All incidentals required in connection with the Work of this Addendum shall be Included in the Scope of Work even if not specially specified. All bidders are responsible to review the Addendum and to acknowledge receipt of this Addendum on the space provided on the Bid Proposal Form. The following items are included in Addendum No. 3 for Mitchell Expansion Phase I Bid Pkg.3 (MEP): I. Bids must be received at the AAPS Administration Building located at 2555 S State Street, Ann Arbor, MI 48108. 2. A bid bond is required, in the amount of 5% of the contractors bid. If the bid amount is less than $50,000.00 a certified check in the amount of 5% of the bid is acceptable. 3. It is reiterated that all bid envelopes must indicate the bid category and must include the AAPS Familial Affidavit and Iran Economic Sanctions Act form. 4. This contractor will layout and mark all concrete slabs in the existing building that need to be sawcut and removed (by others). This contractor will do all excavation and backfill/compaction underneath the removed slab area to complete the work of this bid category and return the area to a condition ready to re-pour the areas(by others). 9. All domestic water piping inside the additions and existing building. 10. All sanitary and storm piping. 11. All water, sanitary, and storm piping shall be extended to 5 outside the building by the bid category where connections are necessary to NEW exterior underground piping. 12. All water, sanitary and storm piping that needs to be connected to existing structures or pipe outside of the building will be performed by this bid category. 13. All rain conductor piping. 14.
