Online Tier 4 Entry Clearance Form Example Form The online entry clearance form is accessible from: https://www.visa4uk.fco.gov.uk/home/index You will need to create a user name and log in. You can save your application as you go through, however it is good idea to have all information to hand before you complete your application. You must provide all information required from the form. Personal Details and Choosing the Correct Visa Application: You will next need to complete some personal details then choose your visa type. You will need to choose your reason for visit, visa type and visa sub type. For most students the reason for visit will be Study ; the visa type will be PBS Tier 4 Student and the visa subtype will be Tier 4 (General) Student. If you are sponsored by your government or international organisation or you will need to choose the sub type of Tier 4 (General) (Sponsored Student). You will also need to choose the appropriate sub category if you are under 18, applying as a dependant or receiving a scholarship for your studies.
Completing the Tier 4(General) application: The website will then generate the online form. You will need to complete all mandatory fields, marked with a red star, to be able to progress through the form. Passport and Travel Information This is information is fairly straightforward, but must be the same as is shown on your passport.
Personal Details and Travel History: Personal Details: You must ensure your contact details are accurate in case the embassy needs to contact you for further information on your application. Travel History: You MUST ensure that you complete ALL details requested. Failure to include all details particularly in regard to any visas you may have been refused can lead to a ban from the UK for up to 10 years for deception. If you indicate yes above, the following box pops up for you to provide further details. You can add as many sections as you need; do not leave any information out.

Medical Treatment: You must indicate truthfully declare on the next section if you have ever received medical treatment in the UK. It is safer to declare a treatment, even if only for a minor complaint, than to leave the information out.

For example you could explain that a previous refusal was only for failure to have held funds in your account for the required 28 day period and that you were then granted a new visa at a future date.
