1 DESCRIPTION OF PERSONAL AUTO COVERAGE The following is a general description of the coverages provided by the Personal Auto coverage form. The policy deductible applies to coverages and endorsements unless otherwise noted. The policy and endorsements should be consulted for specific contract conditions. Liability Coverage - Bodily Injury & Property Damage Liability Bodily Injury coverage and Property Damage coverage will be written as combined coverage (Liability). Neither may be written separately. If Liability is written for one vehicle covered by the policy and Liability is requested on the remaining vehicles, identical limits must be written for all vehicles on the policy. Medical Payments Coverage Medical Payments coverage may not be added to a policy unless Liability coverage is also provided for an auto. If Medical Payments coverage is written for one vehicle covered by the policy, the coverage must be written with identical limits for all vehicles covered by the policy. Uninsured/Underinsured Motorists Coverage If Uninsured Motorists Bodily Injury is written for one vehicle covered by the policy, the coverage must be written for all vehicles covered by the policy. A vehicle that is temporarily withdrawn from service and written with Other than Collision coverage only is not required to have Uninsured Motorists Bodily Injury coverage. Uninsured Motorists Property Damage Uninsured Motorists Property Damage coverage may be selected for any covered vehicle insured for Uninsured/Underinsured Motorists Bodily Injury. Uninsured Motorists Property Damage coverage and Collision coverage may be written on the same vehicle if the Collision deductible is greater than $250. The Uninsured Motorists Property Damage deductible is $250. Coverage For Damage To Your Auto (Other Than Collision And Collision Coverages) Other Than Collision or Collision coverages are optional and must be selected for each auto in order for an auto to be covered. Deductibles are not required to match between autos insured for these coverages. Collision coverage will not be written without Other Than Collision coverage. D-1 (OH)

2 AAMVA UNIFORM FINANCIAL RESPONSIBILITY FORM Form SR-22 and SR-26 applies. A financial responsibility certificate (SR-22) is filed whenever law requires evidence of financial responsibility. If the person requiring the certificate owns no auto, the filing may be made on a policy issued to a family member provided they reside in the same household. An SR-26 will be filed when the policy cancels. The filing fee is fully earned. RENTAL REIMBURSEMENT/TRANSPORTATION EXPENSE No endorsement required. Rental Reimbursement/Transportation Expense coverage is available, for an additional premium, when an auto is insured for Liability, Other Than Collision and Collision coverages. This coverage replaces the Transportation Expense provision in the policy. Available limits are: $30 per day/$900 per occurrence; $45 per day/$1,350 per occurrence; or $60 per day/$1,800 per occurrence. The selected limit per day is subject to the per occurrence total for rental expenses resulting from Other Than Collision or Collision losses. This coverage can be selected for any vehicle on the policy. The limit selected must be the same for all vehicles. An auto temporarily withdrawn from service may be written with Other Than Collision coverage only. However, at least one vehicle on the policy must have Liability, Other Than Collision and Collision coverage. TOWING AND LABOR COSTS No endorsement required. Towing and Labor Costs coverage may be included, for an additional premium, on a personal auto or miscellaneous vehicle insured for Liability (does not have to be included for a trailer), Other Than Collision and Collision coverages. We will pay the fair cost incurred in connection with a road emergency involving the auto. (continued) D-2 (OH)

