4 HOW TO REGISTER If you live within our practice area simply complete our registration form andquestionnaire. Print them off and bring them in together with your photo ID (e.g. driving licence, passport) and proof of address (e.g. utility bill). Morning and afternoon surgeries are our busiest moments at the front desk. To make your registration process smooth and easy we would advise to come and register between 11am and 1pm. Once registered your records will automatically come from your previous practice but this may take several weeks. If your previous practice is able to let us have them electronically this process can happen the same day. We believe that with the benefits of a fully equipped surgery to hand we can provide you with a better standard of medical care. If you are unable to get to the surgery for medical reasons (which may be confirmed over the phone by your doctor) please ring before 10:30am to arrange a visit and give as much information as possible, so that the doctor can prioritise his/her rounds and plan a route.
5 REPEAT PRESCRIPTION If both you and your doctor would like you to continue your medication long term, we will issue you with a repeat prescription order form (on the right hand side of your prescription). When you need a further supply, tick the relevant medication on the form and put it in the repeat prescription post box at the practice or send it to us by post or fax 01202 769700. Your new prescription can be collected after two working days. It will be returned by post if a stamped self-addressed envelope is provided. If you have nominated a local pharmacy the prescription will be delivered to them. To avoid errors requests will not be accepted over the telephone. If you send in an amendment to your re-order form, these take longer to deal with as they are processed by a doctor. Allow four working days and please make sure that your re-order form is updated when you next attend your doctor. Please anticipate your next repeat prescription and give us time to process it before coming to collect it otherwise much time is wasted by our staff trying to track it down in the medical centre. SE DAY APPOINTMETNS WITH DUTY TE To provide rapid access to healthcare services we run a same day duty team. This is made up of a doctor and nurse practitioners. If you require a same day appointment for an acute problem you will be directed into this service. TELEPHONE CONSULTATION Call 01202 752550 If you wish to speak to a doctor there is a telephone advice slot available everyday for them to phone you. These can be booked in the morning before 11am and the requested Doctor will phone you at anytime throughout the day. We can only take one contact number and the Doctor will only try once to phone you. If you miss the call the doctor will not try again and you will have to arrange it for another day.

8 Access: The centre has been designed to provide easy access for people in wheelchairs. Patients with other types of disability who may require assistance should contact reception. Car Park Please be aware that you are using a car park at your own risk. Management will not accept responsibility for any damage, accident or loss. The speed limit on a car park is 5mph. Feedback & Suggestions At Westbourne Medical Centre we make every effort to give the best service possible to everyone who attends our practice. However, we re aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. 9 Change of Name, Address, Telephone/Mobile number Please let the practice know if you change your name, address or telephone number so that our records are kept up to date. However, if you move out of the practice area you should register promptly with a new doctor. Results If you do telephone for results, please leave your call until between 11.00 am and 12.30 pm or after 3.00 pm avoiding the busy times. Training Practice Westbourne Medical Centre is committed to ongoing learning and personal development. We are a training practice and often have doctors in training working with us as part of their Vocational Training Scheme. We also have links with to the Southampton University and London Imperial College. You will be asked if you are happy for the medical students to sit in with your doctor, prior to your consultation. SMS text appointment reminders We are now sending SMS text appointment reminder messages to patients who have registered their mobile number with us. If you do not want to receive these messages, you may opt out by getting in touch with us. These reminders have been proved to reduce the number of did not attends elsewhere, thus avoiding some wasted appointments.
10 Patient Rights and Responsibilities: You have a right to expect a high standard of medical care from our practice and we will try at all times to provide the very best care possible within the resources available. In order to assist us in this way we require that you take full responsibility for ensuring that you do not abuse the service. For example, it is your responsibility to ensure that you keep medical appointments and follow the medical advice given. We respect your right to privacy and keep all your health information confidential and secure. This information is only available to those involved in your care. You have a right to know what information we hold about you. Very occasionally a practice/patient relationship breaks down completely. In this situation the patient may choose to register with a different practice. The practice also has the right to remove that patient from their list. This would generally only follow a breakdown in your relationship with your GP we would normally give the patient a specific reason for the removal. Confidentiality Our members of staff are trained in matters concerning patient confidentiality and it is a disciplinary offence to breach this confidentiality. Everyone working for the NHS has a legal duty to keep information about you confidential. You may be receiving care from other people as well as the NHS. So that we can all work together for your benefit we may need to share some Information about you. We only ever use or pass on information about you if people have a genuine need for it in your and everyone s interests. Whenever we can we shall remove details which identify you. Anyone who receives information from us is also under a legal duty to keep it confidential. We are registered under the Data Protection Act.
11 Violent patients Zero tolerance: The NHS operates a zero tolerance policy with regard to violence and abuse and the practice has the right to remove violent patients from the list with immediate effect in order to safeguard practice staff, patients and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person s safety. In this situation we are obliged to notify the patient in writing of their removal from the list and record in the patient s medical records the fact of the removal and circumstances leading to it. The Primary Care Trust is then responsible for providing further medical care for such patients.
