1 ACCIDENTAL DEATH OR DISMEMBERMENT Endorsement PPT100 applies to Personal Autos. This optional endorsement provides coverage for your death, dismemberment or loss of sight. If within ninety (90) days from the date of an auto accident, the person insured suffers any of the injuries listed below, we will pay the amount indicated. Loss of life OR loss of both hands, feet or eyes.....the principle sum Loss of one hand and one foot...the principle sum Loss of either hand or foot.. one-half of the principle sum Loss of sight of one eye..one-half of the principle sum Loss of thumb and index finger of same hand one-half of the principle sum The principle sum payable will be increased by 20% if death results while the insured person s seat belt was properly fastened. Available limits are $5,000 or $10,000. ADDITIONAL INSURED Endorsement PPT75 applies to Personal Autos and Miscellaneous Vehicles. Adds the individual or organization named in the endorsement to the definition of insured with respect to Liability coverage. ADDITIONAL INSURED LESSOR Endorsement PPT32A applies to Personal Autos and Miscellaneous Vehicles. Adds a designated person or company, who is leasing a vehicle to the insured, as an additional interest. This endorsement requires that if the policy is cancelled, notice will be mailed to the lessor. The endorsement applies to vehicles leased for a continuous period of at least six (6) months under a written agreement. ADDITIONAL NAMED INSURED Endorsement PPT76 applies to Personal Autos. This endorsement adds a named child as an additional named insured on the policy. This may only be used for an operator who is: a youthful (ages 16-21), unmarried operator; a child of the named insured (guardian) and/or spouse; a resident of the same household as named insured (guardian) and/or spouse; and a titleholder of a covered Personal Auto. 2 AGREED VALUE COVERAGE Endorsement PPT165 applies to Antique, Classic or Special Interest Autos. This endorsement provides that in the event of a loss to your covered antique, classic or special interest auto, the limit of liability is: the amount necessary to repair or replace the auto with like kind and quality provided such amount is equal to or less than the limit of liability shown on the Declarations Page; or the Agreed Value shown on the Declarations Page. Requirements are as follows: - Agreed Value must be established through a current appraisal; - Antique, Classic or Special Interest Auto Supplemental Application must be completed; - Other Than Collision is required on the vehicle; - Vehicle must be in good condition; - Vehicle must be primarily used as a hobby vehicle; - Grange must have supporting Home AND Auto business; - At least one operator must be at least 30 years old and all operators must individually have no more than two chargeable events; - Four color photos of the vehicle (front corner, rear view, engine compartment, interior) must be retained in the agent s files; and - Prior underwriting approval required on any vehicle with an Agreed Value greater than $20,000. All documents must be maintained in agent s office. Any additional information (receipts, bill of sale, etc.) that may be useful in the settlement of a claim should also be kept on file. ANTIQUE, CLASSIC OR SPECIAL INTEREST RESTRICTED USE Endorsement PPT164 applies to Antique, Classic or Special Interest Autos. This endorsement allows for a reduced premium if the vehicle is principally used for exhibitions, club activities, parades or other functions of public interest. The restricted use rates charged for the vehicle are lower than the unrestricted use rates. BODILY INJURY & PROPERTY DAMAGE LIABILITY No endorsement required. Applies to Personal Autos and Miscellaneous Vehicles. The minimum required limits are $25,000/$50,000 for Bodily Injury and $10,000 for Property Damage. Bodily Injury coverage and Property Damage coverage must be written together. If Liability is written for one vehicle covered by the policy and Liability is requested on the remaining vehicles, identical limits must be written for all vehicles on the policy. D-2 (KY)