3 TOWING AND LABOR COSTS (continued) This includes: Mechanical labor at the place of breakdown; Towing to the nearest place where necessary repairs can be made during regular business hours if it will not run; Towing vehicle out of the way, if stuck on or immediately next to a public highway; and Delivery of gas, oil, loaned battery or change of tire. OPTIONAL ADDITIONAL INSURED Endorsement A 75 applies to Personal Auto and Miscellaneous Vehicles. The Additional Insured endorsement adds the individual or organization named in the endorsement to the definition of insured with respect to Liability coverage. ADDITIONAL INSURED LESSOR Endorsement A 32A applies to Personal Auto and Miscellaneous Vehicles. The Additional Insured Lessor endorsement adds a designated person or company, who is leasing an auto to the insured, as an additional interest. This endorsement requires that the notice of cancellation also be mailed to the lessor. The endorsement applies to autos leased for a continuous period of at least six (6) months under a written agreement. ADDITIONAL NAMED INSURED Endorsement A 76 applies to Personal Autos. The Additional Named Insured endorsement adds a named child as an additional named insured under the auto coverage. This endorsement may only be used for an operator who is a: Youthful (ages 16-21), unmarried operator; Child of the named insured (guardian) and/or spouse; Resident of the same household as the named insured (guardian) and/or spouse; and Titleholder of a covered Personal Auto. 4 AGREED VALUE COVERAGE Endorsement A 165 applies to Antique, Classic or Special Interest Autos. This endorsement provides that in event of a loss to your covered antique, classic or special interest auto, the limit of liability is: The amount necessary to repair or replace the auto with like kind and quality provided such amount is equal to or less than the limit of liability shown on the Declarations Page; or The Agreed Value shown on the Declarations Page. Requirements are as follows: Agreed Value must be established through a current appraisal; Antique, Classic or Special Interest Auto Supplemental Application must be completed; Other Than Collision is required on the vehicle; Vehicle must be in good condition; Vehicle must be primarily used as a hobby vehicle; Grange must have supporting Home and Auto business; Operator must be at least 30 years old and have no more than two chargeable violations; Four color photos of the vehicle (front, rear view, engine compartment, interior); and Prior underwriting approval required on any vehicle with an Agreed Value greater than $30,000. All documents must be maintained in agent s office. Any additional information (receipts, bill of sale, etc.) that may be useful in the settlement of a claim should also be kept on file. ANTIQUE, CLASSIC OR SPECIAL INTEREST RESTRICTED USE Endorsement A 164 applies to Antique, Classic or Special Interest Autos. This endorsement allows for a reduced premium if the vehicle is principally used for exhibitions, club activities, parades or other functions of public interest. The restricted use rates charged for the vehicle are lower than the unrestricted use rates. CANCELLATION PRIVILEGE Endorsement A 163 applies. The Cancellation Privilege endorsement provides a notice, to a specific party, when the policy has cancelled. It does not extend any coverage provided by the policy to that party. D-4 (OH)

5 COVERAGE FOR DAMAGE TO YOUR AUTO Endorsement A 160 must apply to Antique, Classic or Special Interest Autos. This endorsement adds, to the Part D Limit of Liability provision in the policy, the Stated Amount shown in the Declarations Page for the vehicle insured. The Stated Amount must be established through current appraisal or other acceptable form of value estimate at the time the policy was originally written. This information must be maintained in the agent s office. The Antique, Classic or Special Interest Auto Supplemental Application must be completed for each vehicle and any additional information (receipts, bill of sale, etc.) that may be useful in the settlement of a claim should also be kept on file and maintained in the agent s office. COVERED PROPERTY COVERAGE Endorsement A 159 must be applied to Camping Trailers, Horse Trailers, Travel Trailers and Motor Homes with Other Than Collision coverage. This endorsement provides coverage for awnings, cabanas, or equipment designed to create additional living facilities in all Camping Trailers, Horse Trailers, Travel Trailers and Motor Homes. Other Than Collision is required on the vehicle for this endorsement to apply. The Cost New amount is used with this endorsement. EXCLUSION OF NAMED PERSON(S) AND/OR SPOUSE FROM ALL VEHICLES Endorsement B 137 OH applies to Personal Autos and Miscellaneous Vehicles. The Exclusion of Named Person(s) And/Or Spouse From All Vehicles endorsement excludes coverage for the person(s) and/or spouse named in the endorsement while operating any auto covered by the policy. Use of this endorsement requires all applicable signatures. D-5 (OH)

6 EXTENDED NON-OWNED COVERAGE FOR NAMED INDIVIDUAL Endorsement A 16 applies to Personal Autos. This endorsement extends Liability, Medical Payments and Uninsured/Underinsured Motorists Bodily Injury coverage, if carried, for the designated individual s regular use of non-owned auto. Available to: Individual owners or husband and wife; Co-owners or partner-individuals other than husband and wife; Employees (including Executive Officers); and Relatives of any above named person(s), if resident(s) of the same household. Not available to: Persons operating public or livery conveyances; School bus drivers; Parking lot or service station attendants; and Mechanics or car salesmen. Different rates apply based on whether the coverage is primary or secondary. IDENTITY THEFT COVERAGE Endorsement A 538 applies to Personal Auto. This endorsement provides coverage for expenses incurred as a result of identity theft. Covered expenses include lost wages and incurred costs that may be required by you or a family member to repair the damage caused by the theft or improper use of their credit records. Expenses are covered up to the limit shown on the Declarations Page. Only one endorsement per household is permitted regardless of the number of policies issued or vehicles insured. This endorsement may not be applied to a Personal Auto policy if the HO 538 Identity Theft Coverage endorsement is already included on a Grange Homeowner policy for the named insured. Identity Theft means the unlawful, unauthorized use of the insured person=s identity with the intent to commit activities that are statutorily defined as fraud or theft. A $250 deductible applies to this coverage for each occurrence. D-6 (OH)

