Students who have taken the TEAS exam from another state and/or institution must have all their scores officially transferred to CSN BEFORE their specified application deadline. Dental Hygiene TEAS Testing Policies Applicants who fail to establish the minimum cut-off score in each testing area will be required to re-take the entire exam, prior to the February 1 st application deadline. Students can only be registered for ONE test date at a time. Once the student has taken the test and failed to meet the benchmarks, they may register for the next available test date if space is available. There will be NO combination of scores from separate tests. TEAS exam results are valid for five (5) years. Frequently Asked Questions about the TEAS Exam What is the purpose and format of the test? The Test of Essential Academic Skills (TEAS) is a pre-entry test used to evaluate the academic preparedness of students entering the Dental Hygiene Program. The TEAS consists of a total of 170 questions in four subtest areas: Math, Science, English, and Reading. The Math subtest measures knowledge of whole numbers, metric conversion, fractions and decimals, algebraic equations, percentages and ratio/proportions (45 questions) The Science subtest covers science reasoning, science knowledge, biology, chemistry, anatomy and physiology, basic physical principles, and general science (30 questions) The English subtest measures knowledge of punctuation, grammar, sentence structure, contextual words, and spelling (55 questions) The Reading subtest covers paragraph comprehension, passage comprehension, and inferences/conclusions (40 questions) How long will the TEAS Exam take? The examination session will last approximately 3½ hours. How will I receive my test scores? Test scores will be available immediately following the test. If you are taking the TEAS exam at another institution, please have ATI transfer your scores to College of Southern Nevada, Limited Entry Department BEFORE February 1 st. No. ALL applicants must register by the designated registration cut-off date listed for each test date. (Please refer to the Scheduled Test Dates section of this packet). Can I be admitted to the exam without an Examination Permit? It is NOT possible to take the TEAS Exam without receiving an Examination Permit from the Dental Hygiene Program Staff. (Please refer to the TEAS Exam Registration Instructions section of this packet). What if I want/need to reschedule the date of my examination? Due to the number of dates available for the 2016 selection, CSN cannot allow students to reschedule at this time. What if I miss my scheduled exam date? If you miss your scheduled exam date your exam fee will be forfeited and you must restart the registration process from the beginning. This includes paying the fee and scheduling another exam date. TEAS Exam Registration Instructions Please Note: ONLY CSN Dental Hygiene applicants will be eligible to register and purchase the TEAS Exam through the Dental Hygiene Program. All other students will need to contact the testing center for instructions. 1. All CSN DH applicants MUST send a TEAS Exam Registration Request via email to dentalhygieneteas@csn.edu. Your TEAS Exam Registration Request email MUST include ALL of the following information: Full Name NSHE ID Number Phone Number Preferred Test Date Number of times you have taken the TEAS Exam within the last 5 years Provide a SPECIFIC time & date you are available to complete your registration inperson. The Dental Hygiene Office is located on the West Charleston Campus, Building A, Room 108. Availability: Mon-Wednesday 8am-5pm, Thursday 9am-6pm, Friday 8am-1pm 2. Your registration request will be answered via EMAIL within 72 hours. 3. If spots are available, you will receive (via email) a Dental Hygiene TEAS Exam Registration Form, payment instructions, and appointment confirmation for the time you provided in your request. 4. In order to reserve your test date, you will be REQUIRED to complete the following: Print and complete the Dental Hygiene TEAS Exam Registration Form Follow the payment instructions given and get a receipt of payment from the cashier s office. The test fee is $35.00. ON THE DAY OF YOUR COMFIRMED APPIONTMENT TIME: Deliver your completed Dental Hygiene TEAS Exam Registration Form and payment receipt to the Dental Hygiene Office located on the West Charleston Campus, Building A, Room 108. Test Day Instructions & Information Be sure to bring your Examination Permit, two number 2 pencils, and two forms of identification. One form must be photo identification, such as a driver s license, state issued ID card, military ID, or passport. **You will not be admitted into the examination room without a photo ID and the Examination Permit. Plan to arrive at the testing center at least 30 minutes before the time indicated on your Examination Permit. You will not be admitted into the exam session if you arrive late. Giving, receiving, or exchanging information while the examination is in progress is not permitted under any circumstances. Examinees are not permitted to review study material and/or notes of any kind during the examination. The following items are prohibited from the examination room: any food, drink, candy, or gum books or notes of any kind sunglasses, hats, or any jackets with hoods cell phones, pages, beeper, or PDA headsets, ear plugs, ipods, or any other form of media player CALCULATORS recording or listening devices all watches mechanical pencils, rules, side rules, or compasses purses, backpacks, computer bags, satchels, etc. The examiner has the authority to terminate an exam for any individual who does not adhere to the above rules. SPECIAL TESTING ARRANGEMENTS If a physical handicap or learning disability prevents you from taking the Pre-Admission Examination under standard conditions, contact the Dental Hygiene Program Staff to request special testing arrangements. You must provide documentation of your disability. This documentation must include one of the following: A statement from a representative of your educational institution indicating that you have received special testing accommodation within the past three years. Documentation of an evaluation by a state-licensed professional qualified to diagnose your particular disability. This evaluation must have been made within the past three years, and must include information about the licensing and credentials of the evaluator as well as well as the procedure used to diagnose the disability.