3 CANCELLATION PRIVILEGE Endorsement PPT163 applies to Personal Autos and Miscellaneous Vehicles. This endorsement provides a notice, to a specific party, when the policy has cancelled. It does not extend any coverage provided by the policy to that party. COVERAGE FOR DAMAGE TO YOUR AUTO Endorsement PPT160 must apply to Antique, Classic or Special Interest Autos. This endorsement adds, to the Part D - Limit of Liability provision in the policy, the Stated Amount shown in the Declarations Page for the vehicle insured. The Stated Amount must be established through current appraisal or other acceptable form of value estimate at the time the policy was originally written. This information must be maintained in the agent s office. The Antique, Classic or Special Interest Auto Supplemental Application must be completed for each vehicle and any additional information (receipts, bill of sale, etc.) that may be useful in the settlement of a claim should also be kept on file and maintained in the agent s office. Other Than Collision is required on the vehicle for this endorsement to apply. COVERED PROPERTY COVERAGE Endorsement PPT159 must be applied to Camping Trailers, Horse Trailers, Travel Trailers and Motor Homes with Other Than Collision coverage. This endorsement is automatically printed. This endorsement provides coverage for awnings, cabanas, or equipment designed to create additional living facilities in all Camping Trailers, Horse Trailers, Travel Trailers and Motor Homes. Other Than Collision is required on the vehicle for this endorsement to apply. The Cost New amount is used with this endorsement. CUSTOM EQUIPMENT Personal Autos: Custom Equipment coverage is automatically covered up to $1,000 when Other Than Collision or Other Than Collision and Collision is purchased. Additional coverage is available up to $10,000, but prior approval is required for any amount above $5,000. Vehicles with more than $10,000 worth of Custom Equipment are not eligible (beyond the $1,000 in the base policy) If Custom Equipment replaces the equipment originally installed in the vehicle, no credit will be given for that original equipment. A Vehicle Inspection Report form must be completed and maintained on file in the agent s office on all vehicles with Custom Equipment coverage. In order to further protect your customer, any receipts and/or pictures of the custom equipment should be maintained. D-3 (KY)

4 CUSTOM EQUIPMENT (continued) Loss to any equipment designed for racing, which is installed in or upon your covered auto or nonowned auto is not covered. This includes, but is not limited to, nitrous oxide systems, roll cages, or air intake modifications. Miscellaneous Vehicles: Custom Equipment rating for Miscellaneous Vehicles should be included in either the Stated Amount, Cost New amount or Agreed Value (whichever is required for that respective vehicle). Include the value of all custom equipment when determining this single amount. The $1,000 limit for custom equipment does not apply to Miscellaneous Vehicles. If Custom Equipment replaces the equipment originally installed in the vehicle, no credit will be given for that original equipment. EXCLUSION OF NAMED PERSON(S) FROM ALL VEHICLES Endorsement PPT137 applies to Personal Autos and Miscellaneous Vehicles. This endorsement excludes coverage for the person(s) named in the endorsement while operating any vehicle covered by the policy. Use of this endorsement requires prior approval from Underwriting and all applicable signatures. Kentucky law prohibits the exclusion of the named insured s spouse or a dependent. A dependent is defined as any individual claimed as a dependent on the named insured s tax return. Excluded operators will not be considered in the premium calculation. EXTENDED NON-OWNED COVERAGE FOR NAMED INDIVIDUAL Endorsement PPT16 applies to Personal Autos. This endorsement extends Liability, Medical Payments, Uninsured Motorist Bodily Injury and Underinsured Motorists Bodily Injury coverage, if carried, for the designated individual s regular use of a non-owned auto. Available to: Individual owners or husband and wife; Co-owners or partner-individuals other than husband and wife; Employees (including Executive Officers); and Relatives of any above named person(s), if resident(s) of the same household D-4 (KY)

5 EXTENDED NON-OWNED COVERAGE FOR NAMED INDIVIDUAL (continued) Not available to: Persons operating public or livery conveyances; School bus drivers; Parking lot or service station attendants; Mechanics or car salesmen; or A Named Non-Owner Policy Different rates apply based on whether the coverage is primary or secondary. If secondary is selected, verification of primary insurance must be maintained in the agent s office. FINANCIAL RESPONSIBILITY FILINGS Form SR-22 applies to Personal Autos, Motor Homes, Classic Autos, Antique Autos and Special Interest Autos. A financial responsibility certificate (SR-22) is filed electronically whenever law requires evidence of financial responsibility. If the person requiring the certificate owns no auto, the filing may be made on a policy issued to a family member provided they reside in the same household. Operators with an SR-22 filing cannot be excluded and must have a verifiable driving record. All vehicles owned by the operator requiring the filing must be on our policy. An SR-26 will be filed electronically when the policy cancels. Filings will