The OPTIMUM EHR also includes extensive clinical decision support for drug-drug, drugallergy and drug-formulary services. The OPTIMUM EHR also includes bedside medication verification and electronic medication administration record, as well as electronic prescribing. OPTIMUM's Interoperability solutions also provides the ability to generate, export, as well as incorporate CCDAs for Transitions of Care, and supports Public Health Reporting for Surveillance, Immunizations and Laboratory requirements. In addition, the interoperability solutions also support reporting to Cancer Case Registries as well as incorporating laboratory results. With OPTIMUM's Data Repository, built-in logic for calculating numerators and denominators and generating patient lists that can be created and sorted by variable attributes. Data Repository also supports clinical quality measures based on data from the EHR and can be exported in QRDA I and QRDA III formats. OPTIMUM's Patient Portal enables consumers, (patients) and their approved family to view download and transmit their Care Summary, and give the consumers the ability to securely message their physician. Only customers who request application modifications may incur additional costs to support product customization. Organizations must sign a contract when purchasing our product. The agreement is a perpetual license which, once signed, is effective until they wish to cancel with a 60 day notice. Technical or Practical Limitations We do not provide hardware and this is required to be purchased from a third party. We do certify specific SAN s that run in our environment so organizations must choose from specific third party vendors in that regard.

Formulary Service Vendor criteria: 170.314.a.2, 170.314.a.6, 170.314.a.7, 170.314.a.8, 170.314.a.10 Provides content for drug databases, clinical decision support, drug interactions, and monograph information. FDB (First Databank) FDB supplies OPTIMUM customers with the dictionaries which define drug interactions and monograph information required for using OPTIMUM's Pharmacy solution. FDB's drug monographs, available at the point of medication administration, offer valuable information about medications, facilitating clinician learning, and patient education. Medication orders are checked against the patient's' active medication profile in OPTIMUM, using the FDB drug knowledge database. Individual contract and associated fees - such as a subscriptionbased payment schedule, which are determined by the Formulary Service Vendor. Formulary Service Vendor contracts with organizations individually. Monthly batch update must be maintained to keep the formulary content up to date. DrFirst to support e- Prescribing criteria: 170.314.b.3] Patient Discharge Instructions Content Provider criteria: 170.314.a.15] DrFirst OPTIMUM's e-prescribing functionality plays a critical role in our customers' medication management process by providing a single point of connectivity for a secure exchange of patient information between a wide range of providers, payers, pharmacy benefit managers, and pharmacies. Patient discharge instructions provide organizations with evidence-based patient education information and patient discharge instructions to ensure ready access to evidencebased, medically accurate information. ExitCare ExitCare--an Elsevier Company--is an integrated, enterprise-wide provider of print, video, and interactive solutions for patient education and discharge instructions. With thousands of health topics organized by relevant diagnosis and sourced with medication information, the ExitCare product suite is intuitively designed for all care settings and includes multiple languages and various health literacy levels to maximize workflow efficiency, reduce hospital readmissions, and increase patient understanding of their conditions. implementation service and license fee applies. Recurring monthly maintenance fees apply based upon the number of subscribing providers. Contract and any associated fees are determined by the Content Provider. Typically is a subscriptionbased payment schedule. DrFirst contracts with organizations individually. Must execute agreement with one of the content providers. Contract with ExitCare individually. Internet connectivity is required to be able to submit prescriptions as well as middleware server requirement. Additional hardware for the content is required to support integration with OPTIMUM software. Quarterly batch update must be maintained to keep the formulary content up to date.

IBM DB2 relational database for patient data storage, IBM WebSphere for processing of Web Services calls to the RDBMS, and Optimum Clinicals Java software which supplies the business logic to extract, format, display, and report on the relevant information. criteria: 170.314.a.14, 170.314.c.1, 170.314.c.2, 170.314.c.3, 170.314.g.1 and g.2 Supports the automated numerator and denominator calculations, Patient Lists, and Clinical Quality Measures Organization must purchase IBM DB2 and IBM WebSphere licenses which are from a thirdparty vendor (i.e., IBM) OPTIMUM Clinical software licenses are purchased from NTT DATA. Licensing costs depend on the size of facility and number of users. Organization is responsible for the continual optimization, maintenance, and general administration of the DB2 and WebSphere environments in coordination with NTT DATA recommendations. Optimum Clinical software including updates are distributed by NTT DATA. Organization is responsible for the implementation of OPTIMUMprovided Meaningful Use reports. PACS System for Image Results criteria: 170.314.a.12] A PACS solution is needed to view image results within the EHR. Individual contract with PACS solution provider and associated OPTIMUM interface fees apply. Additional hardware for PACS is required. Encoder Interface criteria: 170.314.b.2] Coded procedures for CCDA Individual contract with Encoder solutions provider and OPTIMUM associated interface fees apply.

Transition of Care criteria: 170.314.b.1, 170.314.b.2] Enables OPTIMUM to send and receive Care Summaries for Transitions of Care through a Direct connection. Enables system users to upload data from the Care Summary into the OPTIMUM solution. The system uses a third party HISP, UpDox. The platform is designed to incorporate other HISPs into the product. Direct Messaging requires a site to create a SubDomain. All fees are included in the purchase of the system. No Limitations. Transmission of Electronic Laboratory Tests and Values/Results to Ambulatory Providers criteria: 170.314.b.6] Allows the transmission of Laboratory results to other vendor ambulatory EHRs. The OPTIMUM Outbound Laboratory Interface requires a license, a one-time implementation with recurring monthly maintenance fees apply. Monthly maintenance fees are flat fees. limitations from OPTIMUM. View, Download, and Transmit to 3rd Party criteria: 170.314.e.1] The OPTIMUM EHR with Patient Portal provides patients timely access to view, download and transmit their health information as well as send messages to their physician via Direct. If a Customer chooses a different Patient Portal, additional fees. Client will need Excel for (e1) reporting.. No
