SCHOOL OPENING CHECKLIST Task/Activity Responsibility Owner Yes No INSTRUCTIONAL MATERIALS & SUPPLIES There is a procedure in place to ensure that school and classroom materials and supplies are inventoried at the closing of each school year. The procedure includes identification, purchase an delivery of materials and supplies needed for the opening of schools. Supplies, textbooks, teacher editions, curriculum guides, and educational software and hardware are available in sufficient quantities in every classroom. SCHEDULING Student schedules are complete and have been reviewed for accuracy by building staff. Teacher schedules and classroom assignments are complete and available for distribution to staff. All staff has been notified of their school assignments. STAFFING All positions have been approved by the Board of Education before the staff is hired and/or assigned. All school level staff (principals, vice/asst. principals, teachers, counselors, nurses, security personnel, etc.) have been hired and meet certification requirements, including those related to NCLB highly qualified teachers. All staff have passed background checks. Appropriate numbers of security staff are allocated to each building based on documented need to ensure student and staff safety. The school district has identified staff at each school to assist with troubleshooting in the event that any problems arise the first week of school opening. 1 of 6

HEALTH & SAFETY District and school emergency procedures have been reviewed with all building administrators by the Chief School Administrator and distributed to building staff. Parents have been informed about district emergency and safety procedures. Food service staff has been hired or contracted, deliveries scheduled and the kitchens and food service areas are clean, operational and meet all safety standards. Required health and safety inspections of all food preparation and service locations and staff training of all food service workers to ensure safety of all food items served by the district have been completed. Confirm staff trained in CPR. Confirm AED Unit (supplies) in place. Ensure that 911 Posters are displayed in designated areas (see HES website). Ensure Student Emergency Cards are completed. Ensure staff is trained to administer medications. Ensure designated health room is adequately stocked with required equipment and supplies. State Mandated Health Screenings conducted for students based on grade level. Obtain new vision screening equipment (Sloan & LEA Charts) Ensure school is in compliance with Core Monitor of State Health Requirements. Ensure Universal Precautions/Bloodborne Pathogens Procedures DVD is reviewed by staff. SCHOOL BUILDINGS & FACILITIES OPERATIONS Classrooms - check for working door lock/handles/knobs, windows repaired and working, lighting, clean, organized and free of clutter, furniture in place, shades functioning, windows on door uncovered, bells working, clocks, keys assigned and ready Lavatories clean and inspected. Fire equipment checked and operational. 2 of 6

Removal of trash and broken furniture, old computers and hazardous materials/chemicals. Pest Control Services performed. Exterior, school yard and fields properly maintained including grass cut, weeds removed, bushes/trees trimmed, cleaned and free of debris, trash and broken glass. Corridors (emergency lighting checked, removal of excess furniture, bulletin boards ready, floor, walls and ceiling free of graffiti and waxed or sealed as needed) Stairwells (obstruction free, clean, lighting in place) Exit/Doors-ensure proper operation of doors Gym/Bleachers/Locker Rooms - All areas clean and repaired. Elevators - All related equipment inspected and clean Trash removal - clean containers in place, recycle bins available Auditorium - lighting checked, exit lights and exit doors, emergency lighting checked, seats clean and repaired, floors cleaned, PA system functioning, check for/identify any unsafe locations or "hiding places", develop on-going safety checks All school construction projects have been inspected and have been issued a certificate of occupancy prior to the opening of school. Annually, the Chief School Administrator ensures that each school building is inspected an is in compliance with the "Annual Facilities Checklist: Health and Safety Evaluation of School Buildings". All safety issues and/or code violations have been corrected and the appropriate certificates have been issued. TRANSPORTATION Transportation schedules, routes and/or bus passes (if applicable) have been finalized, and parents have been notified in writing. Each school has a list of arriving and departing students by bus numbers. 3 of 6

There are procedures in place to address students who have not been issued bus passes. All district employed bus drivers have completed trial runs of each route.