only be provided for Ohio. The SR22 filing may be printed once the application is uploaded. IDENTITY THEFT COVERAGE Endorsement PPT538 applies. This endorsement provides coverage for expenses incurred as a result of identity theft. Covered expenses include lost wages and incurred costs that may be required by you or a family member to repair the damage caused by the theft or improper use of their credit records. Expenses are covered up to $10,000. Only one endorsement per household is permitted regardless of the number of policies issued or vehicles insured. This endorsement may not be applied to a Personal Auto policy if the Identity Theft Coverage, HO-538, is already included on a Grange Homeowner policy for the named insured. Identity theft means unlawful, unauthorized use of your identity or a family member s identity with the intent to commit activities that are statutorily defined as fraud or theft. A $250 deductible applies to each loss. D-5 (KY)

6 INCREASED LESSOR LIABILITY COVERAGE Endorsement PPT101 applies to Personal Autos. Increased Lessor Liability Coverage may be added when an insured wants to purchase lower liability limits to meet his needs but provide 100/300/50 liability limits for the leasing company in order to meet the terms of a standard vehicle lease agreement. This endorsement only increases limits of liability for damages that the leasing company may become obligated to pay. Eligibility Requirements: This coverage is available to personal autos if: the vehicle is insured for Liability limits less than 100/300/50; the vehicle is leased by you for a period of at least six (6) months, as documented by a standard lease agreement with expressly stated insurance coverage requirements; a lienholder and leasing company or leasing company is shown for the vehicle; and the leinholder and leasing company or leasing company is a financial institution. LOAN/LEASE GAP Endorsement PPT152 applies to Personal Autos, All Terrain Vehicles, Dune Buggies, Go-carts, Snowmobiles, Amphibious Vehicles, Hovercrafts, Camping Trailers, Travel Trailers, Utility Trailers, Golf Carts and Motor Homes. Loan/Lease Gap coverage pays the greater of either the difference between the unpaid amount due on the loan or lease agreement less: Finance charges and overdue principal outstanding under a finance agreement used solely to purchase the vehicle and its equipment; Overdue lease payments and any financial penalties associated with those payments; Financial penalties imposed because of excessive mileage, use or wear and tear; Security deposits not refunded by the lessor; Extended warranties, Credit Life insurance, Health, Accident or Disability insurance purchased with the loan or lease; Carry over balances from previous loans or leases; Collection or repossession expenses; or All refunds paid or payable as a result of early termination of lease or loan. and the actual cash value of the vehicle, not to exceed 125%, in the event of a covered total loss of the vehicle. Eligibility Requirements This coverage is available to a vehicle if: the vehicle is insured for Liability, Other Than Collision and Collision coverages; a lienholder or leasing company is shown for the vehicle; and the lienholder or leasing company is a financial institution. 7 MEDICAL PAYMENTS No endorsement required. Applies to Personal Autos and Miscellaneous Vehicles. Medical Payments coverage may not be added to a policy unless Liability coverage is also provided. If Medical Payments coverage is written for one vehicle covered by the policy, the coverage must be written with identical limits for all vehicles covered by the policy. MISCELLANEOUS VEHICLE TYPE ENDORSEMENT Endorsement PPT158 must be applied to All Terrain Vehicles, Dune Buggies, Go-carts, Amphibious Vehicles, Hovercrafts, Golf Carts and Motor Homes. This endorsement adds miscellaneous type vehicle to the list of defined terms in the policy and restates policy provisions to include this term. Cost New amount is used for Motor Homes and Stated Amount is used for all of the other vehicles mentioned above. NAMED NON-OWNER COVERAGE FOR NAMED INDIVIDUAL Endorsement PPT01A applies. Coverage only applies to named insured. There is no coverage for the named insured s spouse or any other person. Permissive use is not covered and this only applies to Personal Autos. Disclosed Operator discount does not apply. This coverage may be written when an insured needs coverage for the personal operation of a vehicle not owned by the insured or a person living in the same household as the insured. This endorsement provides Bodily Injury, Property Damage, Personal Injury Protection and if selected, Medical Payments, Uninsured Motorist Bodily Injury and Underinsured Motorists Bodily Injury. Business Use