7 LOAN/LEASE GAP COVERAGE Endorsement B 152 applies to Personal Auto and Miscellaneous Vehicles. Loan/Lease Gap coverage pays the difference between the unpaid amount due on the new or used loan or lease agreement and the actual cash value of the auto, not to exceed 125%, in the event of a covered total loss of the auto. Coverage does not include: Finance charges and overdue principal outstanding under a finance agreement used solely to purchase the auto and its equipment; Overdue lease payments; Financial penalties imposed because of excessive mileage, use or wear and tear; Security deposits not refunded by the lessor; Extended warranties, Credit Life insurance, Health, Accident or Disability insurance purchased with the loan or lease; or Carry over balances from previous loans or leases. Eligibility Requirements This coverage is available to an auto if: The auto is insured for Other Than Collision and Collision coverages; A lienholder or leasing company is shown for the auto; and The lienholder or leasing company is a financial institution. MISCELLANEOUS VEHICLE TYPE ENDORSEMENT Endorsement A158 must be applied to All Terrain Vehicles, Dune Buggies, Go-carts, Amphibious Vehicles, Hovercrafts, Golf Carts and Motor Homes. This endorsement adds miscellaneous type vehicle to the list of defined terms in the policy and restates policy provisions to include this term. Cost New amount is used for Motor Homes and Stated Amount is used for all other vehicles mentioned above. D-7 (OH)

8 PERSONAL AUTO PLUS ENDORSEMENT Endorsement A 154 OH applies to Personal Auto and Miscellaneous Vehicles. The Personal Auto Plus endorsement provides the insured with the following coverages: Waiver of Deductible Other than Collision, Collision and Uninsured Motorist Property Damage The deductible for a covered loss is waived if the loss is a total, or when the insured is involved in an accident with an identifiable uninsured motorist who was 50% at fault. Waiver of Deductible Other Personal Property The deductible under any other Grange, or an affiliated company, policy is waived for damage to or loss of personal property resulting from an accident (or total theft) involving the auto. Rental Car Loss of Use Provides reimbursement, up to $1000, for rental fees the insured is obligated to pay while a rental auto is out of service due to an insured s covered loss. Tax, Title, and Fees In the event of a total loss to the auto, this coverage provides up to $1,000 reimbursement for expenses incurred for taxes, title, and fees when purchasing a replacement auto. Air Bag Replacement Provides reimbursement for the cost of replacing an air bag if it inflates without the auto being involved in an accident. Lodging Provides reimbursement, up to $40 per day ($200 maximum), for lodging expenses incurred when the auto is disabled due to a Collision loss more than 100 miles away from home. Electronic Equipment and Accessories Provides coverage, up to $300, for loss to radios, tape players, compact disc players, mobile and cellular phones and other similar devices, including accessories to these devices, such as antennas, tapes, compact discs and records while in or upon the insured s vehicle and not permanently installed. The electronic equipment must be equipped to be operated by power from the electrical system of a motorized vehicle. Locksmith Coverage Provides coverage, up to $75, for service of a professional locksmith. (continued) D-8 (OH)

9 PERSONAL AUTO PLUS ENDORSEMENT (continued) Replacement Cost Coverage This coverage applies when a covered auto that is less than 1 year old and insured for Other than Collision and Collision coverage, is involved in a covered total loss. Waiver of Deductible - Damage to Your Covered Auto When multiple covered autos that are insured for Other than Collision or Collision coverage under a Grange Insurance Company policy are damaged by a covered loss resulting from the same occurrence, only one deductible will be applied to all the damaged vehicles. The highest deductible will be applied. Eligibility Requirements This coverage is available when a vehicle is insured for Other Than Collision and Collision coverage. PERSONAL EFFECTS COVERAGE Endorsement A 80 applies to camping and travel trailers, and motor homes. This endorsement extends Other Than Collision and Collision coverage to personal effects carried in a camping trailer, travel trailer or motor home. The only deductible applicable to this coverage is $50 for each loss caused by theft. Refer to the endorsement for items that are considered personal effects and other coverage restrictions. Other Than Collision is required on the vehicle and a minimum of $500 worth of coverage is required for this endorsement to apply. Stated Amount is used with this endorsement. PORTABLE CAMPER BODY PHYSICAL DAMAGE Endorsement A 74 applies to Personal Autos. Other Than Collision or Collision coverage is available for a camper body unit used with a covered auto. The selected coverage(s) and deductible apply separately to the camper body. Truck cap are not considered a portable camper body. Other Than Collision is required on the vehicle to which this unit is attached for this endorsement to apply. Cost New amount is used with this endorsement. D-9 (OH)

10 SNOWMOBILE ENDORSEMENT Endorsement A 161 must be applied to snowmobiles. This endorsement adds snowmobile to the list of defined terms in the policy and restates policy provision to include this term. Stated Amount is used with this endorsement. D-10 (OH)
