WELCOME PROGRAM OBJECTIVES Dear Student: Welcome to the Lake Charles Memorial Hospital Phlebotomy Training Program. Our organization specializes in training for phlebotomy with a strong focus on the clinical aspects of the profession. We are committed to helping students develop to their fullest potential in the phlebotomy field. Our desire is to equip all students with the necessary skills to excel as a phlebotomist and be competent professionals in the healthcare field. 1. Students will learn to utilize necessary scientific principles and to select appropriate phlebotomy actions. 2. Students will be able to identify crisis situations in phlebotomy and apply appropriate skills in their control. 3. Students will gain an understanding of the role a phlebotomist should play in a medical team and how to safely perform their duties. Lake Charles Memorial Hospital has a reputation for quality instruction in many healthcare fields. We aim to assist students in the development of good judgment, effective interpersonal skills and technical skills, which will enable students to function competently as a phlebotomist. Once you have completed this program, you will have new and enhanced skills that will enable you to join the workforce as a highly competent and skilled phlebotomist. CERTIFICATION Upon successful completion of the Lake Charles Memorial Hospital Phlebotomy Training Program, students will receive a Certificate of Completion as a Certified Phlebotomy Technician. Again, welcome to the Lake Charles Memorial Hospital Phlebotomy Training Program! Sincerely, Lake Charles Memorial Hospital is not affiliated with a National Certification Registry. Students completing the program are eligible to sit for a National Certification Registry Exam after one year of employment as a phlebotomist. There are several national certification agencies for phlebotomists. While national certification is not required as a condition of employment at Lake Charles Memorial Hospital, other employers may require this. Julie A. The Lake Charles Memorial Hospital Phlebotomy Training Program operates in compliance with these laws. 2 3

ADMISSION REQUIREMENTS CLASSROOM AND CLINICAL SCHEDULE 1. Applicants for admission to the Lake Charles Memorial Hospital Phlebotomy Training Program must have either a high school diploma or G.E.D. (General Equivalency Diploma). 2. Applicants must also meet the employee eligibility requirements set forth by Lake Charles Memorial Hospital Human Resources. These requirements include, but are not limited to, a background check and a drug screen. 3. Applicants must complete an Application for Admission which includes a personal statement. The Lake Charles Memorial Hospital Phlebotomy Training Program is an 8-week program with a strong focus on practical experience. Most of the students time will be spent in the lab with patient subjects so the student develops real life experience and confidence in their phlebotomy skills. The classroom and clinical schedule is as follows: 1. Clinical 6am 12pm (5 days per week) 220 hours 2. Classroom 12pm - 2pm (2 days per week) 32 hours (Tuesday & Wednesday) Classroom Location: Pathology Lab Conference Room 4. All applicants for admission must have a personal interview with the program director before the scheduled class start date. All other requirements must be completed and on file by the scheduled start date. ATTENDANCE Regular attendance and punctuality are an integral part of the performance criteria in healthcare professions and are therefore required throughout this training program. To successfully complete the program, a student must meet the following attendance guidelines: 1. Tardy Maximum 2 days allowed. 2. Absenteeism a. Practical 95% attendance required. Maximum 2 days allowed. b. Classroom 95% attendance required. Maximum 1 day allowed. Work must be made up. 3. Leave of Absence greater than 3 days must apply for next class. Advanced standing may be granted at the discretion of the program director. Class cancellation - Classes may be cancelled due to inclement weather or dangerous traveling conditions. Cancelled classes will be rescheduled at the discretion of the program director. The Lake Charles Memorial Hospital Phlebotomy Training Program will observe all holidays recognized by the hospital. GRADING SCALE The following grades are used in the calculation of the grade-point average (GPA): Grading Scale Grade Quality Points A 90-100 4 B 80-89 3 C 70-79 2 D 60-69 1 F 59- below 0 4 The following grade designations are not used in the calculation of the final grade. W Withdrawal student voluntarily withdraws prior to the 4 th week of class. I Incomplete assigned if a student has satisfactorily completed most, but not all, course requirements but cannot finish on time due to extenuating circumstances. Any incomplete grade must be made up within two weeks or the I grade is changed to F. 5

STUDENT EVALUATION TOOLS GRADUATION & CERTIFICATION 1. Quizzes and chapter tests consistent with curriculum objectives. 2. Comprehensive final exam 3. Practical exam after completion of clinical training 4. Grade reports a. Mid-term (End of 4th week) b. Final (End of academic portion & end of practicum) All student files are maintained in the program director s office. Students will have access to their files upon request. Grievance procedure 1. Student talk to instructor 2. Student talk to program director FINANCIAL OBLIGATIONS Tuition & fees for student: $800 (Includes textbook and training materials) Payment options: 1. Pay in full upon acceptance 2. $400 payment upon acceptance, $400 payment by midterm (4 weeks) Refund policy No refunds will be given once payment is made and enrollment agreement is signed. UNIFORMS Uniforms are required to be worn daily to clinicals and classroom lectures. Students will have to purchase their own uniform. The selected uniform for students is solid red scrubs and a white lab coat. Students will also be issued a Lake Charles Memorial Hospital name badge, which must be worn at all times. Students will receive a Certificate of Completion from the Lake Charles Memorial Hospital Phlebotomy Training Program upon satisfactory completion of the prescribed courses of study, including theory and clinical experiences. To be eligible to receive the certificate, all of the following must apply: 1. All financial obligations must be satisfied. 2. Cumulative grade of 80% or above. 3. Minimum overall attendance rate of 95%. 4. For clinical, must accumulate a minimum of 300 successful venipunctures and 209 hours. Students must sign statement of understanding eligibility to sit for a national registry upon enrollment. REASONS FOR DISMISSAL A student may be terminated from the program at any point for reasons that include the following, but is not limited to: 1. Failure to maintain a C or 70 in any required course or failure to maintain a cumulative grade of B or 80 in all required courses. 2. Excessive absenteeism (more than three occurrences). 3. Excessive tardiness. 4. Violation of the Lake Charles Memorial Hospital Code of Conduct policy. 5. Cheating on oral, written, or practical examinations. 6. Failure to maintain strict confidentiality of any or all patient records. 7. Unsatisfactory technical performance. 8. Failure to collect specimens exactly as specified. 9. Failure to notify the program director of an absence. All students must adhere to the Lake Charles Memorial Hospital Dress Code Policy. 6 7

READMISSION GUIDELINES CURRICULUM Students who withdraw or are terminated from the Lake Charles Memorial Hospital Phlebotomy Training Program may apply for readmission, which may be granted depending upon the reason for withdrawal or termination. If a student is accepted for readmission, he or she may be granted Advanced Standing under the following conditions: 1. The student must have completed all previous subject areas with a grade of no less than C or 70. 2. The student must re-apply within one calendar year of the date of withdrawal or termination. 3. The student must not have been terminated for reasons of misconduct. If a student is readmitted without Advanced Standing, he or she must repeat the entire program.