is ineligible. SR-22 filings are allowed for the named insured. A vehicle can be added to a Named Non-Owner policy and coverage will apply if we are notified within 30 days of the acquisition of the vehicle. OTHER THAN COLLISION & COLLISION No endorsement required. Collision coverage can not be written without Other Than Collision coverage and Other Than Collision may be written without Collision. If Collision is written, then Liability coverage is required. A vehicle, temporarily withdrawn from service, may be written with Other Than Collision coverage only. However, at least one other vehicle on the policy must have at least Liability. Suspending Liability coverage because a vehicle is used seasonally is not allowed. When Other Than Collision or Collision is present on any private passenger, van or pick-up 15 years old, the Vehicle Inspection Report form must be completed and maintained on file in the agent s office. D-7 (KY)

8 PASSPORT Endorsement PPT02 automatically applies to Personal Autos and Miscellaneous Vehicles. This endorsement adjusts the language in the base policy. PERSONAL AUTO PLUS Endorsement PPT154 applies to Personal Autos and Miscellaneous Vehicles. The Personal Auto Plus endorsement provides the insured with the following coverages: Waiver of Deductible Other than Collision & Collision: The deductible for a covered loss is waived if the loss is a total, or when the insured is involved in an accident with identifiable uninsured motorist who was at least 50% at fault. Waiver of Deductible Other Personal Property: The deductible under any other Grange policy is waived for damage to or loss of personal property resulting from an accident (or total theft) involving the vehicle. Rental Car Loss of Use: Provides reimbursement, up to $500, for rental fees the insured is obligated to pay while a rental auto is out of service due to an insured s covered loss. Tax, Title, and Fees: In the event of a total loss to the vehicle, this coverage provides up to $500 reimbursement for expenses incurred for taxes, title, and fees when purchasing a replacement vehicle. Air Bag Replacement: Provides reimbursement for the cost of replacing an air bag if it inflates without the vehicle being involved in an accident. Lodging: Provides reimbursement, up to $20 per day ($100 maximum), for lodging expenses incurred when the vehicle is disabled due to a Collision loss more than 100 miles away from home. Locksmith Coverage: Provides coverage up to $50 for service of a professional locksmith. Waiver of Deductible - Damage to Your Covered Auto: When multiple covered vehicles that are insured for Other than Collision coverage under a Grange policy, are damaged by a covered loss resulting from the same occurrence, only one deductible will be applied to all the damaged vehicles. The highest deductible will be applied. Eligibility Requirements This coverage is available when a vehicle is insured for Liability and Other Than Collision. D-8 (KY)

9 PERSONAL EFFECTS COVERAGE Endorsement PPT80 applies to camping and travel trailers, and motor homes. This endorsement extends Other Than Collision and Collision coverage to personal effects carried in a camping trailer, travel trailer or motor home. The only deductible applicable to this coverage is $250 for each loss caused by theft. Refer to the endorsement for items that are considered personal effects and other coverage restrictions. Other Than Collision is required on the vehicle and a minimum of $500 worth of coverage is required for this endorsement to apply. Stated Amount is used with this endorsement. Coverage greater than $3,000 requires prior underwriting approval. PERSONAL INJURY PROTECTION Endorsement A 178 applies to Personal Autos and Miscellaneous Vehicles. This endorsement provides Personal Injury Protection benefits for medical expense, work loss, replacement services loss, survivors replacement services loss, and funeral expense incurred with respect to bodily injury sustained by an eligible injured person and caused by an accident arising out of the operation, maintenance or use of motor vehicle as a motor vehicle. PIP limits and deductibles in the household must match. If an individual rejects PIP, the Other Than No Fault rate will apply. They will have the option of purchasing Personal Injury Protection and Added Personal Injury Protection on a buy back basis, available upon request to basic reparation insureds that have rejected the limitation on their tort rights in accordance with the Kentucky No-Fault Law. The Guest PIP rate will be applied to any policy where the individual rejected Personal Injury Protection. PERSONAL INJURY PROTECTION (ADDITIONAL) Endorsement PPT 179 applies to Personal Autos and Miscellaneous Vehicles. This endorsement amends the Basic Personal Protection Coverage as shown in the Declarations Page, but only with respect to any amount payable because of bodily injury to an eligible injured person who is a named insured or relative. 