CAR by a competent person in order to prevent the potential exposure to asbestos and prevent cross contamination during sampling due to incorrect techniques OMC Surveyors are trained in the correct sampling procedures under HSG 248 SLS 005 Issue 2, Dated October 2012 Page 10 of 12

Survey Requirements OMC Qualification Training for Surveyors, HSG 264 All OMC Surveyors and analysts are qualified and trained by both internal and external measures to meet the following requirements: Sampling ACMs Management and Refurbishment and Demolition surveys S301/Oral: Asbestos and other fibres P402: Buildings Surveys and Bulk Sampling for asbestos HSG 248 Asbestos in bulk materials Health & safety, HSG 264 Risk Assessments OMC are able to compile suitable and sufficient risk assessments ahead of site work. Through provision of in-house Health & Safety training, provided to OMC Asbestos Surveyors, amendments to Risk Assessments can be made locally where additional hazards are identified during site investigations. Carry out surveys in accordance with recommended guidelines OMC Quality Manual and Technical procedures for surveying are in accordance with: HSG 264 HSG 248 OMC Quality Manual and Technical Procedures SLS 005 Issue 2, Dated October 2012 Page 11 of 12

Survey Requirements Bulk Sampling Strategy OMC Qualification OMC bulk sampling strategy has been adopted from HSG 264 and the Department of the Environment, Trade and Regions (DETR) "Asbestos and MMMF in buildings". Quality Assurance Procedures, HSG 264 The following OMC surveying and testing activities are to be found in the OMC Quality Manual and Technical Procedures: - training - surveying - bulk sampling of ACMs Air monitoring and bulk analysis of samples by UKAS accredited Laboratory OMC in compliance with CAR 2012, are UKAS accredited for carrying out bulk and air sample analysis and 4-Stage clearance testing. Assessment and Management, HSG 264 OMC partners have in excess of 47 years combined experience in compiling safe management systems for retained asbestos containing materials. Presentation of Results, HSG 264 and Reporting Requirements OMC can provide both written and an electronic Asbestos Management Database to record all specified requirements of the survey, and future reinspection details. All subsequent updates to the database will automatically save and archive historical data for any future reference.