10 PORTABLE CAMPER BODY PHYSICAL DAMAGE Endorsement PPT74 applies to Personal Autos. Other Than Collision or Collision coverage is available for a camper body unit used with a covered auto. The selected coverage(s) and deductible apply separately to the camper body. Truck caps are not considered a portable camper body. Other Than Collision is required on the vehicle, to which this unit is attached, for this endorsement to apply. Cost New amount is used with this endorsement. Coverage greater than $5,000 requires prior underwriting approval. RENTAL REIMBURSEMENT/TRANSPORTATION EXPENSES No endorsement required. This coverage is available when a Personal Auto, antique auto, classic auto, special interest auto or motor home is insured for Liability, Other Than Collision and Collision coverages for an additional premium. This coverage also replaces the Transportation Expense provision in the policy. Available limits are: $30 per day/$900 per occurrence; $60 per day/$1,800 per occurrence; or $100 per day/$3,000 per occurrence The selected limit per day is subject to the per occurrence total for rental expenses resulting from Other Than Collision or Collision losses. Only one limit per policy is permitted but may be selected for certain vehicles. RESIDENT INDIVIDUALS AS NAMED INSUREDS SUPPLEMENTAL APPLICATION This application is available when two individuals, who do not have a husband/wife or parent (guardian)/child relationship, both want to be listed as named insureds on the same policy. To be eligible, the following requirements must be met: 1. both individuals must be listed as named insureds on the policies; 2. at least one named insured must be at least 25 years old; 3. the applicants intend and commit to maintain the joint household on a long-term basis (at least one year); and 4. both individuals sign and date the application. 11 SNOWMOBILE ENDORSEMENT Endorsement PPT161 must be applied to snowmobiles. This endorsement adds snowmobile to the list of defined terms in the policy and restates policy provisions to include this term. Stated Amount is used with this endorsement. TOWING AND LABOR COSTS No endorsement required. This coverage may be included for an additional premium on a Personal Auto or Miscellaneous Vehicle insured for Liability, Other Than Collision and Collision coverages. We will pay up to $75 incurred in connection with a road emergency involving the vehicle. This includes: Mechanical labor at the place of breakdown; Towing to the nearest place where necessary repairs can be made during regular business hours if it will not run; Towing vehicle out of the way, if stuck on or immediately next to a public highway; and Delivery of gas, oil, loaned battery or change of tire. We do not pay for the cost of these items. There is a limit of two occurrences per policy term. TRAILERS No endorsement required. If you have Liability coverage on a Personal Auto or Miscellaneous Vehicle, liability will automatically extend to trailers you use. Physical Damage is available for trailers, but at least one other Personal Auto, Motor Home, Classic Auto, Antique or Special Interest Auto must be listed on the policy with Liability coverage. Cost New is used for trailers. UNDERINSURED MOTORISTS BODILY INJURY No endorsement required. Underinsured Motorists Bodily Injury coverage may be included on any policy providing Uninsured Motorists Coverage. The limits for this coverage must match the Uninsured Motorist limits. The premium applies per policy regardless of how many vehicles are insured. D-11 (KY)

12 UNINSURED MOTORISTS BODILY INJURY No endorsement required. Uninsured Motorist Bodily Injury coverage must be included on any policy providing Bodily Injury Liability Coverage. The limits selected for Uninsured Motorist Bodily Injury coverage may not exceed those for Bodily Injury Liability. The limits selected may be lower than the Bodily Injury Liability limits, but not lower than the limits required under the Kentucky Financial Responsibility Law. Selection/Rejection of Uninsured Motorists Coverage The named insured and the named insured s spouse must sign a Kentucky Supplement To Application (Personal Lines) form when Uninsured Motorists Coverage: is rejected; or limits selected are lower than the Liability limits of the policy. Exception: This form does not need to be completed for full rejection of Uninsured Motorists Coverage as long as the rejection confirmation on the GAINWeb application is properly rejected. The premium applies per policy regardless of how many vehicles are insured. D-12 (KY)